1.A Case of Sebaceous Epithelioma.

Sebaceous epithelioma is an uncommon and primary neoplasm of sebaceous gland. We report herein a case of sebaceous epithelioma in an 85-year-old female. Physical examination showed a solitary, 2.5 x 3.5cm sized, reddish. irregularly surfaced, and well defined tumor on the intercanthal area. Histopathological findings showed tumor parenchyma. romprised of several large nests of three kinds of epithelial cells, mature sebaceous cells, undifferentiated cells arranged in pa1isade fashion at the periphery of a cell nest, and transitional cells showing beginning fatty vacuolizatiorn of the cytoplasm. Electron micocopy revealed immature dark cells, transitional cells, and mature sebaceous cell which showed many scattered foci of sebaceous ditferentiation characterized by intracytoplasmic lipid droplets and glycogen particls.

3.A Case of Familiar Benign Chronic Pemphigus.

A 25-year-old man with familial benign chronic pemphigus presented with a one-year history of a localized pruritic recurrent eruption on his perianal area, Physical examination showed moist, macerated, fissured and scaly patches on erythematous base, A biopsy specimen showed extensive intraepidermal separation containing acantholytic cells. Electron microscopic observation showed widened intercellular space, detachment of the tonofilaments from the desmosomes, and subsequent concentration of the tono filaments around the nucleus. Microvilli were elorigated, thinned and branehed. 1)esmosomes were reduced in number. This case is unique in its clinical location, showing no family tendency.

4.A Case of Pellagra with Psychoneurotic Symptoms and Neurologic Disturbances.

The authors experienced a case of typical pellagra in 45-year old male. He has been a heavy alcoholic history about 30 years duratian. Well-marginated erythematous created scaly patches, hyperpigmentation and fissures developed on both forearms and both ear lobes, both periorbicular areas, and Casals and klace on the aa0erior neck repeatedly. The skin leaions were aggravated in summers, improving a little in winters from 3 years ago. The psychoneurotic symptoms, neurologic disturbances, and GI troubles accompanied the skin lesions. Diagnosis was made by the characteristic clinical findings. He was treated with nicotinamide and vitamin B complex, and most of symptoms improved much. Authors reported the case and reviewed the literatures.

5.Primary Cutaneous Meningioma arising from the Scalp: A case report.

Primary cutaneous meningiomas are extremely rare tumors found in the cutis or subcutis, and generally have a benign course. The tend to be located on the scalp, face, neck. or paravertebral area. The primary cutaneous meningioma bears similarities to developmental defects and probably originates from arachnoid cell rests in the skin, although diverse groups of cutaneous meningiomas seem to arise from several different sources. A case of primary cutaneous meningioma occuring in the scalp of left parietal area of a 27-year-old female is presented. Clinically the lesion appeared as indolent, slow growing cutaneous mass and has no connection with underlying brain tissue, as determined by examination of the roentgenographs. The definite diagnosis was made after pathological examination. Microscopically the tumor is composed of sheets and nests of meningothelial cells. Immunohistochemical and electron microscopic studies reveal the typical findings of meningioma.

6.The Measurement of the Soft Tissue Pressure beneath a Tourniquet in the Arm

The use of a pneumatic tourniquet is potentially associated with injury to underlying muscles, vessels, and nerves if excessive pressure occurs beneath the toumiquet. In order to minimize the risk of soft tissue injury, the lowest tourniquet pressure that maintains a bloodless operative field should be used. A clinical study was undertaken to evaluate the pneumatic tourniquet setting required for adequate hemostasis in upper extremity surgery. From March to September in 1984, the subcutaneous soft tissue pressure of the 20 upper extremities beneath a pneumatic toumiquet in the arm were measured directly and the following results were obtained. 1. The underlying subcutaneous soft tissue pressure was not affected by adult, age, arm circumference, and blood pressure of normal range. 2. The underlying subcutaneous soft tissue pressure showed direct correlation with the tourniquet pressure respectively. 3. A tourniquet preasure of more than 250 mmHg was not rarely required in a normotensive individual.

7.Morphology and Function of Testis in Growth Deprived Mature and Immature Male Rats.

Growth hormone(GH) plays a major role in growth and its effects are mainly mediated by insulin like growth factor-I ( IGF-I). However the role of GH in the regulation of testicular function especially in spermatogenesis is still under debate. Though there were several studies using GH-deficient mutant mice, results are conflicting. Furthermore they are not proper to find out any exact role of GH in normal adults. We have therefore used somatostatin analogue (Sandostatin) for 1 week to block the effect of GH and it's mediator IGF-I, in mature and immature Sprague-Dawley male rats. In immature rats the delayed maturation was recovered after ceasing somatostatin in several weeks and the testicular parameters became normal after maturation. Post-pubertal rats showed diminished spermatogenesis especially in the number of pachytene spermatocytes and round spermatids of stage VII. These results seems to be associated with decreased systemic IGF-I. But the Leydig cell function in other words, testosterone production, remained normal. Somatostatin administered Leydig cells showed strong positive reactions like normal control on IGF-I immunohistochemical stain. We concluded that GH mediated IGF-I plays a major role in spermatogenesis especially on pachytene spermatocytes of post-pubertal male rats and there are GH independent IGF-I actions in Leydig cells.

8.Two Cases of Keratosis Palmaris et Plantaris.

We reviewed two cases of keratosis punctata palmaris et plantaris(KPP) and the scanning electron microscopic findings(SEM). The skin lesions of KPP are multiple, hyperkeratotic on the palms and soles with central pitting. There was no subjective symptoms. The histologic findigs revea,l marked hyperkeratosis, hypergranulosis, and acanthosis with depression of the underlying malpighian layers. The SEM findings reveal one hyperkeratoic plug on punctate lesion(case I ) and three hyperkeratotic plugs on depression of the malpighian layer(case ll ).

9.A Case of Darier' s Disease Improved with Oral Etretinate.

We report a case of Darier's disease occuring in 49-year-old female patient. She had hyperkeratotic crusted eruptions on her forehead, neck and posterior auricular area. The skin lesions aggravated in summer and improved in winter, but never been healed completely. Histopathologic features from neck lesions revealed hyperkeratosis with corps ronds and grains as well as formation of villi and laeunae. We treated her with oral etretinate, and she was almost completely free of disease after 3 months treatment. We suggested that etretinate is an importnat advance in the treatment of Darier's disease.

10.Aggressive Osteoblastoma.

Osteoblastomas were originally thought to be lesions that could be controlled with minimal local therapy, including curettage. However, at least some of the osteoblastomas with an unexpectedly active clinical course had histologic features which differed from ordinary osteoblastomas, and the term aggressive osteoblastoma was given to these lesions. In the aggressive osteoblastoma the osteoblasts are cytologically atypical. In addition, the well organized trabecular patterns of typical osteoblastoma is partly lacking, especially in the areas that are rich in atypical osteoblasts. The case report deals a case of aggressive osteoblastoma of the left iliac wing in a 44 year old male. The interesting radiologic manifestationa(plain x-ray CAT and radionuclide bone scan) and characteristic histologic findings are presented along with review of pertinent literatures.
