1.Treatment of Herpes Simplex Keratitis with IDU and Cryocautery.

The effect of IDU and cryocautery in the treatment of Herpes Simplex Keratitis was presented. The cryoapplicator was a model of Krwawlcz probe (2 mm in diameter) which was refrigerated in a mixture of dry ice and methyl alcohol. The applicator was appJied on the lesions 7 seconds each and antibiotics and atropine solutions were instilled. A good result was experienced in the treatment of several cases of dendritic kenititis by freezing the corneal lesions by means of cryoappJicator, which had not been responded to IDU therapy speedy. The deep neovascularization of the cornea in a case of stromal keratitis was disappeared after cryocautery for the treatment of the superficial corneal ulcers.

2.A Case of Muscle Transplantation in the Lateral Rectus Muscle Paralysis.

Muscle transplanation surgery is presently widely accepted as a good precedure not only for cosmetic, but also for functional results, a concept to which Marina's observations on the absence of fixed and predetermined functions in the nuclei and associated tracts of the oculomotor system give great support; however, not everyone is in agreement with this concept. This paper report one case in which the total transplantation of the vertical rectus muscles midway between their insertions and the insertion of the lateral rectus muscle were carried out, in addition recession of the medial rectus muscle and resection of the paralyzed lateral rectus muscle. Our case have been followed up during 8 months to rule out late complications, aspecially ocular movement and diplopia. No late complications, however, have been found up to remaining underaction of the right superior and inferior oblique muscles and also medial rectus muscle. The eyes were straight with abduction of the right eye up to 40 prism diopters. Binocular vision was present and there was no diplopia. The cosmetic result was excellent.

3.Clonidine Treatment of Clozapine-Induced Hypersalivation.

BACKGROUND: Hypersalivation can be a troublesome side effect of clozapine, limiting its usefulness in the management some cases of schizophrenia. But the pharmacodynamic basis of clozapine-induced hypersalivation remains obscure. Object : The aim of this study was to evaluate the effect of the alpha2-adrenergic agonist clonidine on clozapin-induced hypersalivation in the patients who were receiving clozapine. METHOD: Twenty one schizophrenic inpatients on clozapine participated in the study. The amount of saliva was measur on the 7th day at 8pm after starting clozapine treatment. Of them, 15 patients who had experienced hypersalivation treated with 0.1mg/day of clonidine. RESULT: Of 21 schizophrenic patients treated with clozapine in the psychiatric inpatients clinic, 15(71.4%) complain hypervalivation. After clonidine treatment, mean salivary flow-rate was decreased significantly in these patients. CONCLUSION: Clozapine-induced hypersalivation could be decreased by administration of alpha2-adrenergic agonist clonidine are compliance could be improved. Also our study supports the notion that increased adrenergic tone contributes to clozapine induce hypersalivation.

6.The Clinical Utility of BTA TRAK, BTA stat, NMP22 and Urine Cytology in the Diagnosis of Bladder Cancer: A Comparative Study.

PURPOSE: Various tumor markers, developed recently for the diagnosis of bladder cancer, are reported to be superior to urine cytology in terms of their sensitivity and non-invasiveness. A prospective study was performed to evaluate the efficacies of BTA TRAK, BTA stat, NMP22 tests and urine cytology. MATERIALS AND METHODS: Between August 2001 and December 2001, 154 patients were involved in the study. Voided urine was used to perform BTA TRAK, BTA stat, NMP22 tests and cytology. The final diagnoses were pathologically proven transitional cell carcinoma (TCCa) of the bladder in 34 patients, gross or microscopic hematuria without detectable tumor in 27, history of TCCa without evidence of recurrence in 68 and benign urologic diseases in 25. RESULTS: The sensitivities of the BTA TRAK, BTA stat, NMP22 and urine cytology were 82.4, 79.4, 61.8 and 32.4%, respectively. The specificities were 75.8, 70.8, 95.8 and 92.5%, respectively. When the sensitivity was subdivided according to the tumor stage, grade and size, the bigger size yielded higher sensitivities in the NMP22 and BTA TRAK (p<0.01 and p=0.03, respectively). When the results of the urine cytology were combined with each of the other tests, the specificity was lower than each test alone for the BTA TRAK and BTA stat, while sensitivity and specificity were both higher than each test alone for the NMP22. CONCLUSIONS: The BTA TRAK, BTA stat and NMP22 tests are useful in overcoming the limitations of cystoscopy and cytology as the initial evaluation tools for patients where bladder cancer is suspected. In particular, the NMP22 may be more useful due to its enhanced specificity when combined with the urine cytology.

9.Noncemented Hip Arthroplasty using Anatomic Hip Prosthesis Femoral Stem

The author tried to get result after clinical and radiological studies with 27 patients(35 hip joints) who have been operated non-cemented bipolar hemiarthroplasty or total hip replacement arthroplasty with using Anatomic hip prosthesis(Zimmer, Inc.; Warsaw, IN) femoral stem and who could be followed for the period of more than two years(average 30 months) at the INHA University Hospital, Dept. of Orthopaedic Surgery from the Feb, 1990 through the Dec, 1992. Average sequential Harris hip rating were maintained between 90 and 98 point during the follow- up interval. The Harris hip score was statistically significant in the degree of the initial fixation, degree of the thigh pain and degree of the limping but there was no difference significantly in degree of the final femoral componet fixation. Pain in the thigh was recorded at the yearly examinations from the first to the last follow-up. After one year, there were marked thigh pain in 2.9 per cent but in last follow-up there were no marked painful thigh. The limp was detacted 5.7 per cent after one year and last follow-up. Radiographs demonstrated that the fixation of the femoral componet was by bone ingrowth in 60 per cent of the hip, by stable fibrous fixation in 40 per cent, according to the criteria of Engh et al. The evaluated device was an early-generation prosthesis that was designed for insertion without the use of cement. Further study in necessary to determin the long-term durability of the prosthesis.
