1.New Model of Capsule Measuring Ring: The Possibility to Predict the Capsular Bag Diameter.

PURPOSE: This study evaluates the capsular bag diameter (CBD) with a capsular tension ring (CTR) following cataract surgery and analyzes the predictive factors correlated with capsular bag size to predict CBD preoperatively. METHODS: We inserted the CTR before intraocular lens (IOLs) implantation in 25 eyes undergoing cataract surgery. The axial length (AL), anterior chamber depth (ACD), and lens thickness were measured and keratometry and corneal size were checked preoperatively. Also, the continuous curvilinear capsulorhexis size was measured at 7 days postoperative, and CBD was calculated with CTR in a dilated state at 1day, 1week, 1 month, and 2 months postoperative. RESULTS: The mean CBD gradually decreased from 11.30+/-1.76 mm at 1 day, 10.86+/-1.06 mm at 1 week, 10.23+/-1.43 mm at 1 month, and 9.88+/-1.31 mm at 2 months. The AL (R=0.657), ACD (R=0.481), and corneal size (R=0.348) had a positive correlation with CBD at 1 week postoperative. The AL (P=0.000) and ACD (p=0.015) were statistically significant predictive factors for CBD. CONCLUSIONS: Calculating CBD with the CTR helps to improve visual acuity following cataract surgery after implantation of IOLs having appropriate sizes predicted with AL, ACD, and corneal size.

A review of sixteen cases of the modified Bristow operation for recurrent anterior dislocation of the shoulder from Feb. 1987 to Feb. 1989 was presented and the following results were obtained. 1. The age of initial dislocation varied from 12 years to 27 years of age with the average of 17.3 years. 2. The age at operation varied from 21 years to 30 years of age with the average of 23.3 years. 3. The times of dislocation vaaried form 7 to 40 times with the average 23.8 times. 4. The average loss of abduction and external rotation were 5.9 and 12.5 degree respectively. 5. The complications and recurrence were not observed in this period

6.Taxol as Salvage Therapy in Patients with Recurrent Ovarian Carcinoma After First Line Chemotherapy.

As salvage therapy for recurrent ovarian cancer, Taxol has been tried alone or combined with cisplatin, or carboplatin. Paclitaxel (Taxol) is an antineoplastic agent; isoloated from the back of the western yew which acts as an antimicrotubule agent. In our study, 28 patients were eligible and assessable, Taxol was administered at a dose of 175 mg/m2, infused over 3hr every 21 days. A total of 161 courses of Taxol were infused, and the median treatmemt cycle was 6.25 cycles (1 to 16 cycles). The overall response rate was 21.4%, but we found higher response rate in sensitive group to platinum combined therapy than resistant. The median survival duration was 10.2 months and the median duration of follow up was 25.0 months. The worst severe toxicity was grade 4 leukopenia and expirement with sepsis. 39% of patients experienced myalgia and 25% experienced nausea, vomiting and diarrhea. Other adverse effects were not important or considerable. Taxol has been shown to be the most useful agent in patients with advanced ovarian cancer who had shown sensitivity with platinum previously, and Taxol has yielded low response rate in platinum-resistant patients. Further more study is repuired about Taxol itself, its optimal dose, combinding use with other antitumor agent, and as first line therapy in the treatment of advanced ovarian carcinoma.

8.Simultaneous Aortic and Tricuspid Valve Endocarditis due to Complication of Sinus of Valsalva Rupture.

We experienced a case of ruptured aneurysm of the sinus of Valsalva, and this resulted in simultaneous aortic and tricuspid valve endocarditis through a shunt. The echocardiography showed a ruptured sinus of Valsalva aneurysm to the right atrium with a shunt. The aortic non-coronary cusp was fibro-thickened with vegetation. Vegetations of the septal leaflet and the anterior leaflet of the tricuspid valve were also found. The blood culture grew Enterococcus garllinarum. We replaced both tricuspid and aortic valve with successful surgical result.

9.The Role of B-type Natriuretic Peptide in Predicting Postoperative Complications and Outcomes in Patients Undergoing Coronary Artery Bypass Graft.

BACKGROUND: Circulating levels of brain natriuretic peptide (BNP) provide prognostic information for patients with heart failure. The aim of our study was to investigate whether preoperative and postoperative BNP levels could predict postoperative complications and outcomes in patients after coronary artery bypass graft (CABG). MATERIAL AND METHOD: Data was collected prospectively on 30 patients (M/F=19/11, age 60.0+/-9.6 years) undergoing conventional CABG during a 1-year period beginning on January 1, 2005. Patients underwent off-pump CABG, and combined surgery was excluded. The BNP assay was performed preoperatively, immediate postoperatively at the intensive care unit (ICU), and 1, 3, 5, and 7days postoperatively. RESULT: Preoperative BNP levels significantly correlated with preoperative echocardiographic ejection fraction and an ICU stay of 5 days or more (r=-0.4, p=0.028; r=0.39, p=0.031, respectively). A preoperative BNP cut-off value above 263 pg/mL demonstrated high specificity (90.5%) for predicting postoperative complications using the receiver operating characteristics curves. Preoperative and postoperative (7 days) BNP levels were different depending on the abscence (mean BNP=99+/-23 pg/mL vs. 296+/-74 pg/mL, p<0.05) and presence (mean BNP=212+/-29 pg/mL vs. 408+/-23 pg/mL, p<0.01). CONCLUSION: Preoperative BNP levels > 263 pg/mL predict postoperative complications in patients receivingCABG.

10.A Clinicopathologic Study of 31 Cases with Ovarian Malignant Germ Cell Tumors.

OBJECTIVE: The purpose of this study was to review the clinicopathologic features, recurrent rate, survival rate and controversable issues in the treatment of the ovarian malignant germ cell tumors. PATIENTS AND METHODS: From August, 1991 to November, 1998 thirty-one patients with malignant germ cell tumors of the ovary treated in the department of obstetrics and gynecology, Kosin University Medical college, were eligible and assessable. Demographic characteristics, symptoms, signs, stage, tumor grade, mode of therapy and results of follow up were reviewed retrospectively. RESULTS: The patients with malignant germ cell tumor constituted 6.37% of all ovarian malignancies during this period. Histologic subtypes were 8 dysgerminoma(25.8%), 7 endodermal sinus tumor(22.6%), 10 immature teratoma(32.3%), 3 mixed germ cell tumor(9.7%), 3 choriocarcinoma(9.7%). The age of the patients ranged from 10 to 40 years (mean +/-S.D.; 24.26 +/- 7.51). The most common symptom was abdominal pain(38.7%). Most had stageI(18 cases, 58.0%) or stageIII(5 cases, 16.2%) diseases. All patients underwent surgery as the initial treatment, and nine patients received more than one operation. Postoperative adjuvant chemotherapeutic regimens were VAC, VBP, EP, BEP, EMA, and EMA CO. The mean follow up duration was 26.0(+/- S.D.; +/- 20.3) months. The 2-year and 5-year survival rate were 91.97%(+/- S.E.; +/- 0.05) and 86.86%(+/- S.E.; +/- 0.07).
