1.The Clinical Study on Pin and Plaster Method about open Tibial Shaft Fractures

Recently, the rates of various traffic accidents and industrial injuries accordlng to industrial progression were increased very much. In those injuries, open tibial shaft fractures were one of the most common ones of all long bones of the body. The authors have reviewed and analyzed 30 cases of the open tibial shaft fractures which were treated by pin and plaster method at our hospital from April 1976 to August 1979. The results were as follows: 1. Most of the fractures occurred in 20 and 30 age levels, average age was 34.4. Male 5 > female 1 in accident frequency. 2. The causes were traffic accident (67%), direct blow (17%), falling down from a height (13%), and mechanical trauma (3%) in order. 3. In location of fractures, Lt. 16 > Rt. 11 > both. 3; lower (47%), middle (33%), upper (20%), one thirds and comminuted (60%) > transvers (20%) > oblique (13%) > spiral (4%) > segmental fractures in frequency. 4. The associated injuries were fractures of fibulas (67%) > multiple skin damages (40%) > knee joint patella injuries... etc. Total of those injuries were 68.; One patient gained 2.26 kinds of those injuries simultaneously. 5. By classification of Ellis, minor severity (33%) > moderate one (40%) > major one fractures (27%). And according to results of pin and plaster treatment, bony union took roughly 11, 15, above 23 weeks respectively. 6. The summed contaminatlon and infection rate was below 20%, and cultured bacterias were E. Coli (50%) > staphylocci (33%) > pseudomonas (17%) and susceptabilities to penicillin were 60, 90, 25% respectively. 7. By classification of time interval from injury to apply pin and plaster method, early applied group (17 cases) showed bony union rate (7.6.4%), delayed one (11.7%), nonunion (5.8%), late group showed 50, 12.5, 6.3%, far late group showed 40, 40, 20% respectively. 8. Duration of the cast immobilization was 4–7 weeks and then it was changed by P. T. B. cast until about 4 monthes from injury, after it, vigorous P. T. was carried out. Good results were come in 80% (24 cases). 9. Pin and plaster method will be indicated in minor & moderate severity-fractures, but in major ones, it will be used as a preliminary stage operation.

2.Three Cases of Aneurysmal Bone Cysts

Aneurysmal bone cyst is an uncommon tumor-like lesion of the bone containing distended vascular channels or cvities filled with blood. These cases, twenty-nine old female, eighteen years old male and female patients complained of pain and swelling in Rt. Medial aspect of proximal foot, Lt. proximal tibial portion and Rt. entire talar aspect of foot, respectively. Roentgenograms showed ballooned-out distensions and eccentric buldging of those lesions and soapbubble like multiple radiolucent cystic changes within interiors of the bones at talar neck, Proximal tibia and entire talus. Those cases were treated with radical curettage and autogenous iliac bone grafts and snugly fitted long leg casts in functional positions, respectively. On 2nd postoperative day, Cast windows were made and daily sterile dressings were carried out until complete wound healing. Pathological Examinations grossly and Microscopically revealed typical Aneruysmal bone cysts in all 3 cases. All-Patients had good therapeutic recovery within about seven weeks post-operatively.

3.Comparative Study of Combined Cataract Surgery in Primary Angle-Closure Glaucoma.

Trabeculectomy, as an operation precedure for glaucoman, decrease intraocular pressure(IOP)effectively. But the amount of decrease in IOP is different according to the etiology of glaucoma. Thirty four primary angle-closure glaucoma patients (37 eyes)who had undergone trabeculectomy only or trabeculecomy combined with cataract surgery and had been subsequently followed up for least 6 months, were enrolled in this study. In 12 patients(14 eyes) among 16 patients (18 eyes, Group 1) who had undergone trabeculecto-my only, IOP was controlled without glaucoma medication, while glaucoma medication was needed in the other 4 patients (4 eyes). Among 18 patients (19 eyes, Group 2) who had undergone trabeculectomy combined with cataract surgery, IOP were controlled without glaucoma medication 9 patients (10 eyes); controlled with medication in 8 patients (8 eyes); and not controlled even with 1 patient (1 eye). The amount of decrease in IOP was no statistically different between the two groups. Group 2 had deeper anterior chamber than Group 1 (p<0.01). The above results suggest that the combined pro-cedure may benefit in primary angle-closure glaucoma patients, especially old complicated patients.

4.A Study on the CD34 Expression of Cutaneous Benign and Malignant Vascular Tumors.

BACKGROUND: CD34 is a 115 kD glycoprotein which is expressed on hematopoietic progenitor cells. It is also known as an immunohistochemical marker of endothelial cells. OBJECTIVE: This study was designed to investigate the patterns of CD34 expression on: (1) cutaneous benign and malignant vascular tumors and (2) on the mature and immature vessels of pyogenic grauloma and capillary hemangioma. METHOD: We performed immunoperoxidase staining using a monoclonal anti-CD34 antibody (QBEND/10) on formalin fixed, paraffin-embedded sections of 23 benign and malignant cutaneous vascular tumors. RESULTS: The results are summerized as follows: 1. In 3 cases of nevus flammeus and 6 cases of carvernous hemangioma, vascular endothelial cells of all hemangiomas showed CD34 expressions. In 5 cases of angiokeratoma, endothelial cells of hemangioma, did not express CD34. 2. In all 5 cases of pyogenic granulomas and one case of capillary hemangioma, endothelial cells of mature vessels, endothelial cells near the well-formed lumina and endothelial cells showing intracellular lumina showed strong positivity for CD34, wbile endothelial cells far from the lumina and endothelial cells without lumina formation mostly showed negative staining for CD34. 3. One cese of Kaposis sarcoma showed focall positivity for CD34 both in endothelial cells of the small, well-formed vessels and spindle cells. Two cases of angiosarcoma showed CD34 expression only in endothelial cells of well-formed, normal appearing vessels, whereas atypical endothelial cells of tumor vessels and spindle cells were negative for CD34. CONCLUSION: CD34 could be a marker for endothelium in mature, well-differentiated vascular structures and may serve as a marker of lumen formation or differentiation of endothelial cells.

5.The Study on Relaxed G-tolerance Beween Men and Women.

The relaxed G-tolerances of 16 female cadets of ROKAF academy and 19 male privates were compared. The relaxed G-tolerance level was measured as visual endpoint (gray out or loss of peripheral light loss) during G-force raise steadily at 0.1 G/s. The G-stimulations were repeated twice with the interval of 30 s for 4 days (female cadets, 9 a.m.; male privates, 10 a.m.). The correlation between the relaxed G-tolerance level and their physical factor (height, sitting height, weight and lean body mass) and muscle strength (power of abdomen, back and thigh) was investigated. The average value of the relaxed G-tolerance levels of female cadets for 4 days was significantly higher than the one of male privates (4.23 +/-0.60, n=111; 4.04 +/-0.64, n=92; student 's t-test, P<0.05). However, the training effect of 4 days was not seen in either female cadet or male private group. The weight and lean body mass have significant positive correlation with the relaxed G-tolerance level. Also, the peak torque of thigh muscle have a strong positive correlation with it, but the endurance ratio does not.

7.Effectiveness of Argon Laser Peripheral Iridoplasty in the Treatment of Severe Acute Angle-Closure Glaucoma.

We used argon laser peripheral iridoplasty to reduce intraocular pressure[IOP] in acute angle-closure glaucoma[ACG] which is unrelieved by medical and physical treatments and could not be treated by peripheral iridotomy due to edematous cornea and turbid aqueous. Eight of 16 eyes [16 patients] were successfully treated. These successfully treated eyes had a mean duration of angle closure of 4.1 days and 8 unsuccessfully treated eyes 9.2 days. The 6 eyes of 8 eyes with successful reduction of IOP after argon laser peripheral iridoplasty were treated peripheral iridotomy at average 1.7 day later and had deep anterior chamber. After a mean follow-up period of 2 months, 7 of these 8 successfully treated eyes had an IOP less than or equal to 21mmHg. Argon laser pripheral iridoplasty may be successful in treating ACG which is unrelieved by medical and physical treatments and could not be treated by pripheral iridotomy, especially in cases that are recognized and treated soon after onset.

8.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.

9.The Relationship between Idiopathic Polyhydramnios and Oligohydramnios and Perinatal Outcomes.

OBJECTIVE:Ultrasound estimation of amniotic fluid volume (AFI) is a critical component of antenatal surveillance. Alterations in AFI have classically been considered an indication of fetal compromise. Thus, this study was undertaken to determine whether there is any association between idiopathic polyhydramnios or oligohydramnios and adverse perinatal outcome. METHODS:Women delivered between December 1999 and November 2005, and who underwent ultrasonography from 26 to 41 weeks gestation were analyzed. Twenty-seven consecutive women with singleton pregnancies complicated by idiopathic polyhydramnios (amniotic fluid index>25) and 31 women complicated by oligohydraminos (amniotic fluid index<5) were included in this study. We reviewed the perinatal outcomes including preterm delivery, birth weight, primary cesarean section rate and indication, Apgar score (1-min and 5-min), and admission to NICU (neonatal intensive care unit). These findings were compared with those of 50 matched control patients with normal amniotic fluid volume. RESULTS:We observed that idiopathic oligohydramnios was significantly associated with primary perinatal outcome (Cesarean delivery for fetal distress) as well as secondary outcome variables such as birthweight, SGA (small for gestational age), Apgar score<7 at 1-min, 5-min, and NICU admission rates. On the other hand, idiopathic polyhydramnios did not correlate with any other perinatal outcome, except for rates of LGA (large for gestational age). CONCLUSION:The present study suggests that AFI is a weak predictor of perinatal outcome than has been classically suggested. But we demonstrated that the AFI identification of polyhydramnios was helpful in identifying LGA, and idiopathic oligohydramnios was a significant predictor of poor perinatal outcome.

10.Chronological observation on intestinal histopathology and intraepithelial lymphocytes in the intestine of rats infected with Metagonimus yokogawai.

The relationship between the intestinal histopathology and number and position of intraepithelial lymphocytes (IEL) was observed chronologically in the small intestine of rats experimentally infected with Metagonimus yokogawai. Fifteen Sprague-Dawley rats were orally infected each with 3,000 metacecariae, and 3 were kept uninfected for controls. Three rats each were sacrificed on the day 5, 10, 15, 24 and 70 post-infection (PI) and samples of the small intestine, 5 cm, 10 cm, 20 cm and 70 cm posterior to the pylorus were taken. The samples were processed routinely and stained with Giemsa. The intestinal histopathology was severe during the day 5-15 PI and characterized by villous atrophy, crypt hyperplasia, and decrease of villus/crypt height ratio. After the day 24 PI, the intestinal lesions showed some tendency of recovery. The number of IEL increased at the early stage of infection, but decreased thereafter to a lower level than that of controls, with progression of the pathological changes. Then, the IEL number began to increase again after the day 24 PI. In control rats, the great majority of the IEL were located at the basal region of the epithelium. During the early stage of infection, however, a considerable proportion of IEL was found to have moved to the intermediate or apical region of the epithelium. From the above results, it is suggested that the change of IEL number and position during the course of M. yokogawai infection should be closely related to the progression and recovery of the intestinal histopathology.
