1.A Study on the Method of Description for Mass Casualties Based on Abbreviated Injury Scale and Injury Severity Score

With the industrial development, vehicle, wars, and natural disasters, there are increasing chsnces of mass casuaities, it is very important to classify the injured patients by their severity, to give them immediate emergency care and to transfer them to the other medical facilities effectively. And it is more essential in the army. But in practice, there is no objective and comprehensive scale for injury severity yet. So we devised a method of comprehensive description for the injuredpatients. It is based on Abbreviated Injury Scale and Injury Severity Score and is composed of five parts of the body (General, Head & Neck, Chest, Abdomen, Extremities) and five degrees of severity (Grade I, Il, III, IV, V). For example, a description of a patient with multiple injuries such as generalized abrasion and cotusion, semicomatose mentality with skull fracture, hemoperitoneum, and fracture of right femoral shaft is very diffcult to understand quickly. But, with our method of description, the above is described as follows ; G(I)+(IV)+A (IV)+E(III), ISS =4+4+3 =41. And the high score is described as ISS makes it clear that the patient is very dangerous, even to death, so intensive care is immediately needed. Using this method, we reviewed 1,256 cases of passenger acidents of vehicles treated at our hospital during the last 2 years. The results were as follows ;1. The average ISS of 56 cases of dead is 33. 2. There are only 2 cases of death below 16 of ISS, and 84% mortality above 40 of ISS. 3. The average ISS of passengers in vehicles such as motocycles is 12, taxies 11, bongo 9, and buses 8. 4. We recommend our method of description for mass casualties, especially in the army. Because it is very useful to the injured patients by their severity, to give them immediate emergency care, to transfer them to the other medical facilities and to comprehend the whole condition of all patients.

2.Chondroma of Soft Tissue: A Case Report

A case of chondroma of soft tissue of right hand in 23-year old man was reported. The presenting symptom was a slow growing mass, causing mild tenderness. Radiologic examination revealed a well demarked and lobulated calcified mass, was located extraskeletally. Specimen exised from soft tisssue of the hypothenar area of the hand was 5×4×3.5cm sized mass, had gelatinous and gray-whitish appearance. On microscopical examination, the tumor was classified as a chondroma of soft tissue.

4.An evaluation of cellophane thick smear technique for mass stool examination.

To evaluate the Kato's cellophane thick smear techcique in detection ability of various helminth ova, authors examined 1,843 nationwidely collected stool specimens by the two methods, formalin-ether technique and cellophane thick smear technique. And the results were analyzed. The positive rates of following helminth ova were superior in cellophane thick smear technique to those of formalin-ether technique: Ascaris lumbricoides(62.3% : 51.1%), Taenia species (1.5% : 0.9%). The difference in A. lumbricoides ova detection is highly significant one. The overall positive rate for helminths ova was also higher in Kato's method (92.3% : 90.0%). The positive fates of following helminth ova were infrior in cellophane thick smear technique to those of formalin-ether technique: Hookworm (5.8% : 14.6%), Trichostrongylus orientalis(3.2% : 17.8%), Clonorchis sinensis(8.3% : 11.0%), Metagonimus yokogawai (0.6% : 1.7%) and Hymenolepis nana(0.05% : 0.3%). The positive rate for the ova of Trichocephalus trichiurus by the two methods was incidentally same, 74.1%. The causes of inferior positive rates in some helminth ova by cellophane thick smear technique were shortly discussed and suggests some opinions in improvement of detection ability.

7.Detection of BLV Proviral DNA in Korean Native Goats Experimentally Infected with Bovine Leukemia Virus by Polymerase Chain Reaction.

PCR implication using the primers for gag, pol and rev genes in BLV (bovine leukemia virus) proviral DNA and syncytium assay were carried out for the Korean native goats experimentally infected with bovine leukemia virus to investigate pathogenesis of BLV in the goats, and to establish a model animal for BLV infection. The oligonucleotide primers used in PCR revealed very high specificity, The minimal amount of FLK-BLV cellular chromosomal DNA to detect the integrated BLV proviral DNA was 10 ng. The peripheral blood lymphocytes from the goat infected with BLV were examined at regular intervals by PCR amplification and syncytium assay. Pol or gag genes were detected in none of three infected goats at the 1st week post-infection (p.i.). At the 4th week p.i., one of three goats showed the amplified gag gene. Thereafter detection rates for the genes were increased, indicating that the BLV proviral genes were integrated in all of the lymphocytes from three goats, at the 16th weeks p.i., when it was evident in syncytium assay that the lymphocytes from all of three goats were infested with infective BLV. Investigating the tissues from the necropsied goats at the 8th month p.i., the amplified BLV proviral genes and infective BLV were detected in all of the peripheral lymphocytes from three infected-goats. Among various tissues examined, the amplified BLV proviral genes were observed in spleen and superficial cervical, mandibular and retropharyngeal lymph nodes, and the infective BLV, in superficial cervical and mandibular lymph nodes. It was assumed that the Korean native goat was quite susceptible to BLV infection, indicating that the goat could be a good model animal for BLV.

A study was carried out to evaluate the house rats in southern Korea as reservoir host of intestinal trematodes, in 13 different areas: 7 in inland and 6 in brackish-water zones, during the period from August 1980 to August 1981. A total of 170 house rats was captured; 101 rats from inland and 69 from brackish-water zones. They consisted of 129 Rattus norvegicus, 22 R. rattus rufescens, and 19 unidentified. The infection status of the rats were as follows: Total 29 rats (17.1%) were infected by one or more kinds of intestinal trematodes; 27 from inland and 2 from brackish-water zones by areas. Rats in inland were more heavily infected. The intestinal trematodes collected from rats were identified as Echinostoma hortense, E. cinetorchis, Plagiorchis muris, Fibricola seoulensis, Clonorchis sinensis and Metagonimus yokogawai. As a reservoir host, the rats were very important in Echinostoma hortense and E. cinetorchis infection. Rats infected with these trematodes were found widely in southern Korea, and its infection rates were the highest among the discriminated flukes. In this study, no rats were found to be infected with Heterophyes and Pygidiopsis even in endemic areas. The negligible importance of rats as reservoir host of these heterophyid should be reevaluated in the future.

9.Was the Beauty of Chosun Dynasty Different from that of Japanese Edo Dynasty or Present Korea?: Anthropometric Analysis of Portraits.

The aim of this study is to elucidate whether the beauty of Chosun dynasty was different from that of Japanese Edo dynasty or present Korea by anthropometric analysis. We analyzed faces of 6 portraits of beauty of Chosun dynasty and 5 of Japanese Edo dynasty, and 9 photographs of prize-winner in the beauty contest of Korea during resent 3 years. We measured the 20 anthropometric items on each portraits and calculated 17 proportional indices of the face. Portraits of beauty of Chosun dynasty had the greater value on 1 proportion index(ratio of nasal width to intercanthal distance) and less value on 3 proportion index(ratio of horzontal dimension of palpebral fissure to intercanthal distance, Ratio of height of vermilon to transverse width of mouth, eye fissure inclination) than Edo dynasty. Portraits of beauty of Chosun dynasty had the greater value on 1 proportion index(ratio of height of upper eyelid to total facial height) and less value on 3 proportion index(ration of vertical dimension of palpebral fissure to total facial height, ratio of nasal width to intercanthal distance, eye fissure inclination) than nine prize-winner of beauty contest of Korea. In conclusion, beauty of Chosun dynasty has lower eye fissure inclination, wider nasal ala, and thiner lip than one of Edo dynasty. Beauty of Chosun dynasty has oriental eyelid, wider upper eyelid, narrower eye fissure, lower eye fissure inclination, narrower nasal ala than present beauty.
