1.Clinical Observation of Sporotrichosis during a Twelve Year Period (1967 - 1978).

Observations of 50 sporotrichosis patients, who were confirmed by dinical, histopathological and mycological examination, were made in the department of Dermatology at Chon-nam University Hospital for the twelve year period from 1967 to 1978. The results obtained were as follows; 1. During this period, out of a total of 39, 594 outpatients, there were 50 cases of sporotrichosis for an average of 0. 13 percent. The highest incidence was in 1970 with 7 cases, however, there was no period with a particularly marked increase in the occurrence of the disease. 2. Half of all the cases occurred in the third and fourth decades of life. The male to female ratio was 2. 3:1. 3. Occupationally, the greatest percent were farmers, 36 cases(72%), and the next highest, unemployed. 4. In Kwang-ju an.d surrounding 6 counties, there were 25 cases which were widely scattered thraughout Chonnam province and the southern tip of Chonbuk province. 5. The initial lesion appeared on the upper extremities in 39 cases, particularly the right hand, and the lower extremities in 9 cases. 6. Of the total patients, there were 24 cases(48%) with a history of trauma before the onset of the disease. Of those cases, the disease appeared between I and 2 weeks. after trauma in 10 cases(41.7%) and between 2 and 3 weeks in 8 cases(33.3%). 7. The duration of the skin lesions before initial examination was 2 months in 12 cases(24%) and 3 months in 14 cases(28%). 8. Forty six percent of the total cases visited our department in spring, showing a higher incidence than any other season. The highest monthly incidence was in May, when there were 13 cases. 9, Of the total, 42 cases(84%) were cutaneous lymphatic type, while 8 cases(16%), localized cutaneous type.

2.A Study of Hair Growth in Korean.

The hair growth of the scalp is influenced by many systemic and local factors. So the study of standard hair growth rate is very important for the diagnosis and treatment of some pathological conditions. That study has not been done well in Korea. The present study was to establish the standard hair growth rate of the scalp of healthy Koreans at different ages. The investigations were carried out on 509 individuals of both sexes whose ages ranged between 2 and 69 years. The subjects were grouped according to 5 age periods. The results of this study were as follows: 1. The mean value of the daily scalp hair growth rate was 0.401+ 0.037mm. 2. The daily hair growth rate of each groups were as follows: under 10 years of age; 0.367+0.032mm, in the teenagers; 0.419,+0. 032mm, in the twenties0.398+0.037mm, in the thirties and fourties; 0.394+0.034mm, above 50 years of age 0.386+029mm. 3. Sex differences of each age groups in the scalp hair growth were not statistically significant.

3.The Frequency and Clinical Observation of Ota's Nevus.

The clinical observation was done on 126 patients of Ota's nevus, 113 of whom were seen at Department of Dermatology, Chonnam National University Hospital from January 1973 through July 1983, and 13 of whom were found among 45, 87l male and female students of high school in Kwangju City, Korea. The results obtained were as follows: 1. The incidence of male and female students of Ota's nevus were respectively 0. 013% and 0.043%, with the incidence,as a whole being 0.03%, whereas the frequency of Ota's nevus among the outpatients was 0. 24% with the ratio of male to female being 1: 2. 67. 2. There was only one family of familial occurrence, in which 2 sisters were involved among 5 siblings. 3. Thirty three percent of Ota's nevus showed the onset at birth. 4. According to Tanino's classification, our patients showed that Type II was 41.7% and then Type Ia, III, Ib, and IV being respectively 33. 3%, 16. 7, 4.2, and 4. 2%. 5. Seleral involvement was 54.2%, especially in type III and Type II. 6. The color was increased in 54%, especially Type II by internal and external factors such as fatigue, warm temperature, menstruation and cold temperature. 7. The color intensification was prominent at the age of 10 to 14.

4.SPECT Imaging of Dopamine Transporter with I-123 IPT in Normal Controls and Parkinson's Patients.

OBJECTIVES: Dopamine transporter concentrations have been known to decrease in Parkinson's disease(PD). The aim of the present study was to evaluate the correlation between SPECT measurements of [I-123]N-(3-iodopropene-2-yl)-2beta-carbomethoxy-3beta-(4-chlorophenyl) tropane(IPT) as an imaging agent for measuring changes in transporter concentrations with PD. PATIENTS AND METHODS:IPT labelled with 4.87+/-1.29mCi(180.19+/-47.73 MBq) of [I-123] was intravenously injected into 23 patients(age:58+/-12) with PD and three normal controls(NC)(age:37+/-7) as bolus. Brain SPECT were then performed at 1 hour and 2 hours after injection on a double headed camera. The statistical parameters were the contrast ratio of left basal ganglia(BG) and right basal ganglia to occipital cortex(OCC) per milli curies of injected radiotracer at 1 hour and 2 hours. The correlations were evaluated between these parameters and Hoehn-Yahr classification of the patients. RESULTS: The(BG-OCC)/OCC/mCi ratios at 1 hour and 2 hours for PD and NC were 0.14+/-0.07 and 0.27+/-0.07(1 hour) and 0.12+/-0.07 and 0.34+/-0.04(2 hour), respectively. The(BG-OCC)/OCC/mCi ratios of Parkinson's disease were decreased with higher grade of Hoehn-Yahr classification of the patients. The ratio between BG and OCC for PD were clearly separated from NC and may be useful outcome measures for clinical diagnosis. CONCLUSION: The findings suggest that IPT may be a very useful tracer for early diagnosis and treatment of PD and study of dopamine re-uptake site.

5.Modified Bilhaut-Cloquet Method for Correction of Bifid Thumb.

PURPOSE: As the traditional treatment of the Wassel's type I or II of bifid thumb, Bilhaut-Cloquet, has always been the standard method despite several disadvantages such as tearing of the finger nail, injuries of the growth plate, joint instability, and long visible scarring. To overcome these drawbacks, we applied a modified Bilhaut-Cloquet Method. METHODS: The subjects used for the this study were 10 of 20 patients evaluated. The patients underwent modified methods under every type of Wassel's classification. We designed a central wedge Zig-Zag incision and removed the nail and bony tissues in the remaining digit, but not soft tissue if possible, and transferred the ligaments, tendons, and soft tissue to the remaining thumb from the extra digit. We evaluated the patients' lack of extension, the total ROM of the MP and IP joints, the ROM of IP joints, and the lateral deviations of the reconstructed thumb. RESULTS: The results were encouraging, with all patients showing a good functional and aesthetic outcome. CONCLUSION: The modified method proved a very effective procedure in the treatment of bifid thumb in all types, especially types I or II.

6.Correction of Hook Nail Deformity with Composite Graft.

PURPOSE: Hook nail deformity is caused by inadequately supported nail bed due to loss of distal phalanx or lack of soft tissue, resulting in a claw-like nail form. A composite graft from the foot bencath the nail bed gives adequate restoration of tip pulp. METHODS: From September of 1999 to March of 2004, six patients were treated for hook nail deformity and monitored for long term follow up. Donor sites were the lateral side of the big toe or instep area of the foot. We examined cosmetic appearance and nail hooking and sensory test. The curved nail was measured by the picture of before and after surgery. RESULTS: In all cases, composite grafts were well taken, and hook nail deformities were corrected. The curved nail of the 4 patients after surgery were improved to average 28.7 degrees from average 55.2 degrees before surgery. The static two point discrimination average was 6.5mm and the moving two point discrimination average was 5.8mm in the sensory test. CONCLUSION: Composite graft taken from foot supports the nail bed with the tissue closely resembling the fingertip tissue, making it possible for anatomical and histological rebuilding of fingertip.

7.Thyroid Function Studies in Patients Who Underwent Open Heart Surgery.

PURPOSE: Alteration in the serum level of thyroid hormone can occur following open heart surgery due to major stress and hemodilution after extracorporeal circulation but these changes have been ignored. The purpose of this study is to evaluate the changes of thyroid hormone level after open heart surgery and if these changes influence the prognosis. METHODS: We evaluated 26 children who had undergone open heart surgery from Sept. 1994 to Jun. 1996 at Korea University Hospital. Five ml of blood were collected before surgery, immediately after surgery and 24 hours after surgery. The blood was centrifused and the serum stored at -40degrees C until examinations were made. Serum T3, T4, reverse T3 were measured by sensitive and specific radioimmunoassay. RESULTS: The mean T3 level was 1.49 +/- 0.50ng/ml (normal 0.08-2.0ng/ml) at pre op, 0.75 +/- 0.40ng/ ml immediately after surgery and 0.69 +/- 0.50ng/ml 24 hours after surgery. The mean T4 level was 10.73 +/- 4.40ug/dl (normal 6.1-11.8 microgram/dl) at pre op, 5.80 +/- 1.90ug/dl immediately after surgery and 5.60 +/- 2.10ug/dl 24 hours after surgery. The mean TSH level was 1.69 +/- 1.13uIU/ml (normal 0.25-4.0 uIU/ml) at pre op, 1.37 +/- 0.80uIU/ml immediately after surgery and 1.61 +/- 1.00uIU/ml 24 hours after surgery. Serum T3 and T4 levels in cardiac surgical patients were significantly reduced (P<0.01), and serum reverse T3 levels were significantly increased (P<0.05), and serum TSH levels were not significantly different in comparison with the preoperative state. CONCLUSION: We conclued that patients with congenital heart disease who underwent open heart surgery show euthyroid sick states like other severely ill patients. It is likely that reduction in T3 and T4 without increased TSH represents an adaptive response by the body to minimize catabolism when undergoing major stress.

8.A Case of Mediastinal parathyroid adenoma localized by technetium-99m sestamibi scanning

We present a patient with primary hyperparathyroidism in whom preoperative Tc-99m sestarnibi scanning cleary demonstrated mediastinal parathyroid adenoma. After resectian of tumor through median sternotomy, he was restored to the normocalcemia. This case suggests that Tc-99m sestamibi scanning may be a useful method in the preoperative localization of mediastinal parathyroid adenoma.

9.PCR and RFLP-based CYP2D6(B) and CYP2D6(T) Genotyping for Korean Lung Cancer Cases and Controls.

The genetically determined CYP2D6 activity is considered to be associated with cancer susceptibility with inter-individual variation. Genetic polymorphism of CYP2D6(B) and CYP2D6(T) was determined by the two polymerase chain reaction(PCR) and BstN1 and EcoN1 restriction fragment length polymorphisms(RFLP) for 67 lung cancer cases and 95 healthy volunteer controls. The cases were composed of 26 squamous cell carcinoma, 14 small cell carcinoma, 10 adenocarcinoma, 3 large cell undifferentiated carcinoma, and 14 not histologically diagnosed. The results were gained from the 142 subjects (57 cases and 85 controls) who observed successfully in two PCR and BstN1/EcoN1 RFLP. Only one and no mutant allele of the CYP2D6(B) and CYP2D6(T) gene was detected, that is, the frequency of mutant allele was very low; 0.7%(1/142) and 0%(0/142), respectively. Detected mutant allele of the CYP2D6(B) was heterozygous type(WM). The odds ratios for lung cancer susceptibility with CYP2D6(B) and CYP2D6(T) genotype were not calculated. These results are similar to the previous understanding that the mutant allele is very rare in Orientals compared to Caucasians, therefore, it considered that CYP2D6(B) and CYP2D6(T) genotypes have maybe no association with lung cancer susceptibility in Koreans. This is the basic data of CYP2D6(B) and CYP2D6(T) genotypes for Koreans. It would be hepful for further study to determine lung cancer susceptibility of Koreans with the data about CYP1A1, CYP2E1, GSTM1 from future study.

10.Development of a Chronic Skin Wound Defect: A Swine Model.

PURPOSE: Animal models of a chronic wound are yet to be fully developed, and animal studies on this subject has yet to take place. The purpose of this study is to create the foundation for research on chronic wound healing based on a swine model, the most similar to that of a human. METHODS: Three female 2-3 month old 'yolkshires' were used. Total of eight full thickness skin defects, 6 x 3 cm sized, were created on the back of each pigs. Three groups were created for comparison; Group I (n=4) was left as they were after full skin thickness excision, while the excised tissues of Group II (n=3) were turned inside out and sutured so that the epidermis would come in contact with the fascia. Group III (n=3) were excised full skin thickness in depth and silicone blocks were implanted in them. Dressing was not practised so that the wounds would be vulnerable to infection. RESULTS: In Group III, the skin contraction rate was the least among the three groups for each three weeks of observation respectively. Also during the three weeks, bacteral colonization was at the highest among the comparison. On the third week, inflammatory cells were still active, but the generations of epidermis and collagen synthesis were detected minimally. CONCLUSION: The Group III was relatively the most similar model of chronic wounds. and modification of the silicone blocks, could provide us with a very effective chronic skin wound model similar to human.
