2.A Case of Trichilemmal Carcinoma Showing a Feature of Cutaneous Horn.

Trichilemmal carcinoma is a rare malignant neoplasm of the hair follicle, which is derived from or differentiates towards cells of the outer root sheath. We report a case of trichilemmal carcinoma in an 83-year-old female patient. She presented with a tender dome-shaped crusted papule showing a feature of cutaneous horn on the forehead, which was first detected 6 months before. nstopathologically, lobular patterned tumor cells with peripheral palisade of basaloid cells, nuclear atypia, and clear or pale, PAS-positive, diastase-sensitive cytoplasm were observed beneath the marked hyperkeratosis. After the diagnosis, total excision was done. The patient has been free of recurrence or metastasis till now.

4.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.

5.Clinical Course of Renal Angiomyolipoma.

Renal angiomyolipomas are uncommon benign neoplasms. From April 1990 to June 1993, 10 patients with angiomyolipoma were followed clinically to determine the clinical course of angiomyolipoma. Average age of patient was 50 years( range 24 to 75) and of the patients 80% were women, 10% had tuberous sclerosis and the average size of the renal mass was 9.l x 6cm ( range 2x1.5 to 13.8 x 9.1). Clinically the most common presenting complaint was abdominal/ flank pain and of the patients 40% were spontaneous rupture, 2 cases had hemorrhagic shock. The diagnosis and size was determined on the basis of ultrasonography/CT findings. Of 10 patients, 1 patient was observed, 3 patients were managed with nephrectomy and 6 patients were managed with embolization. 10 patients were followed for an average 28 months( range of 12 to 50) by serial radiological imaging and clinical examination. The followup ultrasonography and/or CT scans were compared to the initial studies with respect to hemorrhage, or change in size or composition of the tumor. One patient was managed with observation, symptoms developed and tumor size increased 35 months later. 3 patients were free of disease for 20 to 50 months after nephrectomy. 5 of 6 patients managed with embolization, a radiological examination performed after the embolization demonstrated the original mass had liquefied and decrease in mass size. But 1 patient did not change in size and composition. These results demonstrate the selective embolization is an effective method as the primary therapy for control of hemorrhagic or symptomatic angiomyolipomas because of its parenchymal-sparing nature.

6.Visual prognosis of patients undergoing cataract extraction and its relationship with that of the contralateral eye.

PURPOSE: This study is to evaluate the visual prognosis according to the ocular morbidity of the fellow eye in a national university hospital located in rural area. METHODS: We retrospectively reviewed the medical records of 390 eyes of 315 patients who had undergone cataract surgery. We assessed the change of postoperative visual acuity and the state of preoperative fellow eye. RESULTS: The age at cataract surgery was the highest in the sixties (29.2%) and seventies (27.7%). The patients older than 80 of age were 10.0%. Preoperative ocular co morbidities (23.3%) were diabetic retinopathy, glaucoma, corneal opacity, uveitis, etc. Preoperatively, 91.3% of all cases had a visual acuity of 0.4 or lower, but 3 months after surgery, 72.1% of all had 0.5 or better. At postoperative 3 months, 27.9% showed visual acuity of 0.4 or less and the ocular co morbidities were found in 67.0%. Preoperative visual acuity of less than 0.1 in the fellow eye was 10.2%. Preoperatively, 97.5% of these cases had a visual acuity of 0.4 or lower, but only 40% had 0.5 or better postoperatively. Preoperative and postoperative visual acuity were poor in patients whose preoperative visual acuity was less than 0.1 in the fellow eye. Extrapelvic endometriosis is a rare disease. The majority of extrapelvic endometriosis cases involve scar tissue following obstetric and gynecologic procedures. We have treated two cases of extrapelvic incisional endometriosis. A 39 year old female patient with cyclic vaginal spotting after laparoscopic assisted vaginal hysterectomy due to uterine myoma and a 35 year old female patient with a painful palpable abdominal mass after cesarean section. Both underwent complete excision and were proven to have endometriosis by pathology. Here we report on both cases and review the medical literatures.

OBJETIVE: To assess the effects of progesterone and acetyl-L-carnitine used after treated with IsolateR gradient before semen cryopreservation-thawing on sperm parameters and membrane integrity. MATERIALS AND METHODS: From April 2001 to July 2001, ten normal male partner of couples who were visited in vitro fertilization (IVF) clinics. the semens were treated with IsolateR gradient before cryopreservation, spermatozoa was incubated with progesterone (1, 5 and 10 micrometer), acetyl-L-carnitine (2.5, 5 and 10 micrometer), or both (progesterone, 1 micrometer; and acetyl-L-carnitine, 5 micrometer) for 30 min. RESULTS: There were no differences in sperm parameters and vital stain among isolate only treated group, progesterone (1, 5 and 10 micrometer), acetyl-L-carnitine (2.5, 5 and 10 micrometer) and both (progesterone, 1 micrometer; and acetyl-L-carnitine, 5 micrometer). But, in high concentration of acetyl-L-carnitine (10 micrometer) treated group, sperm parameters and vital stain were decreased. The statistical method was used ANOVA (Kruskal-Wallis test) and p value was <0.01. CONCLUSIONS: Neither progesterone nor acetyl-L-carnitine show to be protective effect on the cryodamage assessed by sperm parameters and vital stain (eosin-Y stain) in normal sperm. High concentration of acetyl-L-carnitine (10 micrometer), however, was harmful effect on cryoprevention.
