1.CT evaluation of choriocarcinoma with brain metastases

It is well established that the CT is an essential part not only in screening primary brain tumors, but alsoin staging known malignancy. This paper reports various CT findings demonstrated in 12 cases of choriocarciomawith brain metastasis. The CT findings such as the number, location and density of the metastatic lesions, thedegree of brain edema, mass effect and effect of contrast enhancement are reviewed as well as the episode ofstroke syndrome and survival duration after neurologic symptoms attacks. The results were as follows; 1. Ten ofthese cases showed solitary metastatic lesion and remaining 2 cases were multiple lesions. 2. One was isodensedensity and the others were hemorrhagic increased denstiy by CT. 3. All of these showed mass effect to thesurrounding structures along with moderate to marked brain edema. 4. The position of the metastatic lesion werelocated at the supratentorially in all cases. Most of them were at the unilateral frontal or parietal area of bothof them. One which noted multiple metastatic foci showed at the bilateral occipital regions. 5. Nine cases showedring enhancement after contrast infusion. One which noted isodense density on the noninfusion scan showed alsoring enhancement after contrast infusion. 6. Nine cases showed positive stroke syndrom. One of them was perforemdemergency craniotomy. The remainging 3 cases noted progressive neurologic symptoms. 7. Two cases were noted onlybrain metastasis but the others also had various degree of pulmonary metastasis and 2 of latter had hepaticmetastasis, too. 8. Most of the cases were treated with CHAMOCA regimen, and one of them was taken whole brainirradiation (3000 rads/2 weeks). Another one case revealed marked regression of not only metastatic brain lesionbut the pulmonary lesion after the 8th course of CHAMOCA regimen and still alive for over 460 days.

4.Results of a Bioabsorbable Magnesium Screw and K-Wire for the Surgical Treatment of an Upper Extremity Fracture

Purpose:
The treatment of fractures has shown excellent safety enabling solid fixation and early exercise treatment with the development of internal metal fixtures and fixation techniques. On the other hand, complications remain, such as secondary surgery for implant removal and, discomfort caused by internally fixed metal materials. Recently, a bioabsorbable magnesium implant with relatively high strength and low decomposition, manufactured using only body components, was developed in the form of screws and K-wire for use in orthopedic surgery. This study aimed to apply bioabsorbable magnesium screws and K-wires to upper extremity fracture surgery and investigate the results.
Materials and Methods:
From May 2019 to September 2019, 46 cases (clavicle 11, humerus 4, olecranon 2, radial head 4, coronoid process 3, radius 4, ulna 1, phalanx 8, metacarpals 8, and hamate 1) in 44 patients who agreed to use a bioabsorbable magnesium implant among patients requiring internal fixation using screws and K-wires for upper extremity fractures at the author’s orthopedic surgery department were enrolled. The U&I Corporation commissioned this study, and a prospective study was conducted. The radiological findings and The Disabilities of the Arm, Shoulder, and Hand (DASH) score, hydrogen gas generation, and complications were evaluated. In comminuted fractures, the magnesium screws and K-wires were used for additional interfragmentary fixation with a conventional metal plate and screws in 22 cases. There were 24 cases of exclusive usage. The most common surgical method was intramedullary fixation of bioabsorbable magnesium K-wires in 10 cases.
Results:
In all cases, bone union was achieved on average 16 weeks (4–28 weeks) after surgery, and the mean DASH score at the last follow-up was 45.0 (30–116). A hydrogen gas cavity produced around the absorbable magnesium implant was observed at an average of two weeks and six days, and the largest was noted at an average of 12 weeks. There were no interactive reactions with drugs, infection, osteolysis, tendon rupture and swelling with hydrogen gas. There were two cases of the loss of reduction, one case of implant breakage, two cases of urticaria.
Conclusion
Bioabsorbable magnesium screws and K-wires can be applied for upper extremity fractures, but caution is required.

6.A Case of Herpes - Associated Erythema Multiforme Treated with Thymopentin.

Herpes-associated erythema multiforme(HAEM) is a common subtype of erythema multiforme (EM) which usually develops 1-3 weeks after the recurrent herpetic episodes. The role of HSV in the pathogenesis of EM has not been fully defined. Recent studies, however, suggest that the host immune response to HSV, especially cell-mediated immune mechanism, is critical to the development of HAEM and, based on these findings, a variety of immunomodulating agents have been tried for the treatment of recurrent EM. Thymopentin, an immunomodulator that influences the maturation of thymocyte and mature T cell function, has been used as a therapeutic agent for various autoimmune diseases and recurrent HSV infection. A 37-year-old man with HAEM, who had failed to respond to intermittent acyclovir therapy for about 5 years, was treated with a subcutaneous injection of thymopentin 50mg three times a week for 6 consecutive weeks. The EM lesions were completely cleared 5 weeks after starting the treatment, without any sigrificant side effects. We herein, report a case of HAEM who has been treated with a 6-week schedule of thymopentin therapy and has showed no recurrence for the 6 months of follow-up period.

8.The Effect of Bacterial Lipopolysaccharide on the Lymphokine Production of the T Lymphocytes.

It is well known that the murine T helper cell clones are divided by their lymphokine secretory activities. One is the Th-1 cell, producing IL-2 and IFN after stimulation and the other is the Th-2 cell, producing the IL-4 and IL-5. This study was undertaken to evaluate the immunomodulatory properties of the bacterial lipopolysaccharide(LPS) on the lymphokine production in vivo and in vitro. The results were as follows: There were no effects on the lymphokine secretion by the in vitro treatment of the LPS. The in vivo treatment of the LPS decreases the capability of the production of IL-2 and IFN , whereas it increases the capability of IL-4 production. The altered capacity of the lymphokine production was recovered about 2 weeks after the treatment of the LPS. There were no differences on the lymphokine production between E-coli LPS and salmonella LPS. The capacity of the lymphokine production was the same in the treatment of a non-heated LPS or heated-LPS. The lymphokine production of the mice which were desensitized by the long term treatment of the LPS was not different from the control mice. The in vitro treatment of RU486 can block the alterations of the lymphokine production after the treatment of the LPS. In summary, one can tell that the LPS increases the secretion of the IL-4 through the endogenous secretion of the glucocorticoids.

9.A Case of Sporotrichosis Associated with Lupus Vulgaris.

Sporotrichosis is a chronic infection that follows accidental implantation of the fungus Sporothrix schenckii into the skin. The infection begins with skin trauma under conditions when conidia may be present. A 48-year-old woman presented with a 5-year history of an 8X 12 cm sized, centrally depressed, erythematous plaque on the left wrist. She had been treated with antifungal agents at an other hospital for about 6 months but there was no improvement. The histopathological findings showed an infiltration of mononuclear cells and tuberculoid structures on the whole dermis and subcutis. According to the histopathological findings and clinical manifestations, she was diagnosed as having lupus vulgaris. There was much improvement of the lesion after antituberculous therapy. However, 2 months later she developed asymptomatic multiple erythematous papules around the lesion of lupus vulgaris and along the lymphatic drainage of the left arm. The histopathological findings of the newly developed lesion showed a granulomatous reaction and an infiltrate of acute and chronic inflammatory cells. Moist and creamy colonies were changed into brown-to-black ones with a leathery appearance with age. This appearance and clinical manifestations confirmed the diagnosis of lymphocutaneous sporotrichosis. This clinical appearance reflects that the sporotrichosis developed on the ulcerative lesion or biopsy site of lupus vulgaris.

10.A Clinical Study of Dupuytren's Contracture

Dupuytren's contracture is characterized by progressive fibroplasia and contracture of palmar fascia with resultant deformity of hands and fingers. Recently it has been shown that the cellular element, called myofibroblast, contributes significantly in the process of contracture. Among the 6 patients we have seen during the last few years, there was a total of 9 hands with Dupuytren's contracture. Seven of these were treated by a partial fasciectomy or by a dermofascietomy with a full thickness skin graft and the results of these two procedures were compared. Electron microsconic studies were done in 3 of the 6 patients and myofibroblasts were found in 2 of them.
