3.Boundary of Anxiety Disorders: Obsessive-Compulsive Disorder and Schizophrenia.

Although obsessive-compulsive disorder (OCD) is recognized as a major psychiatric illness, few studies have investigated obsessive-compulsive symptoms in patients with schizophrenia. Recent increases in dual diagnosis due to changes to the DSMIV diagnostic criteria, findings of obsessive-compulsive symptoms (OCS) after administration of atypical antipsychotics, increased reports of co-occurrence of schizophrenia and OCD have increased the interest in OCS in schizophrenia. The incidence of OCS in patients with schizophrenia is reported to be 3-59%; however, the rate of progression to schizophrenia from OCD is not higher than in the general population. Level of insight, which differentiates obsessions from delusions, can be confounded by the specifier 'with poor insight' in OCD. OCD with schizotypal personality disorder or poor insight differs from pure OCD in that it responds poorly to treatment, has a poor prognosis, leads to deficits in cognitive functioning similar to those observed in schizophrenia, and responds well to low-dose antipsychotics. In the past, it was believed that OCS could prevent or delay the deteriorative course of schizophrenia. However, recent studies have shown that co-occurrence of OCD and schizophrenia increases the severity of symptoms, delays the treatment response, and reduces socio-occupational functions. Schizophrenia and OCD share the same pathophysiology in that both diseases have defects in the fronto-basal functional circuitry of the brain. The use of serotonin and dopamine has been addressed in schizophrenia due to the use of atypical antipsychotics in patients with schizophrenia. The finding of drug-induced OCS suggested that the two diseases share a similar pathogenesis involving neurotransmitters. Further studies are needed to make an exact diagnosis between the two diseases and to determine the common pathophysiology between OCD and schizophrenia.

6.Reliability and Validity of the Korean Version of Obsessive-Compulsive Inventory-Revised in a Non-clinical Sample.

PURPOSE: The reliability and validity of a Korean version of the Obsessive-Compulsive-Inventory-Revised (OCI-R) was examined in non-clinical student samples. MATERIALS AND METHODS: The Korean version of OCI-R was administered to a total of 228 Korean college students. The Maudsley Obsessive Compulsive Inventory (MOCI), Beck's Depression Inventory (BDI), and Beck's Anxiety Inventory (BAI) were administered to 228 students. RESULTS: The total and each of subscale of the Korean OCI-R demonstrated excellent internal consistency, good test-retest reliability, moderate convergent validity and good divergent validity. CONCLUSION: It was concluded that the Korean version of the OCI-R has strong psychometric properties as the original version.

7.Pheochromocytoma and Renal Artery Stenosis.

In hypertensive patients it is very important to detect renal artery stenosis or pheochromocytoma, since both diseases are curable causes of hypertension. However, renal artery stenosis can be induced by pheochromocytoma, when the diagnosis of the two simultaneous disease is very difficult. We experienced two cases of pheochromocytoma presented as renal artery stenosis. Pheochromcytoma was overlooked when renovascular hypertension was diagnosed. Pheochromocytoma was found during surgery in one patient and after angioplasty in the other. In both cases, BP returned to normal after surgical removal of pheochromocytoma without repair of the stenosis. Prevention of ineffective and unnecessary renal artery angioplasty of surgery requires knowledge of this unusal association between pheochromocytoma and renal artery stenosis and a high degree of clinical alertness for pheochromocytoma.

8.Clinical Outcomes of LASIK Using a 213 nm Solid-State Laser System: 6-month Follow-up.

PURPOSE: To present prospective clinical results of laser-assisted in situ keratomileusis (LASIK) using a solid-state laser system for the correction of mild to moderate myopia with or without astigmatism. METHODS: Thirty-eight eyes underwent LASIK using a 213 nm solid-state laser (Pulzar Z1trade mark, CustomVistrade mark, Australia). Uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), refractive errors, higher order aberrations (HOA) and contrast sensitivity were evaluated preoperatively and postoperatively. RESULTS: The preoperative and postoperative mean spherical and cylindrical refractive errors were -3.27+/-0.85D, +1.04+/-0.69D, -0.36+/-0.7D and +0.14+/-0.2D, respectively. UCVA over 20/25 was obtained in 27 eyes (93%). A result within 1.00D of the desired correction was achieved in 90% of the eyes. There were no decreases in BCVA within the study group. The preoperative and postoperative root-mean-square of HOA at 3 months were 0.196+/-0.092 microm and 0.326+/-0.107 microm respectively. The preoperative and postoperative contrast sensitivity values were similar. CONCLUSIONS: The clinical outcomes of LASIK using a solid-state laser system were comparable to the conventional refractive surgery in mild to moderate myopia. The 213 nm solid state laser may be an alternative option for refractive surgery.

9.Separated Spinal Needle Shaft from the Hub during Subarachnoid Block: A case report.

We report a case of in which the separation of a spinal needle shaft from a hub during subarachnoid block required surgical removal of the retained shaft. A 26-year-old male was scheduled for a transurethral resection of a bladder tumor under spinal anesthesia. A 25-gauge Quincke needle was selected The needle was advanced to the junction of the hub and the L3-4 interspace through a midline approach, but the subarachnoid space was not identified When the needle was withdrawn to change of direction of needle tip, the needle shaft separated from the hub. The separated needle shaft was palpable under the patient's skin, but it's removal using a Kelly through a small incision was impossible. Under the radiographic guidance, the needle was removed after several attempts. We emphasize importance of checking epuipment prior to a subarachnoid block and the use of a careful procedure to reduce complications associated with a faulty spinal needle.
