1.Immunohistochemical Study of S-100 Protein in Salivary Gland Tumors.

An immunohistochemistry for S-100 protein by biotin avidin system technique was done to evaluate the existence and distribution pattern of S-100 protein positive cells in various obtained were as follows. 1) Positive immunostaining for S-100 protein was observed in myoepithelial cell, serous acinar cell and nervous bundle in normal salivary gland. 2) Strong immunoreactivity for S-100 protein was shown in plemorphic adenoma, which was localized not only in myoepithelial cord or sheets of epithelial portion but also in chondrocytes, stellate cells of myxoid stroma and in squamous keratin pearl of mesenchymal metaplastic foci. 3) The S-100 protein was demonstrated in the tumor cells of tubular adenoma, acinic cell tumors and in epidermoid area of mucoepidermoid tumors. 4) Immunoreactivity for S-100 protein, however, was not found in the tumor cells of adenoid cystic carcinoma and adenolymphoma except for stroma reticulum cells. 5) Intensity of positive reaction for S-100 protein varied from cell to cell: Some had intense immunoreactivity, whreas others were only weakly positive or completely negative, even in myoepithelial cell nest of the same pleomorphic adenoma.
2.Correction of malunited zygoma through limited exposure.

This study includes 40 patients of post-traumatic facial bone deformities to whom underwent realignment of zygomatic bone. In 13 cases, we modified the approach technique using limited exposure such as subciliary, intraoral and preauricular incisions instead of conventional wide exposure. Preoperative evaluation of deformity was done by using x-rays, photographs and detail communication with the patients. Through this limited exposure, osteotomy and repositioning of the zygoma were accomplished. There were no postoperative differences between wide and limited exposure, but limited exposure was more advantageous in terms of reduced operative time, transfusion, hospitalization and scaring. This approach was an effective method for the relatively simple tetrapod type-malunited fracture of zygoma without comminution.

4.Congenital Ball-and-Socket Ankle Joint: A Case Report

The congenital ball-and-socket ankle joint is a rare condition and is associated with congenital shortening of the lower extrimity and various skeletal abnormalities of the foot. This disease entity was reported in the German literature by Politzer in 1931 and in the English literature by Lamb in 1958. Several series have been reported since, suggesting that the condition may not be as rare as generally thought. This case is, to our knowledge, the first reported in this country.

Ossification of the ligamentum flavum (OLF) has been recognised as a definite clinical entity as an ossification of the posterior longitudinal ligament. It has been known that the incidence of OLF is high in Japan and OLF usually occurs in the thoracic and lumbar region. Recently OLF has received considerable attention as a cause of myelopathy. OLF is quite distinct from “hypertrophy” of the ligamentum flvum. We experienced 2 cases of OLF with neurological symptom, which were treated by decompressive laminectomy and removal of the ossified ligamentum flavum. So remarkable symptomatic improvement was obtained.

8.Clinical Survey of Patients of Intensive Care Unit.

We have analyzed 785 ICU patients admitted between May 24, 1989 and April 30, 1990, to obtain better guidance and management in the ICU. The results of analysis are as follows: 1) Total number of patients was 785; 459 male and 326 female. The number of patients admitted was 337 (42.9%) from the Department of neurosurgery, 259 (33.0%) from internal medicine and 70 (8.9%) from genreal surgery. 2) 183 patients were involved in 51~60 age group, 166 in 61~70 age group, 138 in 41-50 age group and 85 in over 71 age group. 3) The number of patients by admission days was 223 (28.4%) in 3~4 days group, 197 (250%) in 1~2 days group and 128 (16.3%) in 5~6 days group. 4) The total number of expired patients was 103 (mortality rate, 13,1%); 66 male and 37 female. The expired 63 patients admitteed in ICU due to medical problems and 40 patients due to surgical problems. The number of expired patients was 54 (52.4%) from the Department of internal medicine, 31 (30.1%) from neurosurgery and 5 (4.9%) from general surgery. 5) The highest mortality group was 51~60 age group by age and 1~2 days group by duration of admission.

10.Excretory bladder: the source of cysteine proteases in Paragonimus westermani metacercariae.

The cysteine proteases of Paragonimus westermani metacercariae are involved in metacercarial excystment, host immune modulation, and possibly in tissue penetration. In order to clarify the origin of the enzymes, 28 and 27 kDa cysteine proteases in metacercarial excretory-secretory products were purified through the FPLC system using Mono Q column chromatography. The polyclonal antibodies to the enzymes were produced in BALB/c mice. Immunolocalization studies revealed that both cysteine proteases were distributed at the linings of excretory bladder and excretory concretions of the metacercariae. It was suggested that the excretory epithelium of P. westermani undertake the secretory function of metacercarial cysteine proteases, in addition to its role as a route for eliminating waste products.
