1.Regrowth of Grafted Lower Half of the Hair Follicle.

We have examined the regenerative capabilities of the human scalp hair follicle after grafting the lower half of the follicle. Twenty-eight of 32 intact whole-hair follicles isolated from the human scalp regenerated hairs when grafted onto the forehead of the same person. Seven of the 15 lower-half follicles regenerated complete hair follicles 8 months after grafting showed that the lower-half follicle implant reconstituted the complete hair follicle. The sebaceous gland was not regenerated, but there was an outgrowth in the sebaceous gland regPark ion. Some grafts formed epithelial cysts. Two years after grafting, the histological examination of the regenerated follicle from the lower-half implant showed that the sebaceous gland was completely regenerated. While an intact follicle shows prominent naked shaft outgPark Park Parkrowth, the sheath grows concomitantly with the shaft in lower-half follicles in culture. If grafted lower-half follicles were located too deep, the regrown sheath could not reach the epidermal layer. In this situation, the formation of an epidermal cyst was likely.

3.Intestinal histopathology and in situ postures of Gymnophalloides seoi in experimentally infected mice.

The intestinal histopathology and in situ postures of Gymnophalloides seoi (Digenea: Gymnophallidae) were studied using C3H/HeN and C57BL/6 mice as experimental hosts; the effects of immunosuppression were also observed. The metacercariae isolated from naturally infected oysters, 300 or 1,000 in number, were infected orally to each mouse, and the mice were killed at days 3-21 post-infection (PI). In immunocompetent (IC) mice, only a small number of flukes were found in the mucosa of the duodenum and jejunum during days 3-7 PI, with their large oral suckers pinching and sucking the root of villi. The intestinal mucosa showed mild villous atrophy, crypt hyperplasia, and inflammations in the villous stroma and crypt, with remarkable goblet cell hyperplasia. These mucosal changes were almost restored after days 14-21 PI. In immunosuppressed (IS) mice, displacement as well as complete loss of villi adjacent to the flukes was frequently encountered, otherwise the histopathology was generally mild, with minimal goblet cell hyperplasia. In these mice, numerous flukes were found, and it seemed that they were actively moving and rotating in situ. Several flukes were found to have invaded into the submucosa, almost facing the serosa. These results indicate that in IC mice the intestinal histopathology caused by G. seoi is generally mild, and the flukes do not penetrate beyond the mucosa, however, in IS mice, the flukes can cause severe destruction of neighboring villi, and some of them invade into the submucosa.

4.A Study on the Factors Affecting Physician Participation in Emergency Situations

Background:
This study was conducted on the experiences, limitations, and factors necessary for increasing participation in emergency care outside a medical institution for physicians working in a tertiary hospital.
Methods:
This study was conducted by analyzing data from 150 physicians working in a tertiary hospital. The physicians’ intent to participate in emergencies outside the medical institution was measured as frequencies and percentages, and chi-square test was performed. Multi-logistic regression was used to determine factors affecting the physicians’ willingness to participate emergencies outside the medical institution.
Results:
Out of 103 respondents, 52 (50.5%) said that they do not want to participate in future emergency situations. Among them, 38 (73.1%) said that legal burden associated with medical accidents dissuaded them from intervening in future emergencies. Also, 33 (63.5%) said that ‘a firm guarantee of exemption from legal responsibility’ was needed to convince them to participate in emergency patient care. There were no meaningful results in the analysis of factors affecting future participation in emergency care.
Conclusion
It was found that the reason they do not participate in emergency care is because they feel that they bear the risk of legal liability. We suggest that providing more legal immunity could be a way to encourage doctors to step up in emergency situations. At the same time, it is recommended that a more suitable environment be provided for physicians in such situations, so that they can be able to fulfill their ethical duties in emergency medical care.
5.Analysis of Nasalance according to Pattern of Phonation.

BACKGROUND AND OBJECTIVES: Various phonation patterns yield various voice characteristics. Voice therapy using nasal stimulatory sounds seems to facilitate phonation in voice disodered patients. Under the hypothesis that nasalance may be influenced by the pattern of phonation, we studied the relationship between nasalance and voice disorders by observing abnormal supraglottic movements and vocol cord gaps in phonation. SUBJECTS AND METHOD: There were 143 patients who complained of voice problems and showed abnormal false vocal cord movements under stroboscopy. In addition to the four previously described types of MTD (muscle tension dysphonia), we described two more types of MTD (V: false vocal cord contracted posteriorly, VI: false vocal cord dilated laterally). We measured the vocal cord gaps in phonation and analyzed nasalance. RESULTS: Among those groups showing the pattern of false vocal cord (MTD 1, 2, 4), the vocal cord gaps in phonation were increased and nasalance was significantly decreased in MTD types, III and IV, and showed a tendency to decrease in MTD types, II and V, compared to the normal group. CONCLUSION: The supraglottis has a tendency to contract as the vocal cord gap in phonation increases, and this movement reduces nasalance.

6.Localization of cytoskeletal proteins in Pneumocystis carinii by immuno-electron microscopy.

Pneumocystis carinii causes serious pulmonary infection in immunosuppressed patients. This study was undertaken to observe the cytoskeletal proteins of P. carinii by immuno-electron microscopy. P. carinii infection was experimentally induced by immunosuppression of Sprague-Dawley rats for seven weeks, and their lungs were used for the observations of this study. The gold particles localized actin, tropomyosin, and tubulin. The actin was irregularly scattered in the cytoplasm of the trophic forms but was much more concentrated in the inner space of the cell wall of the cystic forms called the inner electron-lucent layer. No significant amount of tropomyosin was observed in either trophic forms or cystic forms. The tubulin was distributed along the peripheral cytoplasm and filopodia of both the trophic and cystic forms rather than in the inner side of the cytoplasm. Particularly, in the cystic forms, the amount of tubulin was increased and located mainly in the inner electron-lucent layer of the cell wall where the actin was concentrated as well. The results of this study showed that the cell wall of P. carinii cystic forms is a structure whose inner side is rich in actin and tubulin. The location of the actin and tubulin in P. carinii suggests that the main role of these proteins is an involvement in the protection of cystic forms from the outside environment by maintaining rigidity of the cystic forms.

7.The SPM Study on the Change of Titanium Surface Roughness following Air-powder Abrasive and Application Time of Citric Acid.

The Peri-implantitis causes inflammation of periodontal tissue and bone loss. It contaminates surface of implants. Therefore, guided bone regeneration has been used for the treatment of this disease. For the reosseointegration of the exposed surface, various mechanical and chemical methods have been used for cleaning and detoxication of implant surface. Among these methods, air-powder abrasive and oversaturated citrate are known to be most effective. However, these treatments may deform implant surface. In this research, changes of surface roughness they were examined. 10 experimental machined titanium cylinder models were fabricated to be used for control groups. Each of them was air-powder abraded for 1 minute and they were named group 1. And then, group 1 were burnished with cotton pellets soaked with citrate for 30 seconds(Group 2), 1 minute(Group 3), 3 minutes(Group 4), and 5 minutes(Group 5) burnishing were applied for grouping respectively. Each group were examined with SPM, and their surface roughness were measured and analyzed. 1. Surface roughness of titanium decreased when it was air-powder abraded for 1 minute. It was statistically significant. 2. When Air-powder abraded titanium were treated with citrate for 3 minutes, Their surface roughness was the lowest. Titanium treated for 1 minute was the second lowest and 30 seconds was the third and titanium burnished for 5 minutes was the highest. 3. Surface roughness of titanium which was treated with citrate was decreased till 3 minutes, which was statistically significant. There was no statistical significance from 30 seconds to 1 minute and from 1 minute to 3 minutes, and there was statistical significance from 30 seconds to 3 minutes. 4. Oxide layer was formed when titanium is exposed to air, and it was removed when air-powder abraded. It was made when treated with citrate. It is thought that citrate treatment is necessary after the air-powder abrasion, and 1 minute is clinically and qualitatively adequate for burnishing time of citrate.

8.Comparative Sensitivity of ARCHITECT Syphilis TP with Other Antibody Tests.

BACKGROUND: Serological tests detecting antibodies specific to antigens of Treponema pallidum are useful tools for diagnosis and screening of syphilis. Nevertheless, conventional non-treponemal tests, which are commonly being used thanks to their low cost, have limited consistency in procedures and interpretations because of lack of automation. Moreover, they have innate lower sensitivity and specificity. Therefore, clinical use of non-treponemal tests has been significantly substituted by more reliable treponemal antibody assays. Considering such trend, diagnostic sensitivity of a recently introduced automatic chemiluminescent immunoassay, ARCHITECT Syphilis TP (Abbott Japan, Tokyo, Japan), was evaluated with comparison to existing methods to validate its clinical sensitivity. METHODS: Fifty stored sera with reactive results on a non-treponemal syphilis test done by requests from the attending physicians were collected consecutively. Diagnosis of syphilis was confirmed when two or more positive results were obtained among 3 treponemal antibody tests, a hemagglutination, an automated latex turbidimetry and ARCHITECT Syphilis TP. RESULTS: All 50 sera were confirmed as syphilis-infected. Diagnostic sensitivity of ARCHITECT Syphilis TP was estimated to 100%, whilst that of the hemagglutination and the latex turbidimetry was 94% and 98%, respectively. Correlation coefficient between S/COs of ARCHITECT Syphilis TP and quantitative results on the latex turbidimetry was caculated as 0.865. Compared to the hemagglutination titers, better quantitative correlation was observed with S/COs of ARCHITECT Syphilis TP. CONCLUSIONS: ARCHITECT Syphilis was thought to be a reliable test with good sensitivity and its automated feature was thought to be additionally beneficial. Although it is being used for a qualitative purpose, it seemed possible to expand its utility into quantitative use considering its acceptable quantitative correlation with the hemagglutination assay as well as the automated latex turbidimetric assay.

10.Coracoclavicular Ligament Suture Augmentation with Anatomical Locking Plate Fixation for Distal Clavicle Fracture.

BACKGROUND: For Neer type IIB fracture of distal clavicle with coracoclavicular ligament injury, various surgical treatments have been used in literatures. However, there was no consensus on the optimal treatment. The aim of this study is to report the clinical and radiological results of open reduction and internal fixation of unstable distal clavicle fracture and suture augmentation of disrupted coracoclavicular ligament. METHODS: A prospective study was performed in 23 patients with Neer type IIB distal clavicle fracture in Seoul Medical Center, Eulji Hospital, and National Medical Center. Firstly, suture anchors are inserted in the base of coracoid process and preliminary reduction was achieved by tie-off of three suture limbs around the clavicle. Then, the final fixation was completed with anatomical locking plate. Bony union and the distance between coracoclavicular ligaments were evaluated. Clinical results and complications including stiffness and secondary procedures were evaluated. RESULTS: Bony union was achieved in all cases except one (22 of 23). At mean 14.9 months, no significant difference in the mean coracoclavicular distance was observed compared to uninjured shoulder (8.2 +/- 7.9 mm versus 7.3 +/- 3.4 mm, p=0.14). Pain visual analogue scale, American Shoulder and Elbow Surgeons score, Constant score, and Disabilities of the Arm, Shoulder and Hand score were 0.5, 83.4, 78.5, and 6.2, respectively. Revision surgery was performed in one case of nonunion. Four patients who complained of skin irritation underwent implant removal. CONCLUSIONS: In cases of an unstable distal clavicle fracture with coracoclavicular ligament disruption, satisfactory clinical results were obtained by locking plate fixation and coracoclavicular ligament suture augmentation concurrently.
