3.Primary Intrapericardial Lipoma Simulating Pericardial Effusion -Report of A Case-.

Primary pericardial tumors are rare than those originating within the myocardium or endocardium and, moreover, primary benign pericardial tumors are much rare in occurrence. Fine(1986) was able to collect only 43 cases of lipoma. Most cases are diagnosed only at autopsy. One fourth of the cardiac lipoma arise subepicardially, where they suggest the presence of pericardial effusion with compression or displacement of the heart. A case of primary intrapericardial lipoma simulating pericardial effusion is presented. This tumor originated in left anterior aspect of pericardium. The tumor was measured 30x30x5cm in size and 2,200mg in weight. Total excision of the mass was accomplished by posterior lateral thoracotomy incision, resulting in complete cure. This is the first case of primary benign lipoma of the pericardium in literatures in our knowledge.

4.Biological factors influencing the fate of onlay bone graft on the craniofacial skeleton.

The superior volume maintenance of membranous over endochondral bone grafts, which was shown in several studies has provided the basis for its preferred clinical use as an onlay grafting material on the craniofacial skeleton. The scientific rationale for this seeming embryological advantage, however, has never been proven, Since the cortical component of membranous bone is proportionally greater than that of endochondral bone, it follows that membranous grafts would show greater volume maintenance over time. Our hypothesis is that the pattern of onlay bone graft resorption is primarily determined by a graft's micro-architecture (relative cortical and cancellous composition) rather than its embryololgical origin(membranous versus endochondral). Fourty adult New Zealand white rabbits were used for this study. There were 8 animals in each of 4 groups. The rabbits of each group were sacrificed at 3, 8, and 16 weeks. Four types of grafts were placed subperosteally, onto each rabbit's cranium: a hydroxyapatite, a cortical bone graft of membranous origin, a cortical bone graft of endochondral origin and a cancellous bone graft of endochondral origin. Membranous bone grafts were obtained from the lateral mandible and endochondral bone grafts were obtained from the ileum. In order to determine post-sacrifice volume and density of the bone grafts, a caliper technique and bone densitometry(bone densitometer: LUNAR, DPX-L, U.S.A.) were performed on all of the bone grafts. Bone graft specimens were histologically examined at 3, 8, and 16 weeks.The measurement of volume and density show that there is a statistically greater resumption in the cancellous endochondral bone grafts for all parameter, compared to either the endochondral or membranous cortical bone grafts or hydroxyapatite at all time points(p< 0.05). In addition, there is no significant difference in the resorption rates between the endochondral and membranous cortical bone grafts for all parameters at all time points. By placing cortical bone grafts and cancellous bone grafts on the recipient sites separately, we have shown that the former grafts maintain their volumes, widths and projections significantly better than the latter grafts. Futhermore, we found no statistical difference in resorption rates between the two cortical bone grafts of different embryologic origins, a finding which has never been previously shown. Bone volume fraction, measured with bone densitometry, was shown to be higher in cortical bone than in cancellous bone at all time points, further illustrating the differences between cortical and cancellous bone.From our results, we believe cortical bone to be a superior onlay-graftiong material, independent of its embryololgic origin.

7.Functional Evaluation of Tumor Prosthetic Arthroplasty for the Malignant Bone Tumor around the Knee: Interim Report

With the recent development of tumor prosthesis, limb salvage technique has attributed to the preservation of the limb function in patients with malignant bone tumors around the knee without substantial difference of local recurrence and the survival rate. From Feb., 1991 to Sep., 1992, 9 patients were treated with limb salvage operation with total knee arthroplasty due to the malignant bone tumor around the knee at Dept. of Orthopedic Surgery, Kyungpook National University Hospital. We applied the above procedure mainly to young adult whose tumor was expected to be excised wide marginally without damaging major neurovascular structures. We evaluated the functional status of all 9 patients(4 osteosarcoma, 2 chodrosarcoma, 2 malignant fibrohistiocytoma, 1 malignant giant cell tumor) to estimate the efficacy of limb salvage operation with tumor prosthesis total knee arthroplasty. Mean follow up period was 10 months. Primary tumor site was 4 in distal femur, 5 in proximal tibia and stage was 4 IIa, 5 IIb. Range of the motion of knee is maximum 0°

8.Osteomalacia: Report of One Case

This presentation is a case of osteomalacia, the cause of which was a nutritional deficiency. The patient was a 42-year-old female who had pseudofractures of ribs, ulnae, pelvic raml, the right second metacarpai bone and both femoral necks. Following treatment by medication combined with internal fixation of the femoral necks, the result was good.

9.A Clinical Study on the Trochanteric Fracture of Adult

Complications after surgical treatment of trochanteric fracture such as varus deformity or delayed union are frequent and there are many difficulties to maintain the stability of fragments because of the cortical deficit or comminution on medial aspect of the neck and posterior trochanteric fragment. Evans, Dimon, Sarmiento and others suggested several methods of internal fixation, furthermore it is attempted recently to reduce the fragments anatomically with sliding compression hip screw and plate. 31 cases of interal fixation on trochanteric fracture were carried out from March, 1972 to February 1979 at the Orthopaedic Department of Seoul Adventist Hospital and their comparable results were as follows. 1. Acceptable placement of metals was noted 2 cases of 6 cases in Smith-Petersen nail with Thornton plate fixation, 2 cases out of 8 cases in Smith-Petersen nail with McLaughlin plate fixation and 9 cases out of 14 cases in sliding compression hip screw and plate fixation. 2. Complications after internal fixation such as varus deformity and delayed union were noted 1 case out of 3 cases in multiple pinning, 3 cases out of 6 cases in Smith-Petersen nail with Thornton plate fixation, 4 cases out of 8 cases in Smith-Petersen nail with McLaughlin plate fixation, 4 cases out of 14 cases in sliding compression hip screw and plate fixation. 3. Required weeks in union were 18 weeks in Multiple pinning, 20 weeks in Smith-Petersen nail with Thornton plate fixation, 25 weeks in Smith-Petersen nail with McLaughlin plate fixation and 16 weeks in sliding compression hip screw and plate fixation. 4. There are few complications in treatment of trochanteric fracture with surgical procedure using Sliding compression hip screw and plate.

10.A Clinical Study on the Fractures of Tibia with Severe Compounding and Soft Tissue Injury

Fractures of tibia are often trouble-some because of their easy compounding and associated soft tissue injury, especially neurovascular tissue. Initial proper treatments for fractures of tibia with severe compounding are very important for a better prognosis. In the orthopaedic Department of the Seoul Adventist Hospital, open reduction and internal fixation with Sherman plate were performed for 12 cases of tibia with severe compounding from January 1975 to October 1980 and the results were as follows; 1. The age of the patients ranged from 11 years to 50 years, and the majority of the patients were injuryed in traffic accident. 2. All of the patients were suffered from combined injures, so intensive close treatment was needed but 2 patients expired. 3. 7 out of 12 cases were found as vascular rupture and therefore 2 B-K amputations were carried out because of necrosis. 4. Average time for bone union was 6.9 months and infection was controlled without sequela. 5. Outcome of open reduction and internal fixation with Sherman plate for fractures of tibia with severe compounding was rather good, but comparisons are thought to be required after standardization of patients for open reduction without internal fixation. and openreduction with internal fixation.
