2.Frequency of Meniscal Tear according to Ligament Injury

To know wheter the injury of the ligament may induce the meniscal tear, we statistically analyzed the incidence of meniscal tear with χ2 in cases of fresh and old(more than 3 months since injury) ligament injury in 419 joints of 407 adults who had been treated during the time between early 1984 and the end of 1992 in St. Mary's Hospital. The results were as follows: 1. The meniscal tear occurred more frequently in old ligament tear(44/70, 62.9%) than in fresh ligament tear(7/26, 26.9%) in cases of isolated ACL tear(P=0.062). 2. There was no significant difference between the old(10/34, 29.4%) and the fresh tear(24/96, 25.0%) in the combined ACL tear(P=0.615). 3. Overall ACL tear patients showed higher incidence of meniscal tear in the old ligament tear(54/1-4, 51.9%) than in the fresh ligament tear(31/122, 25.4%)(P=0.000). 4. The frequency of meniscal tear was very low in the isolated PCL tear(fresh tear: 0/14, old tear:1/17, 5.9%). In cases of combined PCL tear the incidence was rather higher in the fresh tear(11/33, 33.3%) than in the old tear(2/30, 6.7%)(P=0.015). 5. Overall PCL tear patients showed higher incidence of meniscal tear in the fresh ligament tear(11/47, 23.4%) than in the old ligament tear(3/47, 6.4%)(P=0.020). 6. The meniscal tear occurred more frequently in the ACL tear than in the PCL tear in both isolated(ACL : 85/96, 53.1%, PCL : 1/31, 3.2%) and overall ACL or PLC tear group(ACL : 85/226, 37.6%, PCL : 14/16.7%)(P=0.000). 7. The meniscal tear occurred less frequently in the isolated clootgeral ligament tear(9/94, 9.6%) than in the isolated cruciate ligament tear(52/127, 40.9%)(P=0.000). 8. The incidence of tear between the medial and the lateral meniscus showed no statistically significantly difference(P=0.215). From these results it would be suggested that the ACL tear, especially isolated tear may induce meniscal tear. On the other hand there was no statistically significant relationship between the incidence of meniscal tear and those of either PCL tear or collateral ligament tear.

3.A Case of Acute Idiopathic Scrotal Edema in a Newborn

Acute idiopathic scrotal edema (AISE) is a self-limiting condition that is characterized by acute scrotal swelling and erythema. AISE is a very rare cause of acute scrotum, especially in neonates. We report a case of AISE in a 26-day-old infant who was admitted to the outpatient clinic with swelling and erythema of the penis and scrotum for a week. His vital signs were stable, and laboratory findings were non-specific. A diagnosis of AISE was made using scrotal ultrasonography with color Doppler. His symptoms resolved within four days after the onset of supportive treatment, and he was discharged from the hospital. In neonates with an acute scrotum, AISE should be considered to prevent unnecessary surgical exploration.

4.The Comparison of the Effects on the Regeneration with Xenografts on the Furcation Involvement in Beagle Dogs.

For the regeneration of osseous defect on the furcation area, autogeneous bone graft has been primarily used. But it has the limitation of donor site, additive surgical operation etc. Recently anorganic xenogenic bone graft materials of removing all organic components are commonly used for the regeneration of periodontal defects. This study was the comparison of the effect on the regeneration with two types xenografts(Bio-oss(R) and Ca-P thin coated Bovine bone powder) on the furcation involvement in Beagle dogs. After surgically induced chronic periodontitis in bifurcation area of premolar, Bio-oss(R) and Ca-P BBP were grafted on the osseous defects. Tissue blocks including defects with soft tissues were harvested following a four-& eight-week healing interval and prepared for histologic analysis. The results of this study were as follows: 1. Bio-oss(R) group: there were significant differences among the Bio-oss? group at 4weeks and 8weeks, but the control group had various appearances : new bone formation, resorption of graft materials by multinuclear giant cells, connective tissue cells intervention in the bone graft sites etc. 2. Ca-P BBP group: lots of new bone formation were observed but the arrangement of periodontal ligament was not completed at 4weeks. New bone were replaced mature bone and the periodontal ligaments showed the functional arrangement at 8weeks. 3. By reason of undergrowing the epithelium within the osseous defects, new bone formation was not happened in the upper area of bifurcation in Bio-oss(R) group. 4. In Ca-P BBP group, epithelial undergrowth was not seen and generally showed much more new bone formation. 5. Ca-P BBP group showed the osteocyte-like cells at the inner portion of the graft materials 6. Both groups were similar to resorptive appearances of graft materials, but Ca-P BBP group had the better effects of osteoconduction.

5.Characterization of dental phenotypes and treatment modalities in Korean patients with Parry–Romberg syndrome

Objective:
To investigate the dental phenotypes and treatment modalities (TxMod) in Korean patients with Parry–Romberg syndrome (PRS) using longitudinal data.
Methods:
The samples consisted of 10 PRS patients, who were treated and/or followed-up at Seoul National University Dental Hospital between 1998 and 2019. Using a novel PRS severity index based on the numbers of the atrophy-involved area and asymmetry-involved item, we classified them into mild (n = 3), moderate (n = 2), and severe (n = 5). Dental phenotypes, including congenitally missing tooth (Con-Missing-Tooth), microdontia, tooth with short root (Short-Root), tooth with dilacerated root, and delayed eruption/impacted tooth, were investigated along with Tx-Mod.
Results:
The side of occurrence of all dental phenotypes showed 100% concordance with the side of PRS involvement. The most two common dental phenotypes were Con-MissingTooth and Short-Root (n = 29 and n = 17 in six patients). The sums of the average number of Con-Missing-Tooth and Short-Root increased from mild PRS to moderate PRS and severe PRS cases (1.0, 6.0, and 6.2). In terms of TxMod, growth observation due to mild atrophy, fixed orthodontic treatment, and grafting were used for mild PRS cases. Tx-Mod for moderate PRS cases involved growth observation for surgery due to an early age at the initial visit. For severe PRS cases, diverse Tx-Mod combinations including unilateral functional appliance, fixed orthodontic treatment, growth observation, grafting, and orthognathic surgery were used.
Conclusions
The novel PRS severity index may be useful to provide primary data for individualized diagnosis and treatment planning for PRS patients.
6.Bilateral Retrobulbar Hemorrhage and Visual Loss Following Traumatic Asphyxia.

Retrobulbar hemorrhage and permanent visual loss are rare presentations following traumatic asphyxia. In this case, bilateral permanent visual disturbance developed in a woman after chest-crushing trauma without direct trauma to the orbits. A computed tomography scan confirmed bilateral retrobulbar hemorrhages. An ophthalmologic exam revealed bilateral subconjunctival hemorrhages and severe lid edema. Despite high-dose steroid therapy, visual recovery was limited, and optic nerve atrophy developed. Ischemia of the optic nerve associated with retrobulbar hemorrhage may be postulated as one of the causes of permanent visual impairment following traumatic asphyxia.

7.A radiographical study on the changes in height of grafting materials after sinus lift: a comparison between two types of xenogenic materials.

PURPOSE: The performance of implant surgery in the posterior maxilla often poses a challenge due to insufficient available bone. Sinus floor elevation was developed to increase the needed vertical height to overcome this problem. However, grafting materials used for the sinus lift technique eventually show resorption. The present study radiographically compared and evaluated the changes in height of the grafting materials after carrying out maxillary sinus elevation with a window opening procedure. This study also evaluated the difference between two xenogenic bone materials when being used for the sinus lifting procedure. METHODS: Twenty-one patients were recruited for this study and underwent a sinus lift procedure. All sites were treated with either bovine bone (Bio-Oss(R)) with platelet-rich plasma (PRP) or bovine bone (OCS-B(R))/PRP. A total of 69 implants were placed equally 6-8 months after the sinus lift. All sites were clinically and radiographically evaluated right after the implant surgery, 7-12 months, 13-24 months, and 25-48 months after their prosthetic loading. RESULTS: Changes of implant length/bone length with time showed a statistically significant decreasing tendency (P < 0.05). There was no significant change in the Bio-Oss(R) group (P > 0.05). In contrast, the OCS-B(R) group showed a significant decrease with time (P < 0.05). However, no significant difference was observed between the two groups (P > 0.05). CONCLUSIONS: The results showed that there was significant reduction in comparison with data right after placement, after 7 to 12 months, 13 to 24 months, and over 25 months; however, reduction rates between each period have shown to be without significance. No significant difference in height change was observed between the Bio-Oss(R) and the OCS-B(R) groups.

8.Plasma Concentrations of Lidocaine Associated with Epidural Anesthesia and Axillary Nerve Block .

Forty adult patients who received lumbar epidural anesthesia and axillary nerve block were assigned to four groups. Of these, 15 patients in group 1 (lumbar epidural anesthesia group) were given 1.5% lidocaine 400 mg (27 ml) and 15 patients in group 2 (lumbar epidural anesthesia group) were given lidocaine with epinephrine (200,000:1) in the same volume and concentration as in group l. 5 patients in group 3 (axillary nerve block group) were given lidocaine in the same volume and concentration and 5 patients in group 4 (axillary nerve block group) were given lidocaine with epinephrine (200,000:1). The plasma concentration of lidocaine were measured by immunofluorescence assay and the peak concentration were achieved 15 and 30 min, after injection in group 1, 2, respectively. Similar results were observed in group 3 and 4. The peak plasma concentration in group 1, 2,3 and 4 were 2.85+/-1.17, 1.81+/-0.60, 2.35+/-0.97 and 1.62+/- 0.67 ug/ml (Mean+/-SD), respectively. and the time course of plasma levels of lidocaine in group 2 and 4 lower than those in group 1 and 2. These results indicate the mixed injection of 1.5% lidocaine 400 mg with epinephrine is considered safe when there is no contraindication because plasma concentration is reduced significantly.

9.A new glove port for single incision procedure.

Single-incision laparoscopic surgery has gained increasing attention due to its potential to improve the benefits of laparoscopic surgery. However, inconvenience remains for inexperienced surgeons during surgery when instruments conflict with each other, and a glove port is used hesitantly for such diagnosis related groups (DRG) because of its high cost. Authors made a new glove port by an odd surgical gloves and one wound protectors. This glove port is ease to make besides being convenient to us, and inexpensive. This new glove port has the benefit of easy utilization and cost effectiveness for surgeons performing single-incision laparoscopic surgery.

10.Antiplaque and antigingivitis effects of a mouthrinse containing cetylpyridinium chloride, triclosan and dipotassium glycyrrhizinate.

PURPOSE: The goal of this study was to evaluate the clinical anitplaque and antigingivitis effects of a mouthrinse containing cetylpyridinium chloride (CPC), triclosan and dipotassium glycyrrhizinate (DPZ) in patients with gingivitis and mild periodontitis. METHODS: Thirty-two subjects were randomized into 2 groups. The test group used a mouthrinse containing 0.05% CPC, 0.02% triclosan and 0.02% DPZ, while the control group used a placebo mouthrinse. At baseline, 2 weeks and 4 weeks, the papillary bleeding index (PBI), Turesky-Quigley-Hein plaque index (PI) and Loe-Silness gingival index (GI) were assessed. During the experimental period, the patients used the mouthrinse for 30 seconds, 4 to 5 times/day (10 mL/time) within 30 minutes after toothbrushing. RESULTS: No adverse effects appeared in either the experimental or the control group. Regarding PBI, PI and GI values, statistical significance was detected between values at baseline and 2 weeks for both groups (P<0.05). In the experimental group, statistically significantly lower values were detected at 4 weeks compared to at 2 weeks. However, in the control group, no statistically significant difference was detected between the values at 2 weeks and 4 weeks. Additionally, the mean value after 4 weeks for the control group was slightly higher than the mean value after 2 weeks for the control group. CONCLUSIONS: This study for 4 weeks demonstrated that mouthrinses containing CPC, triclosan and DPZ may contribute to the reduction of supragingival plaque and gingivitis.
