1.A clinical study about the maxillectomized patients and obturators by maxillary cancer.

During 10 years (from 1986 to 1995), 54 patients were diagnosed as maxillary cancer and undergone maxillectomy in department of Oral and Maxillofacial Surgery, Korea Cancer Center Hospital. We studied clinically about their age and sex distribution, TNM staging, histopathologic diagnoses, treatment modalities, overall survival rates, survival rates according to stages and treatment modalities. In them, 23 patients were delivered maxillary obturators, we also studied clinically about them and their obturators. The overall 3-year and 5-year survival rates of these maxillectomized patients were 59.0% and 55.0%. The 5-year survival rates of stage II and stage III patients was 63.0%, and that of stage IV patient was 50.0%. According to treatment modalities, The 5-year survival rates in cases of surgery only was 66.7%, in cases of preoperative chemotherapy and/or radiaiotherapy was 63.5%, in cases of postoperative radiotherapy and/or chemotherapy was 50.4%. The 5-year survival rate of obturator delivered patients was 56.8%, and that of no-obturator delivered patients was 51.8%. The using rates of obturator were 70.5% at 1 year, 59.7% at 2 years, and 53.0% at 3 years.

6.The Effect of Equilibration Temperature and Exposure Time on the Ultrarapid Freezing of 1-cell Mouse Zygote.

The present study was to assess the effect of ultrarapid freezing on the development of 1-cell mouse zygote using cryoprotectants, DMSO (dimethyl sulfoxide) or PROH (1,2-propanediol). We investigated the effect of the type and concentration of cryoprotectant, and of the temperature and time of prefreezing equilibration on their capacity to develop to the blastocyst stage in vitro. The concenration, the equilibration temperature, and the exposure time seemed to serve as an important factor in ultrarapid freezing of 1-cell mouse zygotes. In addition to the exposure time and the concentration of cryoprotectant appeared to play a key role in the development of the embryo. In general, the development of the embryo was more effective at 3degrees C than 23degrees C and 4.5 M than 3 M for 3 to 5 minutes. At 23degrees C the development of the embryo was stimulated by DMSO while at 3degrees C it was stimulated by PROH. Thus it has been suggested that there exists a correlation between the concentration of cryoprotectants and exposure time in the development of the embryo. In conclusion, we found that for ultrarapid freezing of mouse 1-cell embryos in DMSO, or PROH-based solution, viability shown optimum depending on the cryoprotectant, the concentration of the cryoprotectant and on the temperature and the duration of equilibration.

9.Trans - Posterior Septum Posterior Arthroscopic Portals in the Knee Joint.

This paper introduces a safe arthroscopic technique passing through the posterior septum from the posteromedial cotnpartment to the posterolateral compartment, or vice versa, without damage to the PCL, capsule, and/or neurovascular structures. The posterior septum is triangular, bounded by the PCL anteriorly, the posterior portion of the femoral I.ntercondylar notch superiorly, the posterior capsule posteriorly, and devicles the posterior compartment of the knee joint into the posteromedial and posterolateral compartments. The middle genicular vessel pierces the posterior capsule and runs along the superior border of the posteriorseptum. There are no dangerous important structures at the central portion of the triangle of the posterior septum. This arthroscopic posterior portal technique is devided 4 steps. The first step is to make a posteromedial portal under direct arthroscopic visualization. The second step is to make a posterolateral portal. The third step is to make a hole at the posterior septum and to examine throposterolatera] compartment. The fourth step is to examine the posteromedial compartment through the posterior septum. This technique provides complete visualization of the posterior compartment of the knee joint, including the posterior aspect of the both femoral condyles, the posterior hom of both menisci, the PCL, the meniscofemoral ligament (Wrisberg s ligament), the posterior aspect of the pop liteal tendon, the femoral attachment of the ACL, the posterior septum and the posterior capsule. We have used this technique from October, 1994 to June, 1996 in 58 knees; arthroscopic total synovectomy in 43, removal of the loose bodies located behind the PCL in 8, PCL re!construction in 4, meniscal repair with all inside suture technique in 3. In conclusion, this procedure is very helpful in performing arthroscopic total synovectomy in chronic, acute, hemophilic, and crystal induced synovitis, in removing encapsulated loose bodies located behind the PCL, and in suturing peripheral tears of the posterior hom of the medial rneniscus with an all inside suture technique.

10.Osteomalacia: Report of One Case

This presentation is a case of osteomalacia, the cause of which was a nutritional deficiency. The patient was a 42-year-old female who had pseudofractures of ribs, ulnae, pelvic raml, the right second metacarpai bone and both femoral necks. Following treatment by medication combined with internal fixation of the femoral necks, the result was good.
