2.A Case of Eosinophilic Cellulitis.

We report a 44-year-old female patient with Eosinophilic cellulitis who had developed pruritic erythematous plaque on the right lower abdomen since two months ago. The blood eosinophil count was 2,066/mm at first visit. This lesion responded well to cortieosteroid therapy. However new erythematous swelling reappeared on the left lower abdomen and left inner thigh 3 months after the first treatment. A biopsy specimen taken from an erythematous plaque showed many flame figures and diffuse infiltration of eosinophils and histiocytes in the lower dermis and subcutaneous fat tissue.

3.Surgical Treatment of 100 Patients of Osmidrosis.

One hundred patients of osmidrosis were treated by surgical procedure at the Department of Dermatology, Wonkwang University Hospital. The methods of operative treatment were elliptical excision for 37(74), bipedicle incision with apocrine glancl undercutting for 14(28), and single incision wit.h apocrine gland undercutt,ing for 49 (98). The results were summarized as follows : 1. Of the 74 axillae treated by elliptical excision, one occurred recurrence, and 1.7 showed complication, hematoma in 6, infection and delayed union in 4, each necrosis and severe scar in one each, 2. Of the 28 axillae treated by bipedicle incision with apocrine gland undercutting, 6 showed complication; hematoma in 4, infection and delayed union in one each 3. Of t..he 98 axillae treated by single incision with apocrine gland undercutting, 5 showed complication, hematoma in 3, infection and delayed union in one each In conclusion, single incision with apocrine gland undercutting was found to be more excellent compared with the other surgical methods.

6.Efficacy of Dual Energy X-ray Absorptiometry for Evaluation of Biomechanical Properties: Bone Mineral Density and Actual Bone Strength.

INTRODUCTION: Bone mineral density (BMD) is an important index in diagnosis of osteoporosis and other metabolic bone diseases, prediction of fractures, and monitoring treatment. This study was to find a more feasible technique for prediction of osteoporotic fracture between dual energy X-ray absorptiometry (DXA) and quantitative computed tomography (QCT) and to reveal the actual change of bone strength when BMD was changed. METHODS: Ten of these 20 specimens were used as the demineralized group and the other 10 as the control. Each specimen was immersed in HCl solution at for a period of at least 10 minutes, up to 100 minutes, at an interval of 10 minutes for different levels of demineralization. BMD was measured using DXA and QCT. Uniaxial compression tests were conducted to measure biomechanical parameters. Pearson correlation analysis was used respectively between BMD and biomechanical parameters and between DXA and QCT. RESULTS: Elastic modulus (r=0.87) and yield stress (r=0.84) showed a statistically significant correlation with DXA BMD. Through correlation analysis with QCT BMD and elastic modulus, correlation coefficient showed hemi-vertebra (r=0.80) and trabecular (r=0.68). In yield stress, there was a statistically significant correlation in hemi-vertebra (r=0.87) and trabecular bone (r=0.84). CONCLUSION: DXA is a current standard technique not only for diagnosis of osteoporosis but also for prediction of fracture risk compared to QCT. Actual decrease of bone strength was much greater than that of BMD by both DXA and QCT.

7.Morphological Changes in the Ciliary Epithelium by Instillation of Befunolol.

The introduction of beta-adrenergic blocking agents for the treatment of glaucoma was a major advance in the continued search for medications that effectively control the intraocular pressure with minimal side effect. Beta-adrenergic blocking agents block the aqueous pump of the ciliary epithelium resulting in decreased aqueous formation and drop in intraocular pressure. The fact that beta-adrenergic blocking agents had an affinity to intraocular tissues containing melanin granules has been reported. The purpose of this study is to know that mechanism of decrease of intraocular pressure and an affinity to melanin granules of the beta-adrenergic blocking agents. Two drops of 1% befunolol were instilled in the albino and pigmented rabbits, and the eyes were enucleated on 1, 2, 6 and 24 hours after instillation. These eyes were studied with electron microscopic examination. The results were as follows: 1. In the non-pigment epithelium of the ciliary body, mitochondria became swollen. 2. These changes appeared earlier and were more remarkable in albino rabbits than in pigmented ones. 3. In pigmented rabbits, these changes appeared slowly, but continued for longer duration and it seems to be due to affinity of beta-adrenergic blocking agents to the melanin pigment. 4. Swelling of mitochondria at the non-pigmented epithelium of the ciliary body after instillation of beta-adrenergic blocking agents seems to be the pharmacological mechanism of the drugs against aqueous formation in the ciliary body.

8.Three Cases of Parameatal Urethral Cyst.

We herein report three cases of pararneatal urethral cyst, which developed on the male urethral meatus. These cysts occurred since birth or childhood, and did not interfere with urinary of sexual function. Histologically, these cysts were lined by pseudostratified columnar epithelium with variable thickness. No recurrence was evident after simple excision.

9.Choledochal Cysts Aggravated during Pregnancy.

The association between choledochal cysts and pregnancy is unlikely to be etiological, but may rather be an aggravation or precipitation of a preexisting condition. Although choledochal cysts rarely occur in pregnancy, the aim of this study is to elucidate the condition associated with pregnancy because delayed or inappropriate therapy may be catastrophic for both mother and child. Authors reviewed 18 cases of choledochal cyst, of which 2 were aggravated during pregnancy, managed at the Department of Surgery, Hangang Sacred Heart Hospital, College of Medicine, Hallym University, during the period from 1990 to 1995. The results were as follows: The ratio of female to male was 2.6:1 which revealed predominance in female and most frequent age group was above sixty years old(33%). In non-pregnant patients, the most common symptoms were abdominal pain(78%) and all pregnant patients had pain, jaundice, nausea and vomiting, and indigestion. Transaminase and bilirubin were elevated in all pregnant patients. In all patients, US(88%, 100%) was the most common preoperative diagnostic study and one case was diagnosed during explo-laparotomy. Pregnant patients have cholecystitis in two cases and nonpregnant patients had cholecystitis(19%), CBD stone(19%), and others. The operative procedures were performed in 9 cases and pregnant patients were operated by cyst excision and Roux-en-Y hepaticojejunostomy in one case, and also Roux-en-Y choledochocystojejunostomy in one case. According to Todani's classification, in non pregnant patients, type I was the most common(63%), in pregnant patients, two cases were type I. Postoperative complications occurred in 3 cases, wound infection(6%) and cholangitis(6%) in non pregnant patients, pleural effusion(6%) in pregnant patients. Two cases of choledochal cyst in pregnancy were 25 years old at 29.4 weeks and 36 years old at 16 weeks into pregnancy.
