1.Food and dish group diversity on menus of daycare centers provided by Center for Children’s Foodservice Management in Korea: a descriptive study

Objectives:
This study aimed to analyze menu patterns and food group diversity in daycare centers managed by the Center for Children's Foodservice Management in South Korea.
Methods:
Data from 18 Center for Children's Foodservice Management centers across various provinces (excluding Jeju Island) were analyzed. We examined 8,796 meals served in February, May, August, and December 2021, focusing on seasonal lunch and snack menus for children aged 3-5. Foods were categorized into 19 groups for lunch and 21 for snacks. The frequency of food groups and dietary patterns were assessed using the Dietary Diversity Score. Analyses were conducted using Excel 2016 and IBM SPSS Statistics version 28.
Results:
Most lunch menus (89%) included five menu items, with a ratio of grain, meat, and vegetables at 88%. Snack menus typically had one item (57%), with significantly more items in the afternoon compared to the morning (P < 0.001). Regarding snack patterns, 75.2% of morning snacks and 61.1% of afternoon snacks contained only one solid food and drink (P < 0.001). Fruit and milk (22.4%) was the most prevalent pattern in morning snacks, while grain and milk (31%) dominated afternoon snacks (P < 0.001). Only 48% of daycare center menus (all snacks and lunch) included all five food groups (grain, meat, vegetables, fruit, and milk). Notably, only 83% included milk and 57% included fruit.
Conclusions
These findings highlight the need to improve food variety and diversity in the Center for Children’s Foodservice Management-managed daycare center menus.Developing more detailed guidelines for menu structure and food composition is crucial to ensure children receive balanced and diverse nutrition.
2.A Clinical Study of PUVA-Induced Complication in Vitiligo Patients.

BACKGROUND: Oral of topical psoralen photochemotherapy is the most popular arid efficacious treatment, available for repigmentation of vitiliginous patches. However this therapy is often used for prolonged periods and various types of pigmentary changes can be observed and sometimes, may be carcinogenic. The safety of PUVA therapy has been an issue of debate. OBJECTIVE: The purpose of the study was to describe in more detail the complications of PUVA treatment and to investigate a possible relation with multiple factors. PATIENTS AND METHOD: We studied 137 vitiligo patients who received PUVA therapy(using oral 8-MOP) for more than six months in our department between March 1990 and June 1996. RESULT: The results are summarized as follows 1. Among 137 cases of vitiligo, the numbers of male and female patients were 40(29.2%) and 97(70.8%) respectively. The mean age at the start of PUVA was 39 years and the mean duration of treatment was 31 months. There were no patients with skin type I and II. Forty cases had skin type III, 68 skin type IV, 29 skin type V. 2. Acute side effects were nausea(35.8%), pruritus(32:8% ), erythema(30.7%), headache(21.2 %), dizziness(18.8%), burning sensation(16.8%), fatigue(8.8%) and the Koebner phenomenon(1.5 %). These are all reversible and disappear on discontinuation of treatment. 3. PUVA-induced pigmentary changes were PUVA lentigines(39 cases;26.3%), hypopigmented confetti macules(36 cased;26.3%), mottling(31 cases,22.6%), nail pigmentation(8 cases,5.8%) and PUVA keratosis(7cases;5.1%) 4. The mean cumulative UVA dose was 1833J/cm and the mean number of treatments was 171. In the high-dose group and the patients who had the most number of treatments, we observed an increased number of patients with PUVA lentigiens and mottling. However, no relationship was observed between the development of PUVA-induced pigmentary changes and sex or age at the start, of PUVA or in relation to skin type. 5. We did not see any patients with melanoma, nonmelanoma skin cancer or systemic cancer. CONCLUSION: Long term exposure to PUVA significantly increases chronic clinical side effects of PUVA. Therefore carefuI clinical follow-up of patients who receive long term PUVA therapy in necessary. This risk shoud be considered in selection of treatment.

3.Drug Sensitivity and Relapsed Period of Relapsed Pulmonary Tuberculous Patients registered in Some Public Health Offices, in Seoul.

The purpose of this study was examine the general characteristics of relapsed pulmonary tuberculous patients (i.e. age, sex, weight, occupation, previous forms of treatment, drug sensitivity, and the frequency of relapse) in order to improve future treatments of tuberculosis as well as to perpetuate health education. The data was obtained from the medical records of 186 relapsed pulmonary tuberculosis patients who were registered for treatment at various public health officers in Seoul during the year of 1994. The major findings obtained from the study were as follows; l) The male to female ratio of relapsed pulmonary tuberculous patients was about 7:3, more specifically 23.7% of the men and 30.9% of the women were between 20 and 29 years of age. 2) Comparing initial less aggravated states to relapsed states, patients with minimal x-ray findings later proved moderately advanced X-ray findings. Furthermore, patients with negative sputum AEB findings later proved positive sputum AFB findings. 3) of the l86 patients studied, 91.9% suffered, relapse and 8.1% suffered 2 or more relapses. Of the patients who suffered at least 1 relapse, 54.8% received short-term treatment, 26.9% received long-term treatment, and 18.3% received treatment of an unknown during their initial tuberculosis treatment periods. 4) Fifty five point four percent of the patients had no reaction to the drug treatment(not available), 25.9% of the patients had sensitive reaction to the drug treatment, 18.7% of the patients had resistant reaction to the drug treatment. Drug resistance was higher in patients to exhibited positive X-ray findings as well as in patients that exhibited positive sputum findings furthermore, patients receiving treatment of an unknown nature(35.5%) exhibited higher drug resistance than those receiving short-term treatment(13.6%) and long-term treatment(l7.0%). 5) Of the 160 patients who suffered relapses, 8.8% suffered a relapse within 1 year after treatment and 91.2% suffered a relapse at least 1 year after treatment. Furthermore, our study showed that women, under 30, who received short-term treatment and encounterd complications during their primary treatment suffered relapses faster than any other groups studied. In addition, minimal X-ray findings and sputum AFB findings were not correlated to the time relapse occurred. Therefore, the greater efforts are needed to prevent relapsed pulmonary tuberculosis.

4.Correlation of Tumor Angiogenesis and nm23-H1 Expression with Lymph Node Metastasis in Proper Muscle Gastric Cancer.

We studied clinicopathologic features of 44 cases of PM (proper muscle) gastric cancer, correlated the lymph node metastasis and found the result of immunohistochemical staining for tumor angiogenesis using antibodies to Factor VIII-related antigen and nm23-H1, known as meatastasis inhibitory substance. The results were as follows: 1) The average age of these 44 cases of PM gastric cancer was 55.1 years old (range 35-81). The ratio of male to female was 2.2 : 1. The tumor was located at the antrum of stomach in 72.7% of the cases. The average size of the tumor was 4.1 cm (range 0.6-9). The gross features were comprised of Borrmann type I (6.8%), II (29.6%), III (56.8%), IV (6.8%), respectively. The microscopic type was a diffuse type in 70.5% and an intestinal type in 29.5%. There were lymph node metastasis in 25 of the 44 cases (56.8%). 2) The microvessel count was higher in the lymph node positive group (average 69.3) than in the lymph node negative group (average 45.6) (P=0.004). There was a higher microvessel density in diffuse type, over 4 cm of tumor size, proximally located tumor, older than 50 years, Borrmann type II and IV, but there was no statistically significant correlation. 3) The more decreased expression of nm23-H1 was found in the lymph node positive group (56.0%) than in the lymph node negative group (31.6%), but showed no statistical significance (P=0.0142). There was no significant correlation between the expression of nm23-H1 and the other clinicopathologic factors. We suggest that the microvessel count of the tumor angiogenesis may be a prognostic factor for predicting lymph node metastasis and also help to determine the therapeutic modalities of PM gastric cancer.

5.Influences on Biochemical Parameters or Cardiac Structure and Function of Serum Total Homocysteine Concentration in Athletes.

The objective of this study was to compare the difference in serum total homocysteine (tHcy) concentration between controls and athletes, and examine its influence on biochemical parameters or cardiac structure and function of our study participants. Biochemical assay including serum tHcy and folic acid concentration were performed by automatic analyzer, and analyses on cardiac structure and function of study participants carried out by 3D echocardiography, respectively. There were significant positive correlation between serum tHcy and biochemical parameters such as serum creatinine and uric acid concentration in athletes, while there were significant positive correlation between serum tHcy and echocardiographic parameter such as FS (%) value in controls (p..05). In conclusion, our data suggest that the interaction between serum tHcy concentration and clinical parameters in athletes might be at least in part controlled by the difference way with controls. Further studies using larger sample size will be necessary to clarify the precise mechanism behind these interactions.

6.Exceptionally Good Lymphocytic Infiltration with Histiocytes and Multinucleated Giant Cells of Stomach Cancer: A case report.

Stomach of 34 year old man showed an early stage of the cancer with slight involvement of the superficial part of the inner muscle layer and accompanied with one metastatic lymph node. The cancer is that of medium differentiated adenocarcinoma. There is an heavy infiltration of lymphocytes mixed with histiocytic mononuclear cells and multinucleated giant cells. Some giant cells appear as Langhans' type suggesting phagocytic cells of their origin containing PAS positive materials in the cytoplasma. In other places, they appear as atrophic cancer nests suggesting that these tumor nests were arrested and undergone to regressive cellular process because of the over-whelming immunological pressure by the host.

7.Effects of Iron Supplementation on Iron Status and Immunity Status of Elite Female Soccer Players.

This study was performed to evaluate the effect of iron supplement for 4 weeks on iron status, immunity, and antioxidant status of national female soccer players (n = 25). This study was performed at summer hard training period right before competition. A single blind design was used to divide the subjects into iron-supplement (IS) or placebo group (P). Iron-supplement group was supplemented with iron (40 mg/d) for 4 weeks. The mean age of the subjects was 23.3 +/- 2.5 years old. Mean height and body weight of the subjects were 164.4 +/- 5.7 em and 57.4 +/- 4.6 kg, respectively. The mean carrier as soccer player was 11.0 +/- 2.6 years and mean training time was 7.0 +/- 1.3 hr/day. The mean hemoglobin, hematocrit, total iron binding capacity and ferritin concentrations before iron supplementation were not different between two groups. After 4 weeks of summer training and iron supplementation, serum ferritin level was significantly increased only in IS group after supplementation. Mean corpuscular volume and total iron binding capacity were significantly decreased in both groups. Meanwhile, hemoglobin and red blood cell count were significantly lowered only in placebo group. The IgM concentration increased significantly in both groups, but IgG concentration had increasing tendency only in IS group (p<0.064). Therefore, iron supplementation during hard training period may be helpful to improve work capacity of the athletes by improving ferritin status and humoral immune responses.

8.Expression of Green Fluorescent Protein in Both Spodoptera frugiperda Cells and Bombyx mori Larvae by Ac-Bm Hybrid Virus.

We have expressed GFP in Sf9 and Bm5 cells or Bombyx by larvae by using Ac-Bm hybrid virus capable of replicating in both Bm5 and Sf9 cells. Genomic DNA of Ac-Bm hybrid virus expressing P-galactosidase was cotransfected with baculovirus transfer vector containing GFP gene, pBacPAK-GFP in Sf9 cells. The Ac-Bm hybrid virus harboring GFP was named as Ac-Bm hybrid virus-GFP. The Ac-Bm hybrid virus-GFP-infected insect cells were easily selected by detecting the emission of GFP from each well of cell culture dish on the UV illuminator. GFP produced by Ac-Bm hybrid virus-GFP in Sf9 and Bm5 cells or B. mori larvae was confirmed by SDS-PAGE and Western blot analysis using GFP antibody. In addition, B. mori larvae infected with Ac-Bm hybrid virus-GFP was apparently appeared fluorescence from the whole body at 5 days postinoculation. The fluorescence of GFP from the hemolymph and fat body of B. mori larvae infected with Ac-Bm hybrid virus-GFP was also observed by fluorescence microscope. In conclusion, our results demonstrated that in baculovirus expression vector system, use of Ac-Bm hybrid virus have an additional advantage of expanded host range for producing recombinant proteins.

9.Cytologic Findings of Epithelial-Myoepithelial Carcinoma of the Salivary Gland: A Case Report.

The report of aspiration cytologic findings of epithelial-myoepithelial carcinoma(EMC) in the salivary gland is extremely rare. We present a case of fine needle aspiration cytology(FNAC) from EMC in the right submandibular gland of a 46 years old male patient. Neck CT scan revealed a confined lesion in the submandibular gland without enlargement of the regional lymph node. FNAC from the tumor showed several three-dimensional cellular clusters with admixed normal acinar cells. They frequently formed branching tubular structures composed of two type of cells; darker cells having eosinophilic scanty cytoplasm with round dense nuclei and clear cells having abudant pale cytoplasm with vesicular nuclei at the periphery of clusters. The tumor cells of both types did not show pleomorphism or mitoses. The resected submandibular gland showed an ill-defined whitish firm tumor, measuring 2 X1.5X2cm. The histology revealed an infiltrative tumor showing characteristic two cell types in a duct-like arrangement surrounded by thin basement menbrane. An inner layer of darker cells and outer layer of clear cells were postive for cytokeratin in the former and S-100 protein in the latter on the immunohistochemical stain.

10.Neoadjuvant Chemotherapy Decreases the Identification Rate of Sentinel Lymph Node Biopsy.

PURPOSE: We investigated the feasibility and clinical applicability of sentinel lymph node biopsy (SLNB) in patients with neoadjuvant chemotherapy, tumor size > 2 cm, and privious excisional biopsy. METHODS: 175 patients with 176 breast cancer underwent aLNB between October 2001 and October 2002. Among them twenty-five patients with locally advanced breast cancer underwent SLNB after neoadjuvant chemotherapy. Eighty-nine (50.6%) had primary tumor > 2 cm. The recent biopsy method used before SLNB was excision in thirty-one (17.6%) procedures. The identification rate, false-negative rate, negative prediction value and accuracy of SLNB were determined. RESULTS: SLNB was successful in 164 of 176 cases (detection rate, 93.2%). The identification rate of patients who had neoadjuvant chemotherapy was 68% and lower than that of who had not (97.3%), significantly (P=0.00). However, mapping success was not influenced by large tumor size (> 2 cm) or previous excisional biopsy. The false-negative rate and accuracy were 16.5% and 91.5, respectively. The false negative rate was 21.3% (3/14) in those patients received neoadjuvant chemotherapy compared with a 15.5% in those patents not received neoadjuvant chemotherapy (P> 0.05). In addition, Tumor size > 2 cm and previous excision did not adversely impact the false negative rate and accuracy. CONCLUSION: Neoadjuvant chemotherapy was associated with a lower identification rate for SLNB. But if detected, SLNB after neoadjuvant chemotherapy might reliably predict axillary status. SLNB is feasible and accurate in patients with large tumor (> 2 cm) and previous excision.
