1.Studies of the Timolol Effect on Intraocular Pressure and Concentration in Aqueous Humor in the White Rabbit.

The effect of 0.25% and 0.5% Timolol on intraocular pressure and concentration in aqueous humor was studied after topical instillation in the white rabbit. The intraocular pressure was measured with pneuma tonometer and the Timolol concentration in aqueous humor was measured with High Preformance Liquid Chromatograph after fine needle aspiration of aqueous humor at various time intervals. The results were as follows; 1. The percentage reduction of intraocular pressure increased with time and reached a peak after 3 hours. 2. The concentration of Timolol in aqueous humor increased with time and reached a peak after 3 hours. 3. There was close correlation between intraocular pressure and Timolol concentration in aqueous humor.

2.Hand, foot and mouth disease associated with the aseptic meningitis in Seoul, 1990.

We performed clinical and virological studies on 79 children with hand, foot and mouth disease (HFMD) who had been admitted or visited to the department of Pediatrics, Asan Medical Center from February to August, 1990. The results were as follows; 1) There were total of 79 cases with hand, foot and mouth disease during this period. The aseptic meningitis was combined in 18 cases (22.8%). 2) The male-to-female ratio of HFMD was 2.03 to 1, but that of aseptic meningitis was 8 to 1. 3) Hand, foot and mouth disease occurred more frequently in under the age of 5 years, and the aseptic meningitis in under the age of 1 year. 4) Hand, foot and mouth disease and the aseptic meningitis occurred mostly in May and June. 5) In addition to the skin manifestations characteristic in hand, foot and mouth disease, some of the patients with aseptic meningitis had the following symptoms: change of consiousness, seizure, monoparesis in the upper extremity, paraparesis in the lower extremity. 6) The causative agent of hand, foot and mouth disease associated with the aseptic meningitis in Seoul in 1990 seemed to be enterovirus 71 according to the serologic study.

3.CT and MR Findings of Brain Abscess: Focus on Thickeness and Signal Intensity of Abscess Wall.

PURPOSE: The purpose of this study is to evaluate the CT and MR findings of brain absecss with emphasis on the thickeness and signal intensity of abscess wall. MATERIALS AND METHODS: Twenty CT scans and seven MR studies of 23 patients with brain abscess (27 abscesses) were retrospectively reviewed with respect to thickeness and signal intensity of abscess wall, shape and size of abscess, etc. Thickness of abscess wall was measured in the greatest abscess diameter on the CT scans and MR images obtaihed after intravenous administration of contrast material. RESULTS: Enhancing abscess wall was 2mm--6mm (average 3mm) thick. The abscess wall was uniform in thickness in 14 cases (52%), thinner in the roedial wall than lateral wall in 8 cases (30%), and thicker in the medial wall than lateral wall in 5 cases (18%). Signal intensity of abscess wall was isointense relative to gray matter on Tl-weighted MR images and hypointense on both proton- and T2-weighted MR images in 7 out of 8 lesione (88%). The inner margin of the abscess wall was smooth in 14 (52%) and irregular in 13 lesions (48%). The outer margin was smooth in 15 (55%) and irregular in 12 lesions (45%). The size of the abscesses was variable, ranging from lcm to 6cm in diameter. They were round (16 cases), elliptical (6 cases), or multilobulated (5 cases) in shape. Satellite or daughter abscesses were found in 5 patients. CONCLUSION: Abscess wall showed variable thickness in the medial and lateral walls with no specific findings. Other CT and MR findings were also nonspecific, although hypointensity of abscess wall on T2-weighted MR images may be helpful in diagnosis of brain abscess.

4.Two Cases of Transient Neonatal Diabetes Mellitus in Twin Brothers.

Neonatal diabetes mellitus (NDM) is defined as hyperglycemia that presents clinical symptoms within the first month of life and requires insulin therapy to maintain euglycemia. The diabetic condition may be transient, permanent, or transient with recurrence later in life. NDM is a rare clinical disorder. Three cases of NDM have been reported in Korea so far. We experienced two cases of NDM in twin brothers who were born with small for gestational ages. Their HLA typings were DR9 and DR14, and insulin autoantibody, islet cell antibody, GAD-reactive autoantibody, and insulin receptor antibody were not found. The genetic analysis with polymorphic DNA markers for chromosome 6 indicated paternal uniparental isodisomy.

We report here on a case of popliteal aneurysm and rupture that occurred over a 10-day period and this was all secondary to salmonella infection. Computed tomography (CT) angiography of the extremity that was performed before and after aneurysmal rupture showed the aneurysm's rapid evolution to rupture over a short period of time. We also review the pathogenesis, clinical presentation, diagnostic approach and management of salmonella aneurysms.

8.Experience of Administration of Integrative Course for Internship.

PURPOSE: The aim of this study is to improve the integrative course for internship by evaluating and analyzing the questionnaire results. METHODS: After the 10 times of integrative course implemented in 2002, the questionnaires were asked to the 43 interns working in university hospital (27 interns replied) located in Inchon. The item of the questionnaires were relevance of the integrative course, pertinence of the subject matters, usefulness of the integrative course, subjects to be added to the integrative course, subjects that must included in the next integrative course, subject most helpful, subjects least helpful, and suggestions to the integrative course. RESULTS: The ratio of the positive answer to the numbers of the lecture times, relevance of the subject matters and usefulness of the integrative course were high. The subject most helpful and must included in the next integrative course was "Usage of antibiotics" from Department of Infection. The subjects least helpful were "Basic urologic procedure" and "Professionalism of the young physicians". This results suggests that whether the course or subject matter is related to the real clinical setting or not is important. Almost all interns want not being interrupted during the lecture time. CONCLUSION: Because almost all interns are tended to evaluate positively, the integrative course for the internship seems to be successful. The course should be organized to help the clinical experience of interns.

9.Changes in High Levels of 17 alpha-Hydroxyprogesterone at Neonatal Screening.

PURPOSE:There are some cases of false positive in neonatal screening for congenital adrenal hyperplasia. To understand its significance of the increased 17 alpha-hydroxyprogesterone(17 alpha-OHP) levels in the cases of positive result, we followed its levels during infancy. METHODS:From July 1997 to June 1998, totally 1,730 babies were screened for congenital adrenal hyperplasia(CAH) measuring 17 alpha-hydroxyprogesterone(17 alpha- OHP) using an enzyme immunoassay(EIA) on dried blood spots. In infants of positive result, the radioimmunoassay(RIA) was used measuring 17 alpha-OHP in serum. And we followed up the levels of 17 alpha-OHP during infancy. Cortisol and ACTH were also measured and followed up in the morning simultaneously. RESULTS:No CAH was detected during the study period. With the 13 ng/mL cutoff limit for 17 alpha-OHP before March 1998, the recall rate was 3.09%. With the 20 ng/mL cutoff limit after March 1998, the recall rate was decreased to 1.58%. Follow-up of the high levels of 17 alpha-OHP in positive cases showed a sharply decreased curve according to age. CONCLUSION: In clinically normal neonates, the high levels of 17 alpha-OHP at screening decreased continuously during infancy.
