2.Accuracy of Intraocular Lens Power in Cataract Patients Underwent Excimer Laser PRK.

We retrospectively evaluated postoperative refraction of four patients(five eyes)who received intraocular lens implantation after excimer laser photorefractive keratectomy (PRK).The mean age of the patients was 44.8 years and 3 patients were female and 1 patient was male.The mean interval between PRK and cataract operation was 20.2 months (1 to 45 months)and the mean follow-up time after cataract operation was 16.2 months(2~43months).Phacoemulsification and PMMA-PCL(6.0~6.5 mm)implantation were made in all cases.SRK-II formula,the most popular empirical formula was used in first 3 cases(group I),but Idem-lens formula,a primitive theoretical formula was used in another 2 cases (group II)after occurrence postoperative refraction errors in group I .In group I and II,the mean postoperative refraction error was +3.00 diopters and +0.50 diopters and uncorrected visual acuity at postoperative 2 months was 20/60 and 20/24 respectively.The mean of calculated IOL-power was -3.75 diopter in group I and -0.625 diopter in group II.From these results,the empirical formula is definitely unsuitable for the calculation of IOL power in the cataract operation following excimer laser PRK. Moreover,although a primitive one,Idem-lens theory could be temporally useful for the IOL-power calculation in such cases.In conclusion,new formula is required for the cataract operation following excimer laser PRK.

3.Successful Treatment of Bilateral Conjunctival-Corneal Intraepithelial Neoplasia: Case Report and Review of the Literature.

PURPOSE: We report a case of successfully treating bilateral conjunctival-corneal intraepithelial neoplasia (CCIN) with surgical excision and adjunctive cryotherapy. CASE SUMMARY: A 74-year-old male visited our clinic for bilateral foreign body sensation and decreased visual acuity. His initial best corrected visual acuity was 20/50 in the right eye and 20/30 in the left eye. The right eye showed a 9 mm x 11 mm sized, gray-opaque limbal lesion from approximately the 7-o'clock position to the 11-o'clock position with spreading onto the cornea and conjunctiva. Biomicroscopy revealed a 6 mm x 7.5 mm sized minimally elevated, opaque lesion from the 3-o'clock to the 5-o'clock position extending to the central cornea in the left eye. The corneal lesion was well demarcated, opaque, and minimally elevated with bilateral focal pigmentation. Conjunctival lesions were finely vascularized and slightly elevated with melanocytic pigmentation. An excisional biopsy was performed to confirm the diagnosis and for therapeutic purposes, followed by an adjunctive cryotherapy. Postoperative corrected visual acuity improved up to 20/25 bilaterally and the patient had no recurrence 8 months after surgery. CONCLUSIONS: Bilateral conjunctival-corneal intraepithelial neoplasia is a rare condition. We report successful treatment and control of recurrence in a patient with bilateral conjunctival-corneal intraepithelial neoplasia using conventional surgical excision and adjuvant cryotherapy rather than topical chemotherapy.

4.A Case of Pulmonary Vein Aneurysm Simulating Lung Tumor.

A case of pulmonary vein aneurysm is presented. Chest X-ray revealed round density along the right cardiac border in right lower lung field. The round density was suspected as solid tumor initially, but the angiocardiogram showed dealyed opacification of right inferior pulmonary vein. So it was confirmed as pulmonary vein aneurysm. This patient took mitral valve replacement because of mitral and aortic regurgitation. Pulmonary vein aneurysm is considered as congenital in origin and pulmonary venous hypertension has its pathogenetic role in enlarging the varix. Pulmonary vein aneurysm probably has little clinical significance although cases of spontaneous rupture have been reported. There are a few reported cases in the literature, but the actual incidence probably is much higher than reported. This is the first to be reported in Korean literature.

5.The Clinical Feature of Keratoconus in Korean.

This study was performed to evaluate epidemiology and progression of keratoconus in Korean. 122 patients(223 eyes)who visited Kang-Nam St.Mary's Hospital and Bu-Pyung our lady of mercy hospital with keratoconus from 1989 to 1998 were reviewed retrospectively. At the time of diagnosis, prevalence in male(75 patients)was greater than that in female(47 patients)and the rate bilateral involvement was 84.4%(103 patients). The average age was 18.33+/-6.79 years and there is no difference between that of male and female. The greatest number of patients(63 patients, 51.6%)had been diagnosed from 15 to 24 years old. The mean astigmatism was 3.92+/-1.91 diopters. Of the 19 patients with unilateral keratoconus, 10 patients were followed up over three years(average follow period: 61months)and four reported that the disease had later been diagnosed in the contralateral eye. The greatest number of patients(107 eyes, 47.9%)had been examined from 0.02 to 0.1 in visual acuity without correction and 58 eyes(26.0%)achieved visual acuity with hard contact lens correction. Corneal findings included thinning(158 eyes), scarring(62 eyes), Fleischer ring(119 eyes), Vogt's striae(78 eyes). On corneal topography, a half of apex of cone(57 eyes of 107 eyes, 53.2%)was located in inferior temporal quadrant and highest rate of steepening(corneal power change away from the apex to the periphery along its semimeridian)was 2.70+/-1.48 diopters.

6.Clinical Characteristics of Tuberculosis in North Korean Refugees.

BACKGROUND: North Korea's economic and public health problems began in the early 1990s as a result of the gradual loss of economic support from its communist allies, combined with an inordinate number of natural disasters. The decline in public health has increased the incidence of tuberculosis in North Koreans and refugees. This study investigated tuberculosis situation in North Korean refugees in order to prepare for the future impact of tuberculosis control in Korea. MATERIAL AND METHODS: From 2001 to 2005, tuberculosis patients among North Korean refugees who were diagnosed before or after arriving in South Korea, based on the official records of OO hospital, were enrolled in this study. The demographic and clinical data of the cases were evaluated retrospectively. RESULTS: A total of 42 TB cases were reviewed during the study period. Of these, 37 (88.1%) cases were pulmonary TB. based on the cases identified among the number of North Korean refugees' arriving each year, the annual incidence of pulmonary TB were 900 per 100,000 in 2004, 700 in 2003, The number of smear-positive patients was 20 (47.6%) and the number of culture-positive patients was 18 (42.9%). Of the M. tuberculosis isolates, 2 cases were found to be susceptible to all anti-TB drugs available, 4 were resistant to isoniazid, and 3 were multi-drug resistant. CONCLUSION: The prevalence of pulmonary TB in North Korean Refugees is high. In addition, North Korean refugees suffer from more severe tuberculosis in bacteriological and radiological aspects.

The combination therapy of pegylated interferon and ribavirin is the mainstay of treatment for chronic hepatitis C patients. Anti-viral therapy is commonly associated with side effects such as headache, fever, myalgia, and arthralgia. However, anti-viral therapy can continue because these side effects are mostly mild and can be improved with supportive management. Anti-viral therapy should be stopped promptly if serious side effects, such as interstitial pneumonitis or hemolytic anemia occur, although those serious side effects are rare. There were a few case reports of interferon-related interstitial pneumonitis worldwide. In Korea, one atypical case report of interstitial pneumonitis has been reported, which followed the combination therapy of interferon-alpha and ribavirin in a patient with chronic hepatitis C. We present a case of interstitial pneumonitis and pancytopenia following the combination therapy of pegylated interferon and ribavirin in a patient with chronic hepatitis C.

9.First Male Case of Lymphocytic Hypophysitis in Korea.

Lymphocytic hypophysitis is a rare inflammatory disorder which is caused by autoimmune destruction of the pituitary gland. Almost all reported cases have been in women and the disease is often associated with pregnancy. We describe here the first male case of lymphocytic hypophysitis in Korea. The patient presented with headache, impotence, decreased libido, and deteriorated vision. Endocrinologic studies showed panhypopituitarism, and pituitary MRI imaging revealed a homogeneously enhanced pituitary mass with a thickened stalk. Treatment with prednisolone and thyroid hormone for five months was ineffective. Transsphenoidal resection of the pituitary mass was performed successfully with normalization of the visual field defect. Histologic examination revealed diffuse lymphocytic infiltration with dense collagenous fibrosis, consistent with lymphocytic hypophysitis. Lymphocytic hypophysitis should be considered in differential diagnosis even in men with hypopituitarism and an enlarged pituitary gland.

10.Effect of Cardiac Rehabilitation on Physiologic and Inflammatory Markers in Patients with Acute Myocardial Infarction.

BACKGROUND AND OBJECTIVES: here are several reports of cardiac rehabilitation (CR) having beneficial effects on the reduction of cardiovascular mortality and in the prevention of recurrent coronary events in patient with myocardial infarction (MI). An 8-week CR program was investigated to see if it affected the prognostic factors, such as inflammatory markers, after acute MI. SUBJECTS AND METHODS: 33 male and 5 female patients, with a mean age of 55+/-10 yrs, were consecutively assigned to the CR (n=19) and the control (n=19) groups three weeks after acute MI. The 8-week CR program consisted of life style modification and aerobic exercise training. At the baseline, and after 8 weeks, the symptom limited exercise test and peripheral blood sampling were performed to measure the physiologic capacity, the serum levels of high sensitive C-reactive protein (hs-CRP) and the plasma levels of interleukin (IL)-6 and tumor necrosis factor (TNF)-alpha. The interval changes of each parameter were compared between the two groups. RESULTS: The interval increments of the maximal oxygen uptake (14.3% vs. 10.6%, CR vs. control group, p=0.014), anaerobic threshold (18.8% vs. 7.0%, CR vs. control group, p=0.044) and exercise duration (9.4% vs. 3.1%, CR vs. control group, p=0.009) were larger in the CR than in the control group. The magnitudes of the interval changes in hs-CRP, IL-6 and TNF-alpha, as inflammatory markers, did not differ between the two groups (p>0.05). CONCLUSION: This 8-week CR program demonstrated an improved exercise capacity for MI patients, but a larger clinical trial, with modified exercise intensity and duration, will be necessary to detect any possible effect on the inflammatory markers.
