2.A Case of Churg-Strauss Syndrome with Bilateral Pleural Effusions.

A 26-year-old man with a one-year history of asthma and sinusitis presented with bilateral pleural effusions, patch basilar infiltrates on a chest x-ray and a pericardial effusion on an echocardiogram. The peripheral blood showed marked eosinophilia. An obstructive pattern was also observed during the pulmonary fuction test, which was responsive to bronchodilator inhalation. Nerve conduction studies showed right sural neuropathy. Thoracentesis yielded an acidotic exudative effusion with low glucose, low C3 and eosinophilia. An open lung biopsy revealed an eosinophilic interstitial pneumonitis associated with a necrotizing eosinophilic vasculitis, and granulomatous inflammation foci. In the literature, pleural effusions were reported in 29 percent of Churg-Strauss patients, but the number of effusions was low and their characteristics have not been well described. This report describes the characteristic findings of pleural fluid and its histologic features in a case of classical Churg-Strauss syndrome.

4.Hypotensive Efficacy and Safety of Manidipine on the Patient with Essential Hypertension.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

5.Effects of Music Therapy and Rhythmic Exercise on Quality of Life, Blood Pressure and Upper Extremity Muscle Strength in Institution-Dwelling Elderly Women.

PURPOSE: The purpose of this study was to determine the effects of music therapy and rhythmic exercise on health related quality of life, blood pressure and upper extremity muscle strength in the institution-dwelling elderly women. METHODS: The study was designed using a nonequivalent control group pretest-posttest design. The participants consisted of 35 elders (18 in the experimental group and 17 in the control group). The music therapy and rhythmic exercise were developed by the investigators. The experimental group took part in this program twice a week for 8 weeks. The Short Form 36 health survey questionnaire, blood pressure and grasp power scale were used as instruments. The data were analyzed using SPSS 14.0. RESULTS: Repeated measures ANOVA revealed that music therapy and rhythmic exercise had positive effects on quality of life, especially on vitality, general health and mental health. Also, there were statistically significant differences in diastolic blood pressure and upper extremity muscle strength between the pretest and posttest in the experimental group. CONCLUSION: The study suggests that this program can be applied for older women in long-term facilities to improve quality of life, blood pressure and upper extremity muscle strength.

8.Dislocation of the Elbow in Children.

PURPOSE: The purpose of this study was to analyze the clinical manifestations of elbow dislocation in children and to suggest an appro-priate treatment modality for this rare injury. MATERIALS AND METHODS: We analyzed 17 cases, treated from September 1989 to July 2000. The factors examined included; age, sex, etiology, direction, associated injuries, method of treatment, results and complications. Clinical results were evaluated using the grading system of Linscheid and Wheeler. RESULTS: The mean age was 10 years 4 months. Etiologies were; fall from a height 8 cases, slip down 7 cases and unknown 2 cases. The directions of the dislocations were posterior 14 cases, posterolateral 2 cases, and posteromedial 1 case, with no anterior dislocation. Associated injuries were noted in 14 cases (82.4%), and 8 of these cases were treated by open reduction and internal fixation. The remaining 9 cases were treated by closed reduction followed by a long arm cast. The mean period of immobilization was 3.8 weeks. One case was complicated with a myositis ossificans, which resulted in spontanous remission. Results were excellent in 15 cases, and good in 2 cases with no poor cases. CONCLUSION: The incidence of associated fractures (82.4%) was higher in this study than in previous reports. A careful examination is necessary to identify associated injuries and achieve stability with the surgical procedure, which should be followed by early active exercise.

10.The Effect of Intraurethral 2% Lidocaine Gel as Topical Urethral Anesthetic During Cystoscopy.

Despite current practice there is no evidence to demonstrate the efficacy of intraurethral 2% lidocaine gel as an anesthetic for rigid cystoscopy. To evaluate the usefulness of lidocaine on decreasing pain associated with cystoscopy, we performed a randomized controlled study comparing 2% lidocaine gel with a plain water based lubricant. Pain was recorded on a visual analogue scale by the patient and by the physician. Physician perception of pain experienced by the patient was compared with the actual pain experienced. We found decrease in pain perception in men following lidocaine gel instillation with a 5 or 10 minute dwell time compared to instillation of the plain lubricant.
