1.Parosteal Ossifying Lipoma of Femur: A Case Report.

Parosteal lipoma is a rare and generally asymptomatic benign tumor of mature adipose tissue that is located in direct apposition to the external surface of the bone. These tumors are occasionally associated with reactive changes in the underlying bone. The reactive bone formation is generally restricted to the base of the tumor, that is, near the periosteum. We recently experienced a case of parosteal lipoma in the proximal femur, which displayed exuberant bone and cartilage formation, and this led us to a misdiagnosis of chondroblastic osteosarcoma on the initial biopsy. We report here on this case with a special emphasis on making the differential diagnosis from osteosarcoma.

2.Analysis of salpingeal patency in young-aged women underwent diagnostic laparoscopy with Fitz-High-Curtis Syndrome.

OBJECTIVE: To analyze the tubal patency in the young-aged women underwent diagnostic laparoscopy with Fitz-High-Curtis Syndrome (FHCS). METHODS: Clinicopathologic results of young-aged women who underwent diagnostic laparoscopy with pelvic inflammatory disease from March 2005 through April 2007 were reviewed. Twenty six patients aged 19-29 years old and preserved their both tubes after diagnostic laparoscopy were included in this study. All were underwent chromopertubation test (CPT) during laparoscopy and postoperative hysterosalpingography (HSG) 3 months later. RESULTS: Of 26 patients, eight patients were diagnosed with FHCS and the others were not. There was no significant difference in mean age, mean hospitalization days, and frequency in past-history of parturition, abortion, chlamydia and gonococci infection between patients with FHCS and with non-FHCS (P>0.05). There was no significant difference in frequency of tubal obstruction in laparoscopic CPT (P>0.05) but not in postoperative outpatient HSG (P<0.05). CONCLUSION: The tubal obstruction may be more frequent in FHCS than non-FHCS and the reliability of laparoscopic CPT for diagnosing the tubal obstruction is supposed to be low. Therefore, as for FHCS patients, HSG should be executed to investigate tubal factor which is helpful to forecast the fertility.

4.Occupational Contact Urticaria Syndrome Induced by Cefotiam Dihydrochloride in a Nurse.

We herein report a case of occupational contact urticaria syndrome induced by cefotiam di-hydrochloride in a nurse. She had generalized pruritic wheals accompanied by palpitations and tachypnea during the preparation of cefotiam solution. A scratch patch test with cefotiam di-hydrochloride, a major component of cefotiam ingredient showed multiple erythema and wheals within 5 minutes, accompanied by palpitations and tachypnea. No delayed type reaction was observed. Based on her clinical history and scratch test result, we diagnosed her condition as contact urticaria syndrome caused by cefotiam.

5.A Case of Elastosis Perforans Serpiginosa.

We are reporting a case of 25-year-old male patient with elastosis perforance serpiginosa who showed multipte serpiginous plaques on his neck. Some of the lesions showed hypopigmented, hard, scar-tissue like plaques in the central part. Histologic examination showed hyperplasia and transepidermal elimination of elastic tissue. By electron microscopy, also, we could prove that the elastic fibers were increased in the dermis.

Twin reversed arterial perfusion (TRAP) sequence is a rare specific anomaly of twin gestation with fused placenta and umbilical anastomosis. This syndrome occurs once in about 34,600 births and reported first by Beneditti in 1533. We report on prenatal diagnosis of a case of TRAP sequence with color Doppler sonography, this case is presented with a brief review of the literature.

7.A Case of Idiopathic Lymphocytoma Cutis.

The pseudolymphoma of the skin has the architectural and cytological features of a neoplastic proliferation of lymphoid tissue but pursue a benign course. Cutaneous B cell pseudolymphoma (CBPL) shares many histopathologic and clinical features with cutaneous B cell lymphoma (CBCL). Therefore, the differentiation between CBPL and CBCL is often very difficult, but it is important because each of them has a different therapeutic consequence. Recently, immunoglobulin gene rearrangement is considered as a reliable technique for differentiation of CBPL with CBCL. We herein report a case of idiopathic lymphocytoma cuffs, showing a typical nodular infiltrate of lymphocytes that formed a follicular germinal center resembling reactive lymph nodes with numerous tingible bodies, and that revealed a polyclonality in the immunoglobulin gene rearrangement.

9.In Vitro Analysis of Histology, Mechanics, and Safety of Radiationfree Pre-hydrated Human Acellular Dermal Matrix

Purpose:
Acellular dermal matrix (ADM) supports tissue expanders or implants in implant-based breast reconstruction. The characteristics of ADM tissue are defined by the manufacturing procedure, such as decellularization, preservation, and sterilization, and are directly related to clinical outcomes. This study aimed to compare the properties of a new pre-hydrated-ADM (H-ADM-low) obtained using a decellularization reagent reduction process with a low concentration of detergent with those of radiation-sterilized H-ADM and freeze-dried ADM (FD-ADM).
Methods:
ADMs were evaluated in terms of structure, mechanical quality, and cytotoxicity using histochemical staining, tensile strength testing, and in vitro cell viability analysis.
Results:
The tissue structure of H-ADM-low (CGDERM ONE-STEP) was similar to that of native skin despite complete decellularization. By contrast, in FD-ADM, the tissue structure was damaged by the freeze-drying process, and radiation-sterilized H-ADM showed a compact fibrillar arrangement. Furthermore, matrix components such as collagen and elastin were preserved in H-ADM-low, whereas a loss of elastin fibers with fragmented distribution was observed in radiation-sterilized H-ADMs. H-ADM-low's tensile strength (58.84 MPa) was significantly greater than that of FD-ADM (38.60 MPa) and comparable with that of radiationsterilized H-ADMs. The residual detergent content in H-ADM-low (47.45 mg/L) was 2.67-fold lower than that of H-ADM decellularized with a conventional detergent concentration (126.99 mg/mL), and this finding was consistent with the cell viability results (90.7% and 70.7%, respectively), indicating that H-ADM-low has very low cytotoxicity.
Conclusions
H-ADM-low produced through aseptic processes retains the original tissue structure, demonstrates excellent mechanical properties, and does not affect cell viability.Therefore, this newer H-ADM is suitable for use in implant-based breast reconstruction.
10.Development of a Web-based Emergency Medical Information Service Program for Health Promotion.

BACKGROUND: Demand for emergency medical information is high according to the health information use, utilization and demand survey for on the Internet. In spite of high demand for emergency information service, emergency information service on-line and off-line are very limited. Therefore, it is necessary to develop a website providing emergency medical information and emergency medical resources information by linking emergency medicine where timely information is required with the Internet technology for health promotion of the public. OBJECTIVE: This study is conducted to develop and evaluate a Web site to provide information on emergency medicine and resources for health promotion. METHODS: To identify and specify users' requirements for the emergency medicine related information, online survey was performed for 209 Internet users who visited the Healthguide. Based on this survey results, we developed content for emergency medical information service: cause, risk factors, prevention, treatment, facilities, laws and rules, policy and statistics of emergency medicine. To evaluate the web site developed in this study, twenty health and nursing informatics experts were recruited and asked to rate using an evaluation tool. RESULTS: This content was integrated into the healthy life (http://healthguide.kihasa.re.kr/health_life/emergency/pages/index.html), which is a service component of the Healthguide. This approach was expected to increase accessibility and usability of implemented services. The rating score of the website was fairly high with mean 3.9(SD 0.6) out of 5.0. According to each evaluation criteria, purpose of the site, easiness of use, relevancy, and reliability were scored 4.1-4.2 and authority and accuracy 3.8-3.9 and currency 3.5-3.7. This evaluation results will be fed back to the Healthguide for operation, maintenance, and upgrade in the future.
