1.Anterior Plate Fixation of the Racture

Although anterior fusion has several advantages in the management of fracture-dislocations of cervical spine than posterior methods, it has not been used in common. Because, it does not give sufficient stability to prevent the dislodgment of the graft and angular deformity and it requires prolonged rigid external immobilization. In order to promote the stability and to reduce the external support, authors have fixed with plate and screws in anterior spine fusion for various type of fracture-dislocations of cervical spine, and report the results of 29 consecutive cases. Minimum follow-up period was 6 months (Av. 12mos.). Two level fixation after anterior decompression by corpectomy was done in 7 cases. Ambulation was started from the day after operation with aid of orthosis, regardless of the neurological status. Bone union was observed in all, but one expired case, within 2–3 months. There was no failure in fixation and no worsening in neurology. Improvement of neurological status were observed in 17 patient during and follow up periods of treatment, among 23 patient with neurological symptoms on arrival. The observed complications of hoarseness(2 csses), dysphagia(2 cases) and Horner's syndrome(1 case) were improved later, but the patient with pneumonia was expired one month after operation. It is concluded that the procedure is a safe and better alternative for the fracture-dislocations including flexion injury between C2-T1, because it provides good stability, rapid union, short external support and possibility of neural decomprssion.

3.The Clinical Study on the Root Coverage Effects with Free Standing Connective tissue Graft.

A mucogingival grafting procedure has been developed to cover denuded root surface. The subepithelial connective tissue graft technique is very predictable and allows for a good esthetic results and minimum patient discomfort on the palate. However, in areas where there is a lack of vestibular depth and keratinized attached tissue, the presence of frena or heavy muscle attachment, covering the connective tissue graft with a mucosal flap is very difficult. The purpose of this study is to evaluate an alternative technique of root coverage using the free connective tissue graft. The results were as follows: 1. Probing depths didn't seem to vary significantly from the preoperative to postoperative period. 2. The amount of keratinized tissue showed an increase of 5.9+/-0.97mm from the preoperative level. 3. Total clinical exposed root coverage increase 72.2% compare with preoperative level. 4. The shrinkage from gingival margin is 4.2+/-1.15mm and the mean shrinkage rate is 40.1%. 5. The depth of the vestibule increased with the average distance from cementoenamel junction to mucogingival junction being 7.4+/-1.65mm.

Split hand split foot(SHSF) is a rare human developmental defect characterized by mi-ssing digits, fusion of remaining digits, and a deep median cleft in the hands and feet. Cy-togenetic studies of deletions and translocations associated with this disorder have indicated that an autosomal dominant split hand/split foot locus maps to 7q21-q22. We have experienced a case of ectrodactyly in a 25-year-old primigravida woman and her baby and reported out our experience with a review of related literature.

5.The Stereomicroscope and SPM Study on the Marginal Change of Porcelain Crown in Various Repeated Instrumentations for Periodontal Therapy.

Instrumentation for periodontal therapy may induce marginal damage which increases plaque accumulation and result in periodontal disease. But there have not been many reports of instrumentations on the artificial crown so far. Therefore this study is conducted to evaluate the effects of various repeated instrumentations on the porcelain crown marginal portion. Of the 10 extracted periodontally diseased maxillary first premolars, were 12 proximal surface used in this study. The finishing line of the preparation was placed on the root surface below CEJ and then the crown was cast and cemented in usual manner. Every 4 surfaces of the 3 instruments-curet, ultrasonic scaler, and ultrasonic curet- is used. and four samples used in each instruments. The relevant procedures and measurements were repeated 3 times in each surfaces. Marginal gap is measured by the microscope and surface roughness, Scannig Probe Microscope. Measurements are made at 5 points in each surfaces, making 20 points in each instrument. The results evaluated statistically were as follows 1. As instrumentation was repeated, both marginal gap and roughness were increased in all group 2. In the hand curet, marginal gap was increased every instrumentation and roughness was increased after second. 3. In the ultrasonic scaler, both marginal gap and roughness were increased every instrumentation. 4. In the ultrasonic curet, marginal gap was increased after third instrumentation and roughness was increased after second. 5. Marginal gap and roughness used by the ultrasonic curet were lower than the others and no difference was seen between the hand curet and ultrasonic scaler From the results of this study, the ultrasonic curet was useful in some aspect, but careful instrumentation was needed. Furthermore it was important to minimize the instrumentation through complete periodontal therapy before setting and adequate plaque control.

7.A STUDY OF THE MICROLEAKAGE AT THE POST AND CORE AND TOOTH INTERFACE.

Post and core is used to restore endodontically treated teeth, and it is a very important part which supplies retention and support to the prosthesis. But occasionally, caries occurred due to the microleakage at the post and core and tooth interface, the failure of prosthesis has happened. In this study, the microleakage of cast gold post and core, amalgam core, and composite resin core was investigated and compared. The coronal part of the extracted upper anterior teeth were removed and endodontically treated with conventional method. The teeth were divided into three groups. In group 1, cast gold post and core was cemented with resin cement and in group 2, ready-made post was cemented with resin cement and amalgam core was built. In group 3, ready-made post was cemented with resin cement and composite resin core was built. All specimens were thermocycled between 5degrees C and 55degrees C with dwell time of 15 seconds, and immersed in 0.5degrees C aqueous solution of basic fuchsin dye for 24 hours. After embedded in the epoxy resin, the specimens were sectioned longitudinally and observed with stereomicroscope with the magnification of 25. From the findings of this study, the following conclusions were obtained. 1. There was microleakage at the core/tooth interface of all specimens. 2. The microleakage of gold post and core was significantly less than those of the other two groups. 3. There was no significant difference between the microleakage of amalgam core and that of composite resin core.

8.The Biological Effects of Calcium Phosphate Coated Implant for Osseointegration in Beagle Dogs.

The influence of calcium phosphate (Ca-P) coating on the bone response of titanium implants was investigated two types of titanium implants, i.e. as -machined ,as -machined with Ca-P coating, were prepared. The Ca-P coating produced by OCT Inc technique. These implants were inserted into the left and right femur of beagle dog. After implantation periods of 3 days, 1weeks, weeks, 4weeks, 8weeks, 12weeks, 24weeks, the bone-implant interface was evaluated histologically, histomorphometrically, and removal torque. Histological evaluation revealed no new bone formation around different implant materials after 2weeks of implantation. After 4 weeks, Ca-P coated implants showed a higher amount of bone contact than either of the non coated implants. After 12weeks, bone healing was almost completed. And implant were removed by reverse torque rotation with torque-measuring device. Mean torque values for 4weeks control were 2.375Kgf.cm and experimental were 2.725Kgf.cm. And mean torque values for 8weeks control were 1.25Kgf.cm and experimental were 1.0Kgf.cm On the basis of these findings, we concluded that deposition of a Ca-P coating on an implant has a beneficial effect on the bone response to this implant during the healing phase. Besides implant surface conditions the bone response is also determined by local implant site condition.

9.Primary Treatment of Acute Ankle Sprain: Retrospective Comparison of Cast Immobilization and Functional Ankle Brace

PURPOSE: To compare the efficacy between cast immobilization and functional treatment using an ankle brace as a treatment for acute lateral ankle sprain. MATERIALS AND METHODS: This study reviewed the medical records of 157 acute ankle sprain patients who were treated between 2009 and 2014. A total of 101 cases were included in this study except for cases with a combined injury, and could not be followed up for eight weeks after the first visit. The patients were divided according to the treatment modality: a cast immobilization group (64 cases) and functional treatment group (37 cases). The clinical outcomes were assessed retrospectively based on the medical records of each group. The residual symptoms, such as pain, swelling, and instability, at three weeks after the primary treatment and at the last visit were compared. RESULTS: The residual pain and instability were significantly common in the functional treatment group at three weeks. Five cases (7.8%) of pain and one case (1.6%) of instability were in the cast group whereas nine cases (24.3%) of pain and six cases (16.2%) of instability in functional treatment group (p=0.021, p=0.014). On the other hand, there was no meaningful difference at the last follow-up. Residual pain, swelling, and instability at the last visit were noted in three (4.7%), six (9.4%), and four cases (6.3%) in the cast group, and three (8.1%), three (8.1%), and three (8.1%) were observed in the functional treatment group. Six patients refused cast immobilization. CONCLUSION: Although there was no significant difference at the last follow-up, cast immobilization appears to be more effective than a functional brace in terms of early pain relief and early restoration of ankle stability as a treatment for acute ankle lateral sprain in this study.

10.Ultrasound-Guided Subclavian Vein Catheterization.

PURPOSE: To assess the usefulness of the ultrasound-guided subclavian vein catheterization in difficult patients. MATERIALS AND METHODS: We tried subclavian vein catheterization in 18 patients in which conventional blind technique failed(N=9) :or was complicated by hemothorax or pneumothorax(N=3) :or was prohibited by respirator care(N=4) and severe thoracic deformity(N=2). Initially, the patency of subclavian vein was evaluated with ultrasonography, and then, the puncture of the subclavian vein was performed under the guidance of ultrasonography. Under the fluoroscopy, the patency of the proximal subclavian vein and the superior vena cava was evaluated after contrast-media injection and a catheter was inserted into the subclavian vein and accurately positioned at the superior vena cava. RESULTS: Successful catheterization was performed in 17 patients. In the remaining one patient, we did not perform catheterization because of bilateral subclavian vein thrombosis detected during the procedure. There were no procedure-related complications. CONCLUSION: Ultrasound-guided subclavian vein catheterization is an easy and safe method even in difficult cases.
