2.A Case Report and Review of Tetanus.

Tetanus is a rare disease in the developed countries, including Korea, as a result of nearly universal active immunization. Because many physicians have little experience with its diagnosis and management, misdiagnosis and therapeutic delay may result in catastrophic consequences. We report one case of generalized tetanus. A previously healthy 35-year-old man was admitted to the emergency department because of stiffness on jaw and neck. board-like abdomen, extended and rigid both legs, chest tightness, and dyspnea. Seven days before admission, he was injured on the right thing at work. In the emergency department, he was given 5,000 units of human tetanus immunoglobulin intramuscularly, as well as diazepam 2 mg/hr intravenously. On the second hospital day, tracheostomy was performed. Ten days later, his condition was improved. Twenty eight days after admission, he was discharged without any complication.

3.A Case of Intravascular Papillary Endothelial Hyperplasia with Angioleiomyoma.

Intravascular papillary endothelial hyperplasia is a relatively rare benign tumor, which is charaterized by the development of endothelial-lined papillary projections in a vascular lumen. They can occur as a pure form in which endothelial proliferation developes in a dilated vessel, a mixed form in which endothelial proliferation occurs within a pre-existing angioma. We herein report a case of intravascular papillary endothelial hyperplasia coexistent with angioleiomyoma occuring in a 54-year-old man, who had a slowly growing tumor on the right sole for 2 years. The histologic findings revealed a solitary encapsulated mass composed of smooth muscles and blood vessels in deep dermis and papillary endothelial hyperplasia in a neighboring blood vessel.

5.A Case of Intradural Spinal Ossifying Schwannoma.

The spinal schwannomas are the most common spinal cord tumors and they make some regressive changes such as necrosis, mucous or cystic degeneration, caseation or calcification occasionally. We experienced a case of intradural ossifying schwannoma in 17 years old male, which occurred in the 2nd 3rd thoracic region. On operative finding, it was a very hard, pinkish gray and bean sized mass, and it firmly adhered to the spinal cord. We found dark bluish necrotic changes at the adhered sites of the spinal cord. On microscopical finding it revealed multiple foci of myxoid degeneration and osseous metaplasia with fibromatosis structure in schwannoma.

6.Effect of Atenolol on Left Ventricular Function in Essential Hypertension.

This study was made to evaluate the effect of oral atenolol, a cardioselective beta-adrenergic blocking agent without intrinsic sympathomimetic activity, on left ventricular function in patient with essential hypertension. Atenolol, 100mg/day, was given to 11 hypertensive patients for 4 weeks, and its effects on arterial pressure, pulse rate, left ventricular dimensions and ejection phase indices of myocardial performance were examined by echocardiography. Echocardiographic studies were performed before treatment and after 4 weeks of atenolol therapy. Arterial pressure fell form 145/90 mmHg to 138/84mmHg after 4 weeks. Pulse rate fell significantly from 69/min to 58/min(p<0.05). Left ventricular end-diastolic and end-systolic dimensions and mean rate of circumferential fiber shortening(mVcf) did not change significantly. Ejection fraction increased significantly from 0.66 to 0.72(p=0.01). This results indicate that atenolol in the resting state has no depressant effect on left ventricular function in patients with essential hypertension.

7.Effect of Tepid Massage and Antipyretics in the Management of Fever at the Emergency Department.

PURPOSE: Fever is the single most common problem in children visiting the emergency department. Fever is treated with a variety of methods, including the recommended combination of tepid massage, and antipyretics. We compared the effects of antipyretics and tepid massage for the management of febrile children. METHODS: We conducted a prospective, randomized, controlled study to assess 49 children who visited to the ED, Kyung Hee Medical Center, with complaints of fever. Ages ranged from 3 to 60 months. Patients were grouped into one of the following three treatment groups: 1) acetaminophen only, 2) tepid massage only, and 3) acetaminophen and tepid massage. We measured body temperature at 0 and 30 minutes after management. RESULTS: The mean temperature at 30 minutes had decreased by initial temperature 0.58degrees C, 0.48degrees C, and 0.90degrees C for groups 1), 2), and 3), respectively. Although body temperature for group 3) decreased to a greater degree than it did in the other groups, the differences were not statistically significant. CONCLUSION: Although tepid massage have been recommended for managing fever, their effect is still uncertain, so they are not recommended, especially in the cases of crowded emergency rooms.

8.Clinical Applications of Digital Subtraction Angiography(DSA) in Neurosurgical Field.

Methods for the identification and amplification of signals from quantities of intravascularly administered iodinated contrast agent and their combination with image subtraction have evolved into a clinically useful technique called digital subtraction angiography (DAS). The initial motivation for the development of DSA was the desire to replace standard conventional angiographic procedures with simpler, less invasive techniques. We have evaluated IV-DSA and IA-DSA which are made in 150 patients with clinically suspected intracranial diseases from Dec. 1983 to Mar. 1985. Comparison with IA-DSA and conventional angiography indicated that the quality and information of content of the film were equivalent. IV-DSA image quality was inferior to that of conventional angiography. But IV-DSA can be used for intracranial studies to evaluate results of surgery of aneurysm, AVM or tumor and to revaluate vasospasm after subarachnoid hemorrhage before surgery. Also, IV-DSA can be used for neck vessel appearance due to reveal adequate information.

10.A Case of Systemic Lupus Erythematosus with Severe Pulmonary Hypertension and Pericarditis.

Systemic lupus erythematosus (SLE) which is thought to be autoimmune in nature affects multiple organs and produces a diversity of signs and symptoms. However, cardiovascular manifestations of SLE are manifested more frequently by autopsy. Recently, with the prolonged survival and improvement of diagnostic methods in SLE including echocardiography, the morbidity and mortality associated with cardiovascular manifestations of SLE became more apparent and increased. Simultaneous involvement of the pulmonary artery and the myopericardium in SLE is known to be rare. Pulmonary hypertension is known to be associated with poor prognosis. We report a 27 year-old female patient of SLE with pulmonary hypertension, pericarditis and left ventricular systolic dysfunction.
