2.Congenital Herditary Stromal Dystrophy of the Cornea.

In 1978, a nonprogressive corneal dystrophy was seperated from other causes of congenital opacification on the basis of unique clinincal findings and characteristic electronmicroscopic findings. This disorder, termed congenital hereditary stromal dystrophy, appears to the result of disordered stromal fibrogenesis. Recently, the autors have experienced 4 members of a family with typical electronmicroscopic findings of congenital heaeditary stromal dystrophy of the cornea. In this report we describe the characteristic findings of congenital hereditary stromal dystrophy.

3.Clinical Features of Small Intestinal Atresia.

PURPOSE: Intestinal atresia is a well-recognized cause of bowel obstruction in the newborn. The management of neonates with intestinal atresia has improved in recent decades due to refinements in neonatal intensive care, operative techniques, use of total parenteral nutrition (TPN), and neonatal anesthesia. More recently, the survival rate has risen rapidly up to 90%. METHODS: Twenty-five (25) cases of intestinal atresia were encountered at Kwangju-Christian Hospital between January 1985 and December 1998. We reviewed sex, gestational age, body weight, clinical manifestations, associated ano malies, causes, interval to operation, preoperative complications, operative methods, and postoperative complications. RESULTS: Sites involved were the duodenum (n=10; 40%), the jejunum (n=5; 20%), and the ileum (n=10; 40%). The sex distribution was male predominant (1.8:1). The overall survival rate in our hospital was 76%. CONCLUSION: Refinements in neonatal intensive care and perioperative management were important in decreasing postoperative mortality.

4.Change of Biochemical Bone Markers in Pre- and Postmenopausal Women according to their Menopausal Period.

Changes of bone turnover with aging are responsible for bone loss and play a major role in osteoporosis. Among the early postmenopausal women, as are known by previous study, there are about 35% ""fast bone losers and bone turnover is more uncoupled in osteoporotic group than in normal control. So, early detection of such fast losers"" and women who have high turnover rate is important to prevent postmenopausal osteoporosis and spontaneous fracture. Dual-energy X-Ray absorptiometry (DEXA), however, cannot reflect current bone loss because changes in bone mineral density (BMD) are only seen after 1 or more years of bone densities declining. In this study, we have measured a battery of new sensitive and specific markers of bone turnover which reflect current bone loss. To investigate the changing pattern of those markers, 674 healthy women including 451 postmenopausal women were classified, according to their menopausal period (less than 5 years, 5-10 years, more than 10 years). Bone formation was assessed by serum osteocalcin (OC), bone-specific alkaline phosphatase (BSAP) and bone resorption by the urinary excretion of deoxypyridinoline (DPD), cross-linked N-telopeptide of type I collagen (NTX). All Biochemical markers, except OC, significantly increased after menopause (p < 0.05). NTX and BSAP remained elevated after 10 years of menopause. These data indicated that the overall rates of both bone formation and bone tesorption increased after menopause and remained high in elderly women.

5.A Rare Hyperextension Injury in Thoracic Spine Presenting with Delayed Paraplegia.

Hyperextension injury in the thoracic spine is uncommon with only a few cases documented in the literature. The mechanism of these injuries is hyperextension combined with axial or shearing force. These types of injuries are associated with a high risk of dural tears and paraplegia. A 91-year-old female presented with acute back pain from a hyperextension injury in thoracic spine with no neurological deficit. Lumbar magnetic resonance imaging showed a intervertebral disc rupture. On day 20 of hospitalization, the herniated intervertebral disc compressed the spinal cord with incomplete paraplegia. Hyperextension injuries involving the three columns are very unstable and we recommend surgical treatment as soon as possible, not only because of the initial trauma, but a ruptured disc herniation can damage the spinal cord.

6.Clinical Result of Iris Claw Lens and Trans-scleral Fixation Technique of Posterior Chamber Lens.

Recently, with the development of ophthalmic surgical technique, many of aphakic patients want secondary implantation of intraocular lens. In aphakic eye without posterior capsule, implantation of posterior chamber lens can be performed by sulcus fixation of lens with prolene suture. But sulcus fixation technique has some complications as iP~rmanent fistula, rotation or tilting of the optics, hyphema, endophthalmitis and so on. In this study, 24 aphakic eyes, 22 patients implanted withl itis claw lens were compared with 13 aphakic eyes, 13 patients implanted with sulcus fixated posterior chamber IOLs. In 24 eyes implanted with iris claw lens, 18 eyes (75%) had a postoperative visual acuity of 20/40 or better and cystoid macular edema, intraocular lens dislocation were observed on each 2 eyes (8.3%) and iridodialysis on 1 eye (4.1 %). In 13 eyes implanted with sulcus fixed PCLs, 12 eyes (92%) had a postoperative visual acuity of 20/40 or better, and cystoid macular edema, vitreous hemorrhage, acute iridocyclitis and glaucoma were observed in each 1 case (7.7%). In aphakic eyes without posterior capsular support, iris claw lens will be a alternative method of intraocular lens implantation.

7.Clinical Significance of Surgical Exploration on acute scrotum.

PURPOSE: Acute scrotum is a clinical syndrome that is defined as an acute painful swelling of the scrotum. It is well known that early surgical exploration is needed to prevent the testicular loss when testicu lar torsion breakouts. But most of reports were only focused on testicular torsion of prepubertal age group. This fact let us examine the acute scrotum with all age group and the significance of surgical exploration. MATERIALS AND METHODS: We retrospectively reviewed the results collected from 112 patients (131 cases) with acute scrotum who had been admitted to our hospital between June 1994 and November 1999 with the exception of scrotal trauma. RESULTS: The incidence of acute scrotum over 20 years old was 72.4% of total acute scrotum patients. The most common disease was acute epididymitis giving the rate of 71.8% (94 cases). In adult the incidence of testicular torsion was low (3 cases). Surgical torsion was 12 cases and acute epididymitis was 12 cases as the cause of acute scrotum. CONCLUSIONS: Most cases of testicular torsion have been below 20 years old and only 3 cases have been over 20 years old. Although the incidence of testicular torsion over 20 years old has been low. There were severe infectious diseases needing surgical approach in the cases of aged patient. this research shows that the treatment of acute scrotum goes with the need of surgical therapy.

8.MR Findings of Stereotactic Radiofrequency VlM-Thalamotomy.

PURPOSE: To evaluate the role of the MRI after the stereotactic radiofrequency ventralis intermedius nucleus (VIM) thalamotomy for the treatment of tremor. MATERIALS AND METHODS: 156 cases of the postthalamotomy MR findings were analized retrospectively. The sagittal T1 weighted image(WI), axial and coronal Proton and T2WI were obtained by using 0.38 T(Resonex Sunnyvale, U. S. A) machine. The interval between thalamotomy and MR examination was from 3 days to 2 months. The MR characteristics and complications related to thalamotomy were reviewed. In 16 cases, a follow-up MR was done 3 to 13 months after the initial MR study. We also reviewed the follow-up MR findings. RESULTS: The mean size of the thalamus lesion was 16 mm. The thalamus lesions were noted as a single layer in 23 cases and as layers of different signal intensity in 100 cases;(2 layers in 84 cases, and 3 or more layers in 16 cases). In 74 cases of the 84 cases with 2 layers, the inner layer was isointense with gray matter on T1WI, hypointense on T2WI, and the outer layer was hypointense on T1WI, hyperintense on T2WI. There were extrathalamic lesions that were related to mistargetting of stereotactic radiofrequency. The locations of the extrathalamic lesions were the posterior limb of the internal capsule(119 cases), the posterior limb of the internal capsule and the midbrain(39 cases), the posterior limb of the internal capsule and the basal ganglia(11 cases), and the midbrain(9 cases). In 5 cases of the mistargetting, double radiofrequency lesions were visualized because of the repeated coagulation. The other complications were intracerebral hemorrhage(2 cases), subdural hemorrhage(2 cases), epidural hemorrhage(1 case), and intraventricular hemorrhage(1 case). On the follow up MR studies(16 cases), 2 cases showed the hemosiderin deposition in periphery of the lesion. CONCLUSION: The MRI was useful for the evaluation of the thalamic lesions and complications after the stereotactic radiofrequency VIM-thalamotomy for the treatment of tremor.

9.Regional Lymph Node Involvement by Renal Angiomyolipoma.

Renal angiomyolipomas are benign mixed mesenchymal tumors, which are composed of atypical blood vessel, smooth muscle and fat cells. A 34-year-old woman presented with a palpable right abdominal mass, accompanied with discomfort. A CT scan showed a large right perinephric mass, with heterogeneous density, surrounding the kidney, with enlargement of the regional lymph nodes. The presumptive diagnosis was one of a malignant tumor of the retroperitoneum, with lymph node metastasis or an angiomyolipoma, involving both the kidney and lymph nodes. A right radical nephrectomy, including the tumor, was performed. The final diagnosis was an angiomyolipoma arising from the kidney, with the dissected lymph nodes having the same pathology. There was no evidence of a tumor recurrence during the six month follow-up period. This was an unusual case of a multicentric angiomyolipoma.
