Phakomatosis pigmentova.scularis is a disease complex with dermal melanocyto sis such as nevus of Ota, nevus of Ito, or Mongolian spots and extensive hemangiomatous change such as nevus flammeus, which are associated with systemic organ involvement. We report a 23-year-old female, who showed bilateral nevus of Ota and nevus of Ito with widespread nevus flammeus and Mongolian spots since birth. She was also associated with megalocornea due to congenitol glaucoma and cataract.

2.Experience in Treating COVID-19 Elderly Hip Fracture Patients during the Explosive Epidemic of COVID-19 at a COVID-19 Dedicated Hospital

Purpose:
The purpose of this study was to prepare guidelines for the treatment of COVID-19-positive hip fractures based on the treatment experience of patients with COVID-19-positive hip fractures admitted to a COVID-19-dedicated hospital.
Materials and Methods:
A retrospective analysis was performed on COVID-19-positive elderly hip fracture patients treated at a COVIDdedicated hospital when the number of domestic COVID-19 infections increased rapidly. The treatment results were analyzed by comparing the complications of patients who underwent surgery at a COVID-dedicated hospital and those who did not, the time taken from surgery to surgery, and the number of visits to medical institutions.
Results:
The average surgery time for COVID-19 hip fracture patients in the author’s hospital was 3.74 days from the date of injury, and it took an average of 12.8 days for surgery at other hospitals. The average number of patients who visited medical institutions was 3.33 locations in the group who did not undergo surgery at the author’s hospital, which was significantly higher than the 2.83 locations in the group who had surgery. Among the patients unable to undergo surgery, no patient could not undergo surgery due to serious reasons.
Conclusion
The explosive increase in COVID-19 infections has delayed the time of surgery for COVID-19-positive elderly patients with hip fractures and increased the transfer rate of medical institutions, even for relatively non-dangerous causes. Clear criteria for treatment and effective policies are needed to prevent delay delays in treatment.
3.Diagnosis of Incomplete Kawasaki Disease in Infants Based on an Inflammation at the Bacille Calmette-Guerin Inoculation Site.

BACKGROUND AND OBJECTIVES: This study was intended to test how the inflammation at the Bacille Calmette-Guerin (BCG) inoculation site (BCGitis) can be a useful a diagnostic feature of Kawasaki disease (KD). SUBJECTS AND METHODS: All subjects were infants at the time of admission, and had received BCG vaccination during their neonatal period. There were 54 patients with complete KD (group 1) and 29 patients with incomplete KD (group 2). All 83 patients had BCGitis during the acute phase of illness. Data regarding the coronary artery diameters in 31 age-matched controls were used for comparison. RESULTS: The 2 patient groups did not differ in clinical and laboratory variables. During the acute phase, the median z scores of the left anterior descending coronary artery (LAD) diameter were 0.20, 0.42, and -0.48 in groups 1, 2, and control respectively, and that of right coronary artery (RCA) diameters were -0.15, -0.16, and -1.17 respectively. The z scores in both patient groups were greater than those in controls (p=0.0014 in LAD and p<0.0001 in RCA between group 1 and controls; p=0.0023 in LAD and p<0.0001 in RCA between group 2 and controls). A similar pattern was observed during the subacute and convalescent phases. CONCLUSION: BCGitis is a useful feature in the diagnosis of incomplete KD in infants who received BCG vaccine during neonatal period.

4.Clinical Observation of Tumors in Children: A second reports of 402 cases.

A total of 402 cases of tumors in children under 15 years of age diagnosed at NMC. During a period of 10 years from Jan. 1996 to Dec. 1975 were analysed clinically and the following results were obtained. 1) 402 tumors were divided into 185 malignant, 72 bign and 25 unclassified tumors. 2) The age distribution showed more higher frequency in the age group of 0~5 than each age group of 6~10 or 11~15 but not significantly. 3) The sex ratio between male and female was same in begin but male preponderanc in malignant tumors. Age group did not affect the sex ratio. 4) Among 402 tumors, leukemia was the commonest neoplasm showing 21% with 84 cases, and somatic soft tissue(57 cases), larynx and lung (36 cases), genitourinary system (35 cases) and skin (29 cases) in given order of frequency. 5) Among 192 begin tumors, the tumors of somatic soft tissue was the commonest(49 cases), and larynx and lung(36 cases), skin(28 cases), G-I tract(15 cases) in gven order of frequency. 6) Among 185 malignant tumors, leukemia was the commonest(45.4 %) and CNS(15.1%), GU system(8.1%) and malignant lymphoma(7.5%) in given order of frequency.

5.Clinical and Radiological Analysis of THR Using AML Prosthesis

We had replaced 159 hips in 130 patients with cementless porous-coated AML porsthesis at Kwang Ju Christian Hospital between April 1987 and Feb. 1992. Among them, 100 cases(80 patients) could be analyzed clinically and radiologically with an average follow-up of 38.5 months(range 10 months to 74 months). The purpose of this study was to evaluate the relationships of initial fit, follow up stability and clinical results. The results were as follows. 1. The average postoperative Harris hip score was 90 points(preop. 42) at the follow-up of 1 year or more, the pain score 41 points(preop. 15). 2. When the surgical fit of stem was achieved properly, fixation by bone ingrowth could be expected. Simultaneously adequate fit of acetabular cup provided stable fixation. 3. The incidence of thigh pain was high in patients with an unstable implant of stem and the hip pain was more common in group of a definitely unstable cup. 4. Stress shielding was seen in 44 cases(44%). Of the 44 cases, 38 cases(86%) revealed in bone ingrowth. In conclusion, this study showed good clinical result of THR using AML prosthesis at the average follow-up of 38.5 months and it seemed to be influenced by initial fit significantly. But long-term follow-up is needed for accurate clinical and radiological evaluation.

6.Screening for Atrial Fibrillation Using a Single Lead ECG Monitoring Device

Atrial fibrillation (AF) is the most common arrhythmia which needs management for stroke prevention. Therefore, it has emphasized the importance of screening for general population to detect AF earlier. We conducted screening for AF in the Chonbuk region in South Korea. Participants who were older than 50 years were enrolled. The screening test used a single lead electrocardiography (ECG) (KardiaBand, AliveCor, CA, USA). Diagnosis of AF was confirmed by electrophysiologists, if the single lead ECG demonstrated AF of more than 30 seconds. We analyzed the prevalence of AF and the characteristics of newly detected AF patients. A total of 2728 participants, 145 (5.3%) participants had already been diagnosed with AF before. The number of screening positive was 55. Among them, 40 participants were confirmed for AF. Male gender and age older than 70 years were the independent risk factors for AF among the screening positive participants. Most of newly detected AF patients were at high risk for stroke which had more than 2 points on the CHA2DS2-VASc score. We followed up with those patients and encouraged them to visit the hospital. As a result, 31 (77.5%) patients started to manage AF. The additional 1.2% of AF was detected by a screening test with a single lead ECG monitor device. Considering most participants of newly detected AF by screening were at high risk for stroke, it was thought that AF was still undertreated. Therefore, screening tests with simple mobile device might be useful for early detection of AF.

9.Log-transformed plasma level of brain natriuretic peptide during the acute phase of Kawasaki disease is quantitatively associated with myocardial dysfunction.

PURPOSE: Brain natriuretic peptide (BNP) has been considered a biochemical marker for myocarditis in Kawasaki disease. We performed this study to determine its quantitative significance. METHODS: We attempted to correlate log-transformed BNP concentrations (log-BNP) and clinical, laboratory, and echocardiographic variables in 81 children with Kawasaki disease. Stepwise multiple linear regression analysis was used to determine the variables independently associated with log-BNP concentration. RESULTS: Serum C-reactive protein level (P<0.0001), serum alanine aminotransferase concentration (P=0.0032), white blood cell count (P=0.0030), and left ventricular mass index (P=0.0024) were positively related with log-BNP, and hemoglobin level (P<0.0001), serum albumin level (P<0.0001), Na+ concentrations (P<0.0001), left ventricular fractional shortening (P=0.0080), and peak early diastolic tissue velocity of the left ventricular basal lateral segment (P=0.0045) were negatively related to the log-BNP concentration. Multiple regression analysis showed that serum albumin concentration (R2=0.31, P=0.0098) and left ventricular mass index (R2=0.09, P=0.0004) were significantly associated with the log-BNP concentration. CONCLUSION: Elevated BNP levels during the acute phase of Kawasaki disease may be attributable to cardiac dysfunction associated with the increase in left ventricular mass, and log-BNP concentration may be a quantitative biochemical marker of myocarditis in Kawasaki disease.

10.Availability of the Optic Nerve Sheath Diameter Measured by Using Facial CT as a Predictive Factor for Traumatic Brain Injury in the Emergency Department.

PURPOSE: Traumatic brain injury (TBI) occurs frequently in patients with facial injuries. In patients with facial injuries without neurologic deficit, it is a challenging decision for emergency physicians whether brain computed tomography (CT) is necessary or not. Our objective is to evaluate the availability of optic nerve sheath diameter (ONSD) measured by facial CT in predicting TBI. METHODS: From January, 2010, to December, 2012, we conducted a retrospective study of patients with head or facial injuries who underwent both facial CT and brain CT simultaneously in the emergency department. Patients with obvious orbital trauma or ocular disease were excluded. We analyzed correlation between ONSD measured by facial CT and the brain CT findings of TBI. RESULTS: A total of 229 patients were enrolled. Ninety six patients without TBI on facial CT showed mean ONSD of 5.11+/-0.44 mm, and 82 patients with TBI showed mean ONSD of 5.89+/-0.78 mm. The sensitivity and the specificity compared with the presence of TBI findings on brain CT were 78.05% and 82.29%, respectively, when the cut-off value was set to 5.5 mm. The area under the curve (AUC) was 0.826 in the receiver operating characteristic curve (ROC curve). CONCLUSION: ONSD measured on facial CT is available for predicting TBI in patients with facial injuries.
