2.Computed tomography in the staging of esophageal carcinoma

CT was found to be highly accurate in predicting tumor size and assessing invasion of the surroundingstructures and distant metastasis. Also CT played an important role for determination of operability of esophagealcarcinoma. The CT findings with barium esophagogram in 21 patients with histologically proven esophageal carciomawere reviewed from Feb. 1985 to Feb. 1986 at the department of Radiology, Yonsei University ,College of Medicine.The results were as follows: 1. Number of patients in each stages were: 2 in stage 1, 6 in stage 2, 4 in stage 3,and 9 in stage 4. 2. Peak age distribution was in its 6th decasedes as 9 patiens (42.9%). Overall mean age was 60.8 years. Number of male patients were 19 and 2 of female. 3. Histologic types of esohageal carcinoma were 19cases of epidermoid (90.5%) and 2 cases of adenocarcinoma(9.5%). 4. The tumor location was 1 case in upper, 14cases(66.7%) in middle and 6 cases in lower one-third. 5. Various types of esophageal carcinoma were as follows: 3cases of fungating, 4 cases of infiltrating, 5 cases of ulcerofungating, and 9 cases of ulceroinfiltrating type.6. Average length of involvement in each stages were 4cm in stage 1, 5.5cm in stage 2, 8.8cm in stage 3, and 8.3cmin stage 4. The involved length was longer in advanced cases. In 11 cases(52.4%), the involved length was between4 and 8cm. 7. Angle of periaortic fat plane obliteration of the aortic circumference were as follows: Below 45degrees( 7 cases 33.3%), 45degrees-90degrees (3 cases 14.3%), over 90 degrees(11 cases, 52.4%). 8. Method oftreatment of esophageal carcinoma were as follows: Only radiotherapy in 11 cases(52.4%), radiotherapy withoperation in 5 cases, only operation in 1 cases, and no treatment in 4 cases. 9. Distant metastatic sites were:brain in 1, pericardium in 5, liver in 5, trachea in 2, bronchus in 9, and distant lymph node in 5 cases.

3.The Clinical Study on Pin and Plaster Method about open Tibial Shaft Fractures

Recently, the rates of various traffic accidents and industrial injuries accordlng to industrial progression were increased very much. In those injuries, open tibial shaft fractures were one of the most common ones of all long bones of the body. The authors have reviewed and analyzed 30 cases of the open tibial shaft fractures which were treated by pin and plaster method at our hospital from April 1976 to August 1979. The results were as follows: 1. Most of the fractures occurred in 20 and 30 age levels, average age was 34.4. Male 5 > female 1 in accident frequency. 2. The causes were traffic accident (67%), direct blow (17%), falling down from a height (13%), and mechanical trauma (3%) in order. 3. In location of fractures, Lt. 16 > Rt. 11 > both. 3; lower (47%), middle (33%), upper (20%), one thirds and comminuted (60%) > transvers (20%) > oblique (13%) > spiral (4%) > segmental fractures in frequency. 4. The associated injuries were fractures of fibulas (67%) > multiple skin damages (40%) > knee joint patella injuries... etc. Total of those injuries were 68.; One patient gained 2.26 kinds of those injuries simultaneously. 5. By classification of Ellis, minor severity (33%) > moderate one (40%) > major one fractures (27%). And according to results of pin and plaster treatment, bony union took roughly 11, 15, above 23 weeks respectively. 6. The summed contaminatlon and infection rate was below 20%, and cultured bacterias were E. Coli (50%) > staphylocci (33%) > pseudomonas (17%) and susceptabilities to penicillin were 60, 90, 25% respectively. 7. By classification of time interval from injury to apply pin and plaster method, early applied group (17 cases) showed bony union rate (7.6.4%), delayed one (11.7%), nonunion (5.8%), late group showed 50, 12.5, 6.3%, far late group showed 40, 40, 20% respectively. 8. Duration of the cast immobilization was 4–7 weeks and then it was changed by P. T. B. cast until about 4 monthes from injury, after it, vigorous P. T. was carried out. Good results were come in 80% (24 cases). 9. Pin and plaster method will be indicated in minor & moderate severity-fractures, but in major ones, it will be used as a preliminary stage operation.

4.Three Cases of Aneurysmal Bone Cysts

Aneurysmal bone cyst is an uncommon tumor-like lesion of the bone containing distended vascular channels or cvities filled with blood. These cases, twenty-nine old female, eighteen years old male and female patients complained of pain and swelling in Rt. Medial aspect of proximal foot, Lt. proximal tibial portion and Rt. entire talar aspect of foot, respectively. Roentgenograms showed ballooned-out distensions and eccentric buldging of those lesions and soapbubble like multiple radiolucent cystic changes within interiors of the bones at talar neck, Proximal tibia and entire talus. Those cases were treated with radical curettage and autogenous iliac bone grafts and snugly fitted long leg casts in functional positions, respectively. On 2nd postoperative day, Cast windows were made and daily sterile dressings were carried out until complete wound healing. Pathological Examinations grossly and Microscopically revealed typical Aneruysmal bone cysts in all 3 cases. All-Patients had good therapeutic recovery within about seven weeks post-operatively.

5.Fixation with Harrington Rods of Unstable Thorace-Lumbar Spine Fracture and Fracture-Dislocation

Harrington Instrumentation and Spinal Fusion to treat the unstable fracture and fracture-dislocation of the thoraco-Iumbar spine is a very effective method. This method not only decompresses the spinal cord and nerve roots by anatomical reduction and preserves spinal stability, but also makes possible early rehabilitation and prevention of complications. A clinical study was made of twelve patients who were hospitalized and treated at the Department of Orthopaedic Surgery, Chung-Nam National University Hospital from Oct. 1978 to J une 1980. The following results were obtained: 1. Of twelve patients, five(42%) had a flexion-compression fracture, four(33.3%) had a shear fracture-dislocation, two(16.7%) had a flexion-rotational fracture, and one(8.3%) had a bursting fracture. 2. Seven(58%) had a complete neural deficit and five (42%) had an Incomplete neural deficit. Two (28%) of the seven patients with complete neural deficit showed slight neurological recovery, but two patients(40%) with incomplete neural deficit gained complete recovery. 3. Correction of the kyphotic deformity was 16 degrees on the average, and the displacement was completely corrected in five of the six patients. 4. During the follow-up period, there was a final loss of about five degrees (range 3-16 degree) of kyphotic correction In our patients and there was no difference in loas of correction between laminectomy group and control group. 5. After an average of 18 postoperative days, ambulation was started, and acute hospital days were 77 days on the average. 6. There was no difference in spinal fusion rate between brace-wearing group (4 cases) and cast Immobilized group (8 cases). 7. Two cases (16.6%) of the twelve cases showed spontaneous interbody fusion.

6.A case of persistent pustulosis and antecedent scabies.

In 1978, Bjornberg and Friis reported 32 patients with persistent pusi ulosis(PP) on distal extremities, of whom 13 patients were initially diagnosed as scabies. In 1984, Elpern described 7 patients with infantile acropustulosis(IA), of whom 4 patients had antecedant scabies. We report herein a case of PP, which was found in a 18 month-old mal who was treated for scabies at the age of 5 month and 12 month. The pruritic vesicopustules dev',lo ed 2 months after the second treatment for scabies and relapsed for 4 months. We also discuss the relationship between scabies and PP and also between PP and IA.

7.Two Cases of Oral Lichen Planus Associated with Chronic Liver Disease.

We present two cases of oral lichen planus associated with chronic liver diseases. One patient was a 56-year-old man that had advanced liver cirrhosis with hepatitis B viral infection, and the other, a daughter of patient 1, was identified as chronic active hepatitis with hepatitis B viral infection. Clinical and histopathological features of oral lesions were consistent with lichen planus in both cases.
