1.A study on the lipids of Chinese liverfluke, Clonorchis sinensis.

The present study was undertaken to observe the quality and quantity of lipids in the adult worms of Chinese liverfluke, Clonorchis sinensis. Lipid extraction was done by the methods of Folch et a1. (l957) and Kenny (1952), and then the extracted lipid fractions of the worm were separated by thin layer chromatography. Those fractions were also subjected to perform the quantitative analyses of glycerides, cholesterols and phospholipids. The results obtained were summarized as follows: Total amount of glyceride was 37.56 mg per gram of worm tissue and the amount of monoglyceride was 8.34 mg per gm; diglyceride, 15.46 mg per gm; and triglyceride, 12.86 mg per gm. Total amount of cholesterol was 3.30 mg per gm of worm tissue, and the esterified cholesterol (1.72 mg/gm) was a little more than that of free cholesterol (1.26 mg/gm). The following 8 phospholipids were detected in the worm tissue of C. sinensis, i.e., lysophosphatidylcholine, phosphatidylcholine, phophatidylinositol, sphingomyelin, phosphatidylglycerol, phosphatidylserine, phosphatidylethanolamine and an unknown phospholipid.

2.Late-onset Brachial Artery Occlusion caused by Subclavian Artery Stenosis after Clavicular Fracture: A Case Report.

Although vascular injury after clavicular fracture is a extremely rare, it is a complication which is serious problem. Vascular injury associated with the fracture can be immediate or delayed. We report a case of late-onset brachial artery occlusion caused by subclavian artery stenosis with excessive scar tissue after open reduction and plate fixation for clavicular fracture and include a review of the literature.

4.Evaluation of the development of high risk low birth weight infants using bayley developmental test.

Forty two high risk low birth weight infants who had been admitted in neonatal intensive care unit at Yongdong Severance Hospital from August 1987 to July 1990 and followed up in the high risk infants follow-up clinic were studied for evaluation of their development using the Bayley develop-mental test at the age of 9 month(corrected age). The results obtaincd were as follows: 1) Their Mental Developmental Index(MDI)and Psychomotor Developmental Index(PDD)were 99.4+/-12.8 and 101.8+/-16.6 MDI and PDI in birth weight 1000~1500 gm group were 95.3+/-9.8 and 99.3+/-14.3 and the indexes in birth weight 1500~2500gm group were 102.2+/-14.3 and 103.5+/-17.9. 2) There is no statistical significant relationship between the results of Bayley scales and birth weight, cranial ultrasonographic finding, APGAR score, ventilator care and neonatal seizure. However, the MDI was significantly lower in small for gestational age group than appropriate gestational age group. 3) The body weight, height and head circumference measured at the age of 9 month, at the same time when Baylcy developmcntal test was performed, were 45.0+/-29.6, 40.2+/-25.7 and 46.2+/-26.8percentile of Korean Pediatric Growth Standard(1985).

5.A Case of Early Gastric Cancer in Childhood.

The incidence of gastric cancer is high in old age group and low in young age group, extremely rare in child age. So the diagnosis of gastric cancer in young age is often delayed, and this, with other factors such as poorly differentiated histopathologic tendency and rapid growing nature, makes the prognosis poorer than in other age guoup. Therefore it should always be remembered that the young who has gastric symptom may have malignancy in his stomach in spite of the age. We report a case of early gastric cancer in a 16-years old male with the review of the literature.

6.The Incidence of Venous Thromboembolism Following Shoulder Surgery: A Pilot Study.

BACKGROUND: To assess the incidence of venous thromboembolism (VTE) following shoulder surgery and to evaluate the role of postoperative duplex ultrasonography. METHODS: The study comprised a total of 224 patients who underwent shoulder surgery, including 180 shoulder arthroscopic surgeries, 28 shoulder arthroplasties, and 16 plate fixations for proximal humerus fracture between January 2014 and December 2014. The mean age of patients was 59.0 years, and there were 81 men and 143 women. Clinical data, including body mass index, blood tests, metabolic work-up for liver and renal function, previous, and present medical history, were evaluated. Duplex ultrasonography in the operative arm was performed on 2 to 4 days after surgery. RESULTS: The overall incidence of VTE following shoulder surgery was 0.45% (1/224). One patient with open reduction and plate fixation for proximal humerus fracture had asymptomatic deep vein thrombosis that showed complete remission after anticoagulant medication during a 2-month period. Four patients had asymptomatic superficial cephalic vein thrombosis and complete remission without any treatment. There was no case of pulmonary embolism. CONCLUSIONS: The incidence of VTE following shoulder surgery was extremely low in Asians. Duplex ultrasonography may be not considered a routine follow-up of shoulder surgery and can be selectively performed in high-risk or symptomatic patients for VTE.

7.Classification of Vitiligo Based on its Clinical types.

BACKGROUND: Vitiligo is a distressing skin condition in which acquired destruction of melanocytes causes depigmented patches of various shapes & sizes by as yet unclear mechanisms. Its classification is as varied and unresolved as the theories on its pathogenesis, the most plausible of which are autoimmune, neurohumoral and self-destruction theories. OBJECTIVE: Our purpose was to devise a classification scheme which would correlate well with the disease course and prognosis. METHOD: We classified vitiligo into 3 types(symmetric: type 1, segmental; type 2, localized; type 3) based on different proposed pathogenetic mechanisms and analyzed the differences between these types in the clinical manifestations of 502 Korean cases. RESULTS: 1. There were 203 males(40.4% ) and 299 females(59.6%). 2. The most frequent clinical type was the symmetric type(49.8%), followed by the localized type(34.1%) and the segmental type(16.1%). 3, The male-to-female ratio was highest in type 2, but not significantly so. 4. Poliosis and halo nevus were associated in 22.3% and 3.8%, respectiuely. 5. The mean age at the first visit was 32.7 years(male, 29.2 years, female; 27.5 years), and there was a significant difference between the clinical types(type 1; 41.0 years, type 2; 20.5 years, type 3; 26.2 years). 6. The mean age of onset was 25.6 years(male; 23.0 years, female; 27.5 years), and there was a significant difference between the clinical types(type 1; 31.3 years, type 2; 15.7 years, type 3; 22.1 years). 7. A family history of vitiligo was observed in 11.2% of patients, whose sisters were most frequently affected, and there was no significant difference between the clinical types. 8. Associated diseases were seen in 12.1% of the patients, the frequency of which was significantly higher in type 1 vitiligo. 9. Abnormal laboratory findings were found in 14.7% and were significantly more common in type 1 vitiligo. CONCLUSIONS: The 3 types differ significantly in the mean age at the first visit, the mean age of onset, disease association and abnormal laboratory findings. Type 1 develops by autoimmune mechanisms, as shown by the continuous progression and the close association with other diseases and abnormal laboratory findings, Types 2 and 3 have a different disease course, becoming stationary after initial progression, Thus, the 3 clinical types classified by the authors are thought to be useful in predicting the course and prognosis of vitiligo and in determining the appropriate therapy. For a more optimal classification, further study on the pathogenesis of vitiligo seems to be in order.

8.Normal Fetal Echocardiography.

Fetal echocardiography is used by means of decleration of fetal cardiac anaztomy, to estabilish the diagnosis of congenital heart disease in utero. We attemped fetal echocardiography to ninty three pregnant women after intra uterine period 24 weeks, and estimated cardiac circumference, cardiac axis, pulmonary atery root diameter, arortic root diameter, diameter of inferior vena cava, diameter of superior ve studied how these estimates associate with following gestational na cava, and fractional shortening of ventricles. We ages. Cardiac axis was on the average 37.28 degree and cardiac apex was located in anterior left side of chest area. Aortic root diameter was 0.227 GA-0.043mm (GA=gestational age) at systolic phase, 0.203 GA+0.421mm at diastolic phase. Pulmonaly root diameter was 0.271 GA-0.029mm at systolic phase, 0.251 GA-0.067mm at diastolic phase. Thoracic aorta diameter was 0.195 GA+0.109mm at systolic phase, 0.198 GA+0.794mm at diastolic phase. Fractional shortening was 0.24 (1 Standard Deviation=0.11) in right ventricle, 0.23(1 SD=0.154) at left ventricle, and so ratio of right and left ventricle was 1.04(1 SD=0.51). Once normal fetal cardiac anatomy is understood, structural defects and/or alternation of function can be evaluated antenatally.

9.The changes in the Rate of C -Reactive Protein in Orthopaedic Surgery.

To define the effectiveness of C-reactive protein (CRP) as indicator for early detection of the post-operative infection, it is essential to exclude possible normal changes of CRP influenced by surgery itself. We analyzed 44 patients who had done orthopaedic surgeries without any evidence of infection preoperative and postoperatively from May to Aug. 1995 at Dongsan medical center, Keimyung University. We checked the levels of Erythrocyte sedimentation rate (ESR) and CRP levels at preoperative and postoperative 1, 3, 5, 7, 9, 11, 13, 15, 17, 19 and 21st day in all cases. In all cases, ESR increased up to 60mm/hr maximally and CRP increased to more than 10mg/dl at least one occasion within the first 3 weeks postoperatively. However CRP showed more rapid changes than ESR. The mean value of the CRP was increased maximally (7.2mg/dl) at 3 day after operation and then decreased rapid until 11 day. ESR was also increased at 3 day after operation, but it sustained high level until 21 day after operation. CRP appears to be more sensitive and rapid indicator as an acute phase reactant for the operation itself because CRP revealed more earlier and accentuated change after the operation. The changes of CRP showed same pattern regardless of operation time, use of tourniquet, use of transfusion and gender. In conclusion, within 3 day after operation, the high titer of CRP can not to be an indicator for early detection of the infection, because normal high titer can be developed by surgery itself. If there notes persistent high titer or more increasing pattern of CRP after postoperative 3 day or abnormal high titer of CRP after 11 day postoperatively, infection may be highly suspicious.
