3.A Case of Myocardial Ischemia and Myocardial Injury Caused by Coronary Vasospasm Associated with Hyperthyroidism.

There are many kinds of cardiac complications of hyperthyroidism such as angina pectoris, myocardial infarction, atrial fibrillation, congestive heart failure, heart block and sudden death. While angina is not uncommonly seen in association with hyperthyroidsm, it is rare that myocardial ischemia or infarction may be caused by coronary vasospasm. A 46 year-old man, who had been diagnosed with hyperthyroidism at the first admission, presented with chest pain. The serum thyroid hormone was still elevated. Serial ECGs showed significant ST elevation suggesting transmural ischemia, but coronary angiogram did not reveal any significant lesion. ECG at discharge was normalized without evidence of myocardial infartion. We report this case as an example of severe transmural ischemia with myocardial injury caused by coronary spasm associated with hyperthyroidism.

4.Marchiafava-Bignami Disease with Abnormal PET Findings: Case Report .

We report the FDG PET findings in a patient with Marchiafava-Bignami disease (MBD) in whom there was diffusely reduced metabolism in the whole brain cortex and strongly decreased metabolism in the thalami. The use of FDG PET helps provide an understanding of the neurologic manifestations and prognosis of MBD.

8.Role of Serotonin in Pathophysiology and Treatment of OCD.

The clinical efficacy of serotonin reuptake inhibitors such as clomipramine in the treatment of obsessive compulsive disorder(OCD) has fueled interest in the neurobiological basis of the illness. OCD is responsive exclucively to potent serotonin reuptake clomipramine, fluoxetine, fluvoxamine, sertraline, and paroxetine and the point forms the important evidence supporting a cental role for serotonin in the pathogenesis of the disorder. Other serotonergic medications such as lithium, buspirone, trazodone, or fenfluramine may be useful as adjuvant treatments in treatment-refractory OCD and adjuvant antipsychotics are useful in tic disorder, personality disorders, and psychotic disorders. This paper reviews results of treatment studies, investigations of biological markers, and neuroendocrine challenges and implications for the role of serotonin in pathophysiology and treatment of OCD.

10.A Case of Villous Adenomas in the Common Bile Duct and Cystic Duct.

Benign tumors of the extrahepatic bile duct are rare. They occur mostly in the common bile duct. Among them, papillomas and adenomas are most common. We experienced a case of villous adenomas in the common bile duct and also in the cystic duct. The ultrasound and the CT scan of the abdomen revealed markedly dilated common bile duct, subtle filling defect, and irregular mass narrowing the lumen of the common bile duct. The cholangiogram demonstrated movable, feathery, and marginated papillary mass with tumors, a broad base, and biopsy of this mass was done through the PTBD. The biopsy specimen showed villous adenoma. Even though it was diagnosed as benign, cholecystectomy and excision with Roux-en-Y hepaticojejunostomy was performed because of the high incidence of recurrences and malignant potential.
