1.Arthroscopy after Meniscectomy of the Knee: The Posterior Horn in 40 Cases

Diagnosis of the disorders following menlscectomy is occasionally difficult, as ther has frequently been involved many causes other than the torn menisci. The retained fragments of the menisci have been blamed to be responsible for postoperative knee disorders. Arthroscopic examination was carried out in 40 knees after meniscectomy and the frequency of retained posterior horn in relation to the postoperative trouble was observed. 1. In 11 out of total 40 knees, the retained posterior horn could be diagnosed, consisting of 27.5% of all. 2. Only flve of these 11 knees showed some kinds of postoperative disorders,while the reat had no trouble. 3. Those knees in which the excision of the posterior horn was incomplete, were often associated with Irregular healing pattern of the local tissue, which might be closely related to the occurrence of the secondary Intraarticular change.

2.Preoperative Staging of Endometrial Carcinoma by MRI.

BACKGROUND: In patients with endometrial carcinoma, preoperative evaluation of exact staging has important prognostic and therapeutic implications. The incidence of pelvic and aortic lymph node involvement in endometrial carcinoma depends on grade of tumor differentiation and depth of myometrial invasion. MATERIAL AND METHOD: To evaluate whether MRI provides a preoperative assessment for staging of endometrial carcinoma, MRI was undertaken in 28 patients, a few weeks before operation. Myometrial invasion was devided in three categories, and involvement of cervix, adnexa, and pelvic cavity were classified. RESULTS: The results of MR imaging were compared with these of pathology. The preoperative MRI staging of endometrial carcinoma was correct in 22 out of 28 patients. In the evaluation of myometrial invasion, the MR imaging underestimated in 4 cases and overestimated in 1 case. CONCLUSION: In patients with endometrial carcinoma, MR imaging is very useful in the assessment of the depth of myometrial invasion, stromal invasion of cevix, lymphatic & pelvic metastases and extent of the lesion.

4.Influences of Socioeconomic Status on Short Stature in Childhood

Objectives:
Short stature in childhood is defined to the cases in which the stature is below 3 percentiles of the standard value in accordance with that of those in the same age and gender group. The influence of the socioeconomic status on the short stature in childhood are analyzed.
Methods:
154 children from the community child center in a region of poor socioeconomic status and 78 children in normal socioeconomic status who visited the Busan Medical Center due to the issue of short stature were selected for examination and analysis.
Results:
The prevalence rate of short stature at the community child center in 2 municipalities in Busan was confirmed to be 7.3%. In the comparison of the average growth parameters of poor socioeconomic status and normal socioeconomic status in the short stature group, there was no observation of significant difference in terms of the chronological age, midparental height, bone age, bone age/chronological age, height standard deviation score (SDS), body mass index(BMI) percentile and insulin like growth factor binding protein 3 (IGFBP3) SDS. In the short stature suspicious group, there was observation of significant difference in the averages of bone age, weight, BMI percentile , IGFBP3 and IGFBP3 SDS.
Conclusions
Although the prevalence rate of short stature in children belonging to the poor socioeconomic class was observed to be higher than the existing results, there was no significant difference in the growth parameters associated with the growth of the height from those of the children in normal socioeconomic status.
5.Concentration of Tobramycin in the Cornea and Sclera After Subconjunctival Injection of Negatively-charged Liposome Encapsulated Tobramycin in Rabbits.

Bacterial keratitis is a common ophthalmic disease. Recently, topical and subconjunctival therapy were equally effective in reducing the number of viable bacteria in experimental corneal ulcers. Subconjunctival injection produced high but transient concentrations followed by persistent low levels. In contrast, eyedrops produced moderate but sustained concentrations throughout the treatment period. Liposomes are small, biodegradable lipid vesicles with an aqueous core. Incorporation of drugs into liposomes provides a convenient way to retard their release from a relatively inert depot without changing the intrinsic characteristics of the agents.

In two patients with chronic corneal ulcer, resistant to conventional therapy, analysis of tear fluid and observation of the corneal state were performed before and after treatment using autologous fibronectin and aprotinin for the purpose of estimating the effect of treatment. The plasmin activity which was revealed before treatment was absent after treatment, and corneal reepithelialization was observed after treatment. We think the combined therapy with autologous fibronectin and aprotinin may be effective for the treatment of therapy-resistant chronic corneal ulcer.

8.Clinical Results of AMO Array(R) Multifocal Intraocular Lens.

PURPOSE: To evaluate the clinical results of a AMO Array mutifocal lens. METHOD: we performed a prospective clinical trial of 20 bilateral cataract patients for at least 6 months. RESULT: Mean monocular uncorrected distance acuity(Snellen) was 0.75, which increased to 0.9 with best correction. Mean biocular uncorrected distance acuity was 0.85,which increased to 1.0 with best correction. Mean monocular near acuity was 0.62, which reached 0.88 with best correction. Mean biocular near acuity was 0.71, which reached 1.00 with best correction. Mean contrast sensitivities were within normal range for all spacial frequencies. In glare discomfort, 10 patients had no complaint, 7 patients had no idea, and only 3 patients had some problem. Reading newspaper without glasses was achieved by 28 of 40 eyes(70%) and 15 of 20 patients(75%). CONCLUSION: AMO Array lens can be used as a method of treatment of presbyopia, when combined with correct biometry.

9.Attenuation of endothelial relaxation in umbilical arteries from preeclampsia patients.

OBJECTIVES AND METHODS: To directly examine the function of the endothelial cell(EC) and smooth muscle cell in umbilical arteries acquired from preeclampsia patients between June 1998 to November 1999, using a conventional tension measurement and bioassay experiment. RESULTS: Relaxation responses to EC-dependent relaxing agents including bradykinin and A23187 in human umbilical artery rings were significantly decreased in preeclampsia(p<0.01). Relaxation responses to EC-independent agents(SNP and SNAP) were also inhibited in umbilical artery rings acquired from preeclampsia patients(p<0.01). To test the change of endothelial cell function in preeclampsia without involvement of smooth muscle dysfunction, we used human umbilical artery and rabbit femoral artery as a donor and detector, respectively, in bioassay experiment. Relaxation responses to EC-dependent agents(A23187 and bradykinin) showed similar results to conventional tension measurement (p<0.01). Relaxation responses to 8-bromo-cGMP in human umbilical artery rings were also significantly decreased in preeclampsia(p<0.01). CONCLUSIONS: It can be concluded that increased vascular resistance in preeclampsia is not only due to the disturbance of endothelial function, but also to smooth muscle dysfunction.

10.Serovar Identification and Genetic Characterization of Leptospira Isolates by Arbitrarily Primed PCR and Ribotyping.

Serovars of 22 leptospiral field isolates from rats trapped in Korea were identified by cross-agglutinin absorption test (CAAT). Genomic characteristics of 7 selected isolates and 6 antigenically closely related reference serovars of lai, yeonchon, birkini, gem, mwogolo, and canicola were differentiated by arbitrarily primed PCR (AP-PCR) and southern blot hybridization using 16S rRNA gene probe from Borrelia burgdorferi. Among the 22 isolates, 21 strains were identified as serovar lai by CAAT, while the serological reactivity of NR13 did not accord with that of serovar lai. Results of AP-PCR using primers RSP, KF and PB-1 were in general agreement with those obtained by serological identification, and all 7 isolates including NR13 showed the same profile with serovar lai or yeonchon. In the southern blot hybridization with 16S rRNA gene probe, the isolates were divided into two ribotype groups when HindIII and BamHI digests were employed: isolates NR4, NR13, and serovar lai showed the same profile, and isolates JR34, JR57, KR48, JR77, and JR82 were classified as the another ribotype group. Isolate NR13 and serovar yeonchon, which were isolated in Korea and showed serological differences with serovar lai, were indistinguishable from serovar lai in this DNA study using AP-PCR and ribotyping. These results demonstrate that Korean leptospiral isolates were closely related in DNA level, and ribotyping would be useful for subgrouping of field isolates.
