1.A Clinical Analysis of Fatigue Fractures among the Navy Recruits: Report of 18 Cases

Among two groups of navy recruits undergoing training, one consisting of 500 enlisted men and another 100 newly enrolled midshipmen, 18 reported to Chinhae Naval Hospital with fatigue fractures during the period from February though September 1969. Following observations were made on these cases: 1. The incidence of fatigue facture was 1.4 percent among the enlisted men and 11 per cent among the midshipmen. 2. There were 6 cases of complete fraeture and 2 cases of incomplete fracture. In the remaining 10 cases, only periosteal reaction was evdent roentgenographically with no visible fracture line initially: fracture lines, however, became apparent on subsequent examinations in all the cases. 3. The fractures involved the femur in 4 cases and the tibia in 14 cases, a ratio of 1:3. 5 in latter s favor. 4. The site of predirection in the femur was the distal one third of the shaft, occurring in 3 of the cases. In the tibia the junction of the upper and middle thirds was most frequently affected, occurring in 10 of the cases. 5. All the cases in our series healed without any complications.

2.Photochemotherapy of Vitiligo with Topical Methoxsalen and longwave ultraviolet light.

Vitiligo is a common acquired Thypornelanotic disease but the pathogenesis is still unknown and also the traatment is still unsatisfactory. The introduction in 1947 of psoralen therapy in vitiligo by El Mofty announced a new era in the treatment of this cosmetically disfiguring disease. Many investigators have developed therapeutic regimens for both topical and systemic poralen using various ligbt sources. The erythema action spectrum in UVA range of psoralen were defined, which consequently enabled the development of a more accurate regimen, namely psoralen and UVA(FUVA, exposure. The present study was undertaken to evaluate the efficacy of topical PUVA therapy in patients with vitiligo and to compare the result of PUVA therapy according to the age, sex, type of disease, total exposure dose, duration of disease nd involved site. Twenty one patients selected in the Department of Dermatology of Kyung hee University Hospital with various clinical types of vitiligo were treated with 03% 8-MOP ointment followed by exposure to longwave ultraviolet energy. Clinical response was graded as follows: Grade 0: No repigmentation Grade 1; Up to 25% repigmentaton Grade 2; 25% to 50% repigmentation Grade 3; 50% to 90% repigmentation Grade 4; Greater than 90% repigmentation A responsive case wss defined as a case with greater than 25% repigmentation. A effective case was defined as a case with greater than 50;o repigmentation.

3.Two Cases of Marjolin's Ulcer.

Marjolin's ulcer seated upon an old cicatrix, especially old burn scar, which may chiefly degenerated into a squamous cell carcinoma with propensity for metastasis. We are experienced two cases of Marjolins ulcer recently. A 54-year-old male who had a Marjolins ulcer(15x40cm) on his left lower extremity. He was experienced burn at the age of eight and rice sized ulcer was developed at the site of burn scar about 21 years ago. The ulcer progressively enlarged in size to reach 15x40cm for 21 years. The pathologic diagnosia was squamous cell carcinoma grade 1, but cancer cells invaded to deep dermie and subcutaneous tissues. Severe skin lesion and irreversible osteoporotic degeneration on knee joint of affected limb were present. So he was treated by high above knee amputation. Another 58-year-old female who had a Marjolins ulcer(10x20cm) on her right lower extremity. She was experienced burn at the age of forty eight and bean sized ulcer was developed at the site of burn scar about 1 year ago. The pathological diagnosis was squamous cell carcinoma grade 1 and invasian of cancer cells was limited upper dermis. On lymph node biopsy, the histological diagnosis was within normal limit. So she was treated radical surgical excision with split thickness skin graft(Mesh).

4.Artificial Retina Mimbrane Assembly Utilzing Intelligent Materials: Biocompatibility and Electorphysiologic Features.

In recent studies there have been various attempts at replacing a damaged retina with an artificial one. This paper outlines the assembly of an artificial retina membrane by incorporating a photorective protein bacteriorhodopsin into an electrochemically syntheszed conducting polymer polypyrrole. An electrophysiologic test was conducted to evaluate the photoresponsiveness of the bacteriorhodopsin and rabbit eyes were used to examine the biocompatibility of the artificial retina. The electrophysiologic test analyzed both wave forms and amplitudes obtained by photostimulating the artificial retina membrane with various light intensites(0.2, 2, 20J). In the biocompatibility test, the artificial membrane was inserted into the anterior chambers(4 eyes) and vitreous cavities(8 eyes) of rabbits. The condition of the eyes was then observed for one month. At the end of the first moonth, the eyes were enucleated and a histological examination was carried out. The electrophysiologic study displayed negative reflection waves, which are characteristic in rhodpsin, and their amplitudes showed a correspondign increase with stronger light intensities. The results of the biocompatibility test demonstrated that inflammatory reactions were not prominent in either the anterior chambers or the vitreous cavities during the first month and the histological examinations revealed no specific findings. In conclusion, a membrane assembled utilizing an electroactive polymer and a phocial retina.

5.Primary Carcinoid Tumor of the Testis.

Primary testicular carcinoid is a rare disease, accounting for less than 1% of all testicular neoplasms, with the potential for distant metastasis. We report a case of primary testicular carcinoid in a 44-year-old-man who presented with a painless, palpable mass in the left testis. Preoperative testicular ultrasound examination revealed a solid mass. Radical orchiectomy was performed, and pathologic examination showed a carcinoid tumor confined to the testis. There was no evidence of carcinoid syndrome, and a 24-hour urinary 5-HIAA assay was normal. Abdominopelvic CT scan and gastrointestinal contrast study showed no evidence of carcinoid in other organ. At 12 months postoperatively, the patient was without evidence of tumor recurrence.

6.Hepatic Zinc Concentration in Patients with Chronic Viral Hepatitis.

BACKGROUND AND AIMS: Zinc is an essential, mostly intracellular, trace element which participates in many oxidative or deoxidative reactions and in a protective action on liver cell activity. Plasma zinc levels are known to decrease in patients with liver disease including chronic viral hepatitis. The aim of this study was to reveal whether hepatic zinc concentrations have a correlation with grades of necroinflammation or stages of fibrosis in the patients with chronic viral hepatitis. METHODS: This study consisted of 50 subjects (43 chronic hepatitis B, 4 chronic hapatitis C, and 3 cirrhosis). Each specimen of liver tissue was classified with the grade of lobular inflammation, portal/periportal inflammation, and stage of fibrosis according to Scheuer's method. Hepatic zinc concentration was determined by ICP-Atomic Emission Spectrometry. RESULTS: The mean hepatic zinc concentration in the 50 chronic viral hepatitis patients was 233.66 g/g dry weight of liver tissue. The hepatic zinc levels were significantly correlated with the grades of portal/periportal inflammation (rs=-0.385, p=0.006), and grades of lobular inflammation(rs=-0.342, p=0.015). The stages of fibrosis were also negatively related (rs=-0.423, p=0.002). The zinc concentrations differed significantly among grades of lobular inflammation (p=0.013) and among stages of fibrosis (p=0.044). CONCLUSIONS: Hepatic zinc concentrations showed negative correlation with grades of portal/periportal inflammation, lobular inflammation, and stage of fibrosis in the patients of chronic viral hepatitis. These results suggest that decreased hepatic zinc concentration might be associated with severe hepatic injury and reflect decreased protective activity on liver cell injury.

7.Optimal salt concentration of vehicle for plasmid DNA enhances gene transfer mediated by electroporation.

In vivo electroporation has emerged as a leading technology for developing nonviral gene therapies, and the various technical parameters governing electroporation efficiency have been optimized by both theoretical and experimental analysis. However, most electroporation parameters focused on the electric conditions and the preferred vehicle for plasmid DNA injections has been normal saline. We hypothesized that salts in vehicle for plasmid DNA must affect the efficiency of DNA transfer because cations would alter ionic atmosphere, ionic strength, and conductivity of their medium. Here, we show that half saline (71 mM) is an optimal vehicle for in vivo electroporation of naked DNA in skeletal muscle. With various salt concentrations, two reporter genes, luciferase and beta-galactosidase were injected intramuscularly under our optimal electric condition (125 V/cm, 4 pulses x 2 times, 50 ms, 1 Hz). Exact salt concentrations of DNA vehicle were measured by the inductively coupled plasma-atomic emission spectrometer (ICP-AES) and the conductivity change in the tissue induced by the salt in the medium was measured by Low-Frequency (LF) Impedance Analyzer. Luciferase expression in-creased as cation concentration of vehicle dec-reased and this result can be visualized by X-Gal staining. However, at lower salt concentration, transfection efficiency was diminished because the hypoosmotic stress and electrical injury by low conductivity induced myofiber damage. At optimal salt concentration (71 mM), we observed a 3-fold average increase in luciferase expression in comparison with the normal saline condition (p < 0.01). These results provide a valuable experimental parameter for in vivo gene therapy mediated by electroporation.

9.The Usefulness of Proximal Radial Motor Conduction in Acute Compressive Radial Neuropathy.

BACKGROUND AND PURPOSE: The objective of this study was to determine diagnostic and prognostic values of proximal radial motor conduction in acute compressive radial neuropathy. METHODS: Thirty-nine consecutive cases of acute compressive radial neuropathy with radial conduction studies-including stimulation at Erb's point-performed within 14 days from clinical onset were reviewed. The radial conduction data of 39 control subjects were used as reference data. RESULTS: Thirty-one men and eight women (age, 45.2+/-12.7 years, mean+/-SD) were enrolled. All 33 patients in whom clinical follow-up data were available experienced complete recovery, with a recovery time of 46.8+/-34.3 days. Partial conduction block was found frequently (17 patients) on radial conduction studies. The decrease in the compound muscle action potential area between the arm and Erb's point was an independent predictor for recovery time. CONCLUSIONS: Proximal radial motor conduction appears to be a useful method for the early detection and prediction of prognosis of acute compressive radial neuropathy.

10.A Case of Giant Adrenal Adenoma Presenting Primary Aldosteronism

Primary aldosteronism is a syndrome chracterized by hypokalemic alkalosis and hypertension. Small sized adrenal cortical adenomas have been the major cause of this syndrome in most of the patients. However, if the adrenal mass is larger than 6cm in diameter and with irregular consistency, malignancy is more favored. We experienced a patient who had a giant adrenal adenoma with primary aldosteronism. A 24-year-old female presented with hypertension, hypokalemia, low plasma renin, and high plasrna aldosterone levels, was found to have a 6×5.5×5 cm sized left adrenal tumor by MRI. Her clinical laboratory feature did not revealed any evidence of Cushing's syndrome or pheochromocytoma. Preoperatively adrenal carcinoma presenting pure adrenal aldosteronism was suspected due to large size and heterogenous signal character of the adrenal mass in radiologic study. At operation well encapsulated, round giant adrenal tumor weighing 65gm(4.5×4×4 cm) was removed. There was no evidence of metastasis with return of adrenal function to normal after surgery. Benign adrenal adenoma was confirmed by the gross morphology and the histologic features.
