1.Analysis of percutaneous nephrostomy in 72 patients

Percutaneous nephrostomy has been effectively performed 107 times in 72 patients for relief of obstruction,drainage of urinary fistula and renal or extrarenal fluid collection, and functional preservation of urinarysystem, and follow-up check has been made for recent 3 years at department of Radiology in Seoul NatinalUniversity Hospital. All procedures were successful and bilateral nephrostomy was done in 1 case of bilateral congenital UPJ obstruction. Percutaneous nephrostomy was done in left kidney in 38 cases(52.8%), permanent drainagewas done in 55 cases(76.4%) and external drainage was done in 65 cases (90.3%), and the duration of follow-up wasup to 8 months. Underlying disease were supravesical obstruction in 63 cases(87.5%), urinary fistula in 3 casses,inflammatory disease in 5 cases and infravesical obstruction in 1 case. Satisfactory outcomes were made in 68patients, whose BUN and serum creatinine levels have decreased near to upper normal range in at least 3 weeks. Major complications have occurred in 7 cases; symptomatic urinary infection in 3 cases, fracture of the catheterin 2 cases, pneumothorax in 1 case and sepsis in 1 case. Minor complications have occurred in 76 cases; catheterdislodgement or obstruction in 42 cases, fever in 10 cases, perirenal extravasation in 2 cases and periureteralleakage in 2 cases, etc. These complications can be prevented by adequate method duing Seldinger technique undnerultrasonography, CT or fluoroscopy, prophylactic and post-nephrostomy antibiotics, use of disposable catheterkits, frequent irrigation of the catheter and dressing of catheter fixation site when contaminated, prevention ofextraction of the catheter due to patient's carelessness, adequate fixation of the catheter to skin, and continuous follow-up of patients, It is expected that the territory of this iterventional procedure will beextended by instrument insertion and drug instillation through the nephrostomy tract.

2.Evaluation of the development of high risk low birth weight infants using bayley developmental test.

Forty two high risk low birth weight infants who had been admitted in neonatal intensive care unit at Yongdong Severance Hospital from August 1987 to July 1990 and followed up in the high risk infants follow-up clinic were studied for evaluation of their development using the Bayley develop-mental test at the age of 9 month(corrected age). The results obtaincd were as follows: 1) Their Mental Developmental Index(MDI)and Psychomotor Developmental Index(PDD)were 99.4+/-12.8 and 101.8+/-16.6 MDI and PDI in birth weight 1000~1500 gm group were 95.3+/-9.8 and 99.3+/-14.3 and the indexes in birth weight 1500~2500gm group were 102.2+/-14.3 and 103.5+/-17.9. 2) There is no statistical significant relationship between the results of Bayley scales and birth weight, cranial ultrasonographic finding, APGAR score, ventilator care and neonatal seizure. However, the MDI was significantly lower in small for gestational age group than appropriate gestational age group. 3) The body weight, height and head circumference measured at the age of 9 month, at the same time when Baylcy developmcntal test was performed, were 45.0+/-29.6, 40.2+/-25.7 and 46.2+/-26.8percentile of Korean Pediatric Growth Standard(1985).

3.Santonin-kainic acid complex as a mass chemotherapeutic of Ascaris lumbricoides control in Korea.

Santonin-kainic acid complex was evaluated as a chemotherapeutic of the mass treatment of the Ascaris lumbricoides infection in Korea. The results could be summarized as follows: The negative conversion rate was 82.9% in average in 4 treated groups. Some variations of negative conversion rate among the treated groups were noticed. The egg reduciton rate was 97.7% in average and the results were rather uniform among the three evaluated groups. By the analysis of egg reduction, it seems that the lightly infected cases whose E.P.G. were under 5,000 were resistant to treatment with the less reduced egg output. After the treatment with this complex, the number of egg discharged cases were reduced to 1.84% and the average number of discharge eggs per incompletely treated or untreated cases were reduced to 12.5% compared with the level of before-treatment egg output. The successive observations of the pattern of worm expulsion after drug intake was made. The worms were mostly expelled in the stool within 3 days, and 58.9% of total expelled worms were collected within 24-hour stool. The minimum length of the immature worms expelled was 7.6cm. Among the 659 Ascaris collected in the first-day stool from 91 rural people, 8.5% were in the range of 7.6-12.2 cm-long, immature worms. The sex ratio, male: female= 0.69: 1.

5.Report of two cases of double outlet left ventricle

Double outlet left ventricle (DOLV), even defined as origins of both great vessels from left ventricle,implies varieties of morphology in concern with loop pattern, location of ventricular septal defect, conotruncalanatomy, and presence or adsence of pulmonary stenosis. With the improvement of surgical tenchnique, correctivesurgery of DOLV became possible, and preoperative diagnosis and determination of the accurate anatomicalrelaltionship between great vessels, ventricular septal defect, and ventricles is very important in surgicalimplication. Authors have recently experienced two cases of DOLV. The one is with subpulmonic conus, subpulmonicventricular septal defect, patent ductus arteriosus, and preductal type coarctation of aorta. The other is withbilateral coni and subaortic ventricular septal defect. Loop patterns are [S,D,D] in both cases.

8.Contracture of the Hip Secondary to Fibrosis of the Gluteus Maximus Muscle

Four patients were treated who had limited flexion of the hips and various degrees of contracture of the abduction and external rotator muscles because of fibrosis of the gluteus maximus muscle. Each patient had a typical restriction of motion such that an affected hip could not be flexed in the usual sagittal plane, but had to be flexed in abduction. Genetic, congenital and postnatal factors have been suggested as the cause of fibrosis of gluteus maximus muscle. Three of the 4 patients reported here are of congenital origin and another one is of postnatal factor repeated intramuscular injections. Excellent correction of the hip contracture was achieved in all patients by division of the fibrotic bands.

10.Management of High Grade Cervical Intraepithelial Neoplasia Using CO2 Laser Conization and LEEP.

Between June 1990 and May 1994, 350 laser conization and 200 LEEP were performed. Indications of conization were that directed biopsy specimen was proved CIN II, III or suggests possible microinvasion. In all the cases the procedures were carried out with the patients under local anesthesia. Excisional cone sections(6,600) were evaluated for lesion length,depth and margin status. Invasive cancer was found in 5(1.4%) women of laser group. Operative time was shorter LEEP group than laser group significantly. In laser group, 25(8.3%) women had bleeding that required treatment. One case(0.3%) of pelvic infection and 7 cases(2.3%) of cervical stenosis were observed. In LEEP group, 10(5.3%) women had bleeding, 3(1.6%) patients became cervical stenosis. The diameter of bumed tissue was 0.28mm in laser group and 0.25mm in LEEP group. Success rate were 97.4% in former and 96.3% in later. This study demontrated that CO2 laser conization and LEEP were effective methods for treating high grade cervical intraepithelial neoplasia and added benefit of preserving reproductive function and rule out invasive carcinoma.
