1.An In-Vitro Wear Study of Ceromer and Human Enamel.

The ideal restorative material should mimic the properties of the tissues it replaces. Dental composite resins have been used widely as restorative materials due to its advantages such as excellent esthetics and ease of manipulation. But inadequate wear resistance has been a major factor limiting the use of composite restorative materials. Improved manufacturing techniques have allowed the development of hybrid composites, with a greater percentage volume filler loading, which have improved physical an mechanical properties. However they are lacking the study wear resistance. The purpose of this study was to evaluate the wear of human enamel against ceromer by the use of a pin-on-disk type wear testers. Discs of ceromer(Targis : Ivoclar, Vivadent, Amherst. NY) and discs of type III gold alloy as a control were used for test specimens. Intact cusp of premolar and molar were used for enamel specimens. The wear of enamel was determined by weighing the cusp before and after each test, and the weight converted to volumes by average density of enamel. Surface profilometer was used to quantify wear of the ceromer and gold specimens. Vicker's hardness tester was used to evaluate the surface hardness of test specimens. The SEM was used to evaluate the wear surfaces. The results were as follows; 1. Ceromer produced less enamel wear than gold(p<0.05). 2. The wear volume of ceromer was greater than that of gold(p<0.01). 3. The hardess of ceromer was lower than that of gold, but there was no correlation between the hardness and wear of the ceromer and gold. 4. SEM analysis revealed that there were many voids and microcracks in the wear tract of ceromer. in gold group, many minute V-shaped grooves were examined.

2.Tension Band Fixation in the Treatment of the Olecranon Fracture

Anatomically olecranon fracture is an intra-articular fracure, so reduction must be exact. Using the principle of Zuggurtungsosteosynthese by Weber, Pauwels' clinical study of olecranon fracture showed the superiority of tension band. 15 patients with olecranon fracture of the ulna who were treated with tension band and could be followed at the Department of Orthopedic Surgery of Han Il Hospital from January, 1974 to December, 1980 have been reviewed. The result obtained is as follows. We obtained the more rigid fixation with modifying the standard tension band locating the end of K-wire in the cortex of opposite side cortex of ulna. So, the result of treatment was satisfactory.

4.Chronological observation on intestinal histopathology and intraepithelial lymphocytes in the intestine of rats infected with Metagonimus yokogawai.

The relationship between the intestinal histopathology and number and position of intraepithelial lymphocytes (IEL) was observed chronologically in the small intestine of rats experimentally infected with Metagonimus yokogawai. Fifteen Sprague-Dawley rats were orally infected each with 3,000 metacecariae, and 3 were kept uninfected for controls. Three rats each were sacrificed on the day 5, 10, 15, 24 and 70 post-infection (PI) and samples of the small intestine, 5 cm, 10 cm, 20 cm and 70 cm posterior to the pylorus were taken. The samples were processed routinely and stained with Giemsa. The intestinal histopathology was severe during the day 5-15 PI and characterized by villous atrophy, crypt hyperplasia, and decrease of villus/crypt height ratio. After the day 24 PI, the intestinal lesions showed some tendency of recovery. The number of IEL increased at the early stage of infection, but decreased thereafter to a lower level than that of controls, with progression of the pathological changes. Then, the IEL number began to increase again after the day 24 PI. In control rats, the great majority of the IEL were located at the basal region of the epithelium. During the early stage of infection, however, a considerable proportion of IEL was found to have moved to the intermediate or apical region of the epithelium. From the above results, it is suggested that the change of IEL number and position during the course of M. yokogawai infection should be closely related to the progression and recovery of the intestinal histopathology.

5.Two Cases of Extrapelvic endometriosis following Laparoscopy-assisted vaginal hysterectomy and Cesarean section.

Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

6.A rare case of a large solid pseudopapillary neoplasm with extensive liver metastasis

Solid pseudopapillary neoplasms (SPNs) are uncommon pancreatic tumors that primarily affect young females. We report a case of a 24-year-old female diagnosed with SPN and liver metastasis during a routine examination. Imaging revealed an 8-cm pancreatic mass with multiple liver metastases. Histopathology confirmed SPN. Subsequent next-generation sequencing revealed a CTNNB1 mutation.The patient underwent a total pancreatectomy with splenectomy, right hemihepatectomy, and intraoperative radiofrequency ablation.Two years after the surgery, she remained complication-free. She is under regular surveillance. This case underscores the importance of early detection and comprehensive management of SPN.

9.Experience of Administration of Integrative Course for Internship.

PURPOSE: The aim of this study is to improve the integrative course for internship by evaluating and analyzing the questionnaire results. METHODS: After the 10 times of integrative course implemented in 2002, the questionnaires were asked to the 43 interns working in university hospital (27 interns replied) located in Inchon. The item of the questionnaires were relevance of the integrative course, pertinence of the subject matters, usefulness of the integrative course, subjects to be added to the integrative course, subjects that must included in the next integrative course, subject most helpful, subjects least helpful, and suggestions to the integrative course. RESULTS: The ratio of the positive answer to the numbers of the lecture times, relevance of the subject matters and usefulness of the integrative course were high. The subject most helpful and must included in the next integrative course was "Usage of antibiotics" from Department of Infection. The subjects least helpful were "Basic urologic procedure" and "Professionalism of the young physicians". This results suggests that whether the course or subject matter is related to the real clinical setting or not is important. Almost all interns want not being interrupted during the lecture time. CONCLUSION: Because almost all interns are tended to evaluate positively, the integrative course for the internship seems to be successful. The course should be organized to help the clinical experience of interns.

10.Serovar Identification and Genetic Characterization of Leptospira Isolates by Arbitrarily Primed PCR and Ribotyping.

Serovars of 22 leptospiral field isolates from rats trapped in Korea were identified by cross-agglutinin absorption test (CAAT). Genomic characteristics of 7 selected isolates and 6 antigenically closely related reference serovars of lai, yeonchon, birkini, gem, mwogolo, and canicola were differentiated by arbitrarily primed PCR (AP-PCR) and southern blot hybridization using 16S rRNA gene probe from Borrelia burgdorferi. Among the 22 isolates, 21 strains were identified as serovar lai by CAAT, while the serological reactivity of NR13 did not accord with that of serovar lai. Results of AP-PCR using primers RSP, KF and PB-1 were in general agreement with those obtained by serological identification, and all 7 isolates including NR13 showed the same profile with serovar lai or yeonchon. In the southern blot hybridization with 16S rRNA gene probe, the isolates were divided into two ribotype groups when HindIII and BamHI digests were employed: isolates NR4, NR13, and serovar lai showed the same profile, and isolates JR34, JR57, KR48, JR77, and JR82 were classified as the another ribotype group. Isolate NR13 and serovar yeonchon, which were isolated in Korea and showed serological differences with serovar lai, were indistinguishable from serovar lai in this DNA study using AP-PCR and ribotyping. These results demonstrate that Korean leptospiral isolates were closely related in DNA level, and ribotyping would be useful for subgrouping of field isolates.
