Down syndrome is the most common autosomal chromosomal abnormality characterized by mental and growth retardation, and by various typical features including prominent epicanthal fold, oblique palpebral fissure, flat nasal bridge, short and broad hand, wide toe interspace, etc. The overall incidence has been shown to be 1:800 deliveries, increasing with advancing maternal age. However, twin cases are extremely rare, and thus far only 500 cases were reported worldwide. We have recently observed 10-day-old male monozygotic twins with Down syndrome, born to a mother of 30 years of age with one normal child. Both have VSD confirmed by 2D-echocardiography, in addition to various typical features. Cytogenetic examination revealed that both have karyotypes of 47, XY, +21. This is the first report in Korea as the authors are aware of.

We report magnetic resonance imaging findings of massive spinal subarachnoid hemorrhage (SAH) caused by repeated lumbar punctures during spinal anesthesia in a 36-year-old man. The signal intensities of spinal SAH were similar to those of the conus medullaris on both T1 and T2- weighted spin-echo images. Although spinal SAH is hardly recognized on MR, spinal SAH of sufficient amount may cause alteration of the cerebrospinal fluid signal.

Presentation of research work is an important first step toward effective scientific communication of the research results. To determine the current puhlication patterns of the paper presented at the annual fall congress of the Korean Orthopaedic Association, we analysed 357 orally presented ahstracts at the 38th annual fall congress of the Korean Orthopaedic Asssociation which was held in 1994. This search revealed that full-length articles of 156 ahstracts (44%) were puhlished in the Journal of the Korean Orthopaedic Asssociation (121 ahstracts, 34%) and the Journals asssociated with the Korean Orthopaedic Asssociation (35 ahstracts, I 0%). The number of published papers was greatest in fracture part, followed by spine part, hip part, etc. It took ahout 13 months mean interval to publish at journal since oral presented date. The puhlication rate 44% was relatively low compared with the reports of various foreign disciplines. So endeavors to increase publication rates of orally presented papers will he needed.

Transcatheter embolization was done in 15 patients wtih various diseases at departement of radiology, SeoulNational University Hospital from Sept. 1978 to July 1983 using absolute ethanol, autologous blood clots andgelfoam particles as embolic agents. The results were as follows; 1. Successful embolizations were done in 14 of15 cses. (Success rate:93%). 2. Most of complication was post-embolization syndrome such as fever, nausea, painbut minimal. Serious complicaiton was very little. 3. Transcatheter embolization appears to be highly valuableadjuvant therapy in debulking of preoperative and inoperable hypervascular mass or treatment of bleeding fromvarious etiology.

Transcatheter arterial embolization was applied in 7 cases of hepatic disease including hemobilia of various causes and malignant tumors at Department of Radiology, Seoul National University Hospital during recent several years. The embolic materials were autologous blood clot and Gelfoam particle. Successful control of bleeding or devascularization of tumor vessel was made in 6 of the 7 patients. There was no serious complications; however, post embolization syndrome was found including pain, fever, nausea and vomiting in all cases and subsided after a few days. Transcatheter hepatic arterial embolization is suggested to be a safe and effective treatment in control of hemobilia and devascularization of malignant hepatic tumors.

There is much controversy concerning the method of treatment on the tibial fracture because of frequent complications, such as non-union, delayed union and infection. Authors reviewed and analyzed 110 tibial fractures which had been treated at the Department of Orthopedic Surgery, Sung Ae General Hospital from June 1983 to March 1987 by the groups, conservative treatment, open reduction and internal fixation with compression plate and closed flexible intramedullary nailing. The results were as follows :1. In regard to the fracture location, there was no difference in the healing time among the treatment methods. The I.M. nailing group healed 1.5 weeks earlier than other groups in each location. 2. In minor and moderate severity by Ellis, the I.M. nailing group revealed better result, and in major one, the compression plate group showed better result. More complications occured in major severity. 3. Union time among the trestment methods was not affected by the fibular fractures, but most of the complications occured in the concomitant fibular fracture case. 4. In open fracture, the healing time showed no difference among the groups, and was prolonged about 2 weeks than closed fracture. In closed case, the I.M. nailing group showed good result. 5. The cases, which were reduced earlier in the conservative group, and treated within 3 days in the I.M. nailing group revesled better result. 6. The radiological union time from the definite treatment was 15.80 weeks in the conservative treatment group, 15.18 weeks in the compression plate group and 13.92 weeks in the I.M. nailing group.

Congenital dislocation of the knee is very rare deformity and presents with anterior displacement of the tibia in relation to the femur. Early gentle manipulation and serial splintage or plaster cast is recommended for treatment and successful in the majority except the case of quadriceps contracture or late correction. We experienced two cases of bilateral congenital dislocation of the knee who had been treated with early gentle manipularion, immobilization with serial corrective dual splint, passive stretching and night splint. Our results are good in both cases.

BACKGROUND: Members of the genus Malassezia, lipophilic yeasts, are considered to be one of the exacerbating factors in atopic dermatitis. When the response of anti-inflammatory treatment of atopic dermatitis to adults was poor, antifungal treatment had a good response to atopic dermatitis. We compared Malassezia species detected in head and neck lesions of atopic dermatitis between children and adults. OBJECTIVE: To compare Malassezia species between children and adults in head and neck lesions of atopic dermatitis and to reveal the correlation levels of P. ovale specific IgE and clinical severity among these groups. METHODS: Malassezia species were incubated in Leeming and Notman media from skin lesions of 28 patients with head and neck lesions of atopic dermatitis (13 children, 15 adults). And ITS1 amplification and sequencing method was done for detection of Malassezia species. We also assayed P. ovale specific IgE and clinical severity grade of the patients. RESULTS: M. globosa was the predominant species in the children group. And M. furfur was the predominant species in the adults group respectively. The levels of P. ovale specific IgE increased more in the adults group than the children group. Clinical severity grade also more increased in the adults group. There was statistically significant correlation between P. ovale specific IgE and clinical severity grade (p<0.001). Conclusion: Malassezia species are aggravating factors in head and neck lesions of atopic dermatitis particularly in adults. We must keep in mind Malassezia species infection when resistance to antiinflammatory therapy in atopic dermatitis of adults.
