1.Hospital Services Utilization by Insured and Non-insured Patients for Cesarean Section in a University Hospital.

In order to discover difference that may exist in quantity of medical care services, length of stay and hospital changes between insured and non-insured patients, records for primary Cesarean section patients discharged between July 1978 and June 1980 from a university hospital were examined. In addition, Cesarean section rates among the total deliveries for a two-year period between the groups were studied. The results showed that volume of services was greater and length of stay was longer among the insured, however, charges were higher among the non-insured. Cesarean section rates were statistically significantly different between insured and non-insured patients for every age group except the group of 35 or more.

2.Comparison of ANG II-mediated Ca2+i, IP3 Production and ATPi in Isolated Renal Proximal Convoluted Tubules of Adult SHR and WKY.

BACKGROUND: The renin angiotensin syaimstem plays an important role in hypertension. Therefore, the purpose of this study was to investigate the comparison of responsiveness to angiotensin II (ANG II) in isolated renal proximal convoluted tubules of spontaneously hypertensive (SHR) and Wistar Kyoto (WKY) rats. METHODS: Intracellualr calcium concentration ([Ca2+i) was measured using Fura- 2/AM, inositol trisphosphate (IP3) accumulation was determined by radioimmuno assay and cellular ATP content measured using the microchemilunescene method in renal proximal tubule suspension or isolated renal proximal tubules. RESULTS: When measured the ANG II-induced [Ca2+i, the young rats showed a greater response to ANG II than adult rats in both strains. The ANG II (10-7 M)-induced [Ca2+i transient in the cortical tubule suspension from adult SHR was significantly lower than that in age-matched WKY. In isolated proximal tubule segments, ANG II-induced [Ca2+i increment was only observed in S1 segments. Comparing responsiveness to ANG II in SHR and WKY, similar phenomenon was observed as experiment using tubule suspension. IP3 accumulation by ANG II also attenuated in adult SHR. The 20-minutes incubation without any exogenous substrate in proximal convoluted tubule (S1) significantly decreased cellular ATP content and ANG II (10-7 M) inhibited decrement of cellular ATP level. The effect of ANG II on cellular ATP restoration was disappeared by the treatment with losartan. CONCLUSION: From these results, the responsiveness of ANG II to AT1A receptor is attenuated in the proximal convoluted tubules of adult SHR comparing the age- mached WKY.

4.A Case of Giant Keratoacanthoma.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

6.The Effect of Intraverious Continuous Infuction Midazolam against Refractory Status Epilepticus in Children.

PURPOSE: Refractory status epilepticus refers to patients who do not respond properly to adequate first-line drug treatment such as diazepam, phenytoin, phenobaribital, lorazepam and show longer than 60 minute seizure. This study was designed to determine the efficacy and safety of midazolam given as a continuous infusion in the treatment of status epilepticus in children. METHODS: Midazolam was infused to twelve children with seizures, for whom two time repeated doses of 0.3mg/kg of diazepam, 20mg/kg of phenobarbital, and 20mg/kg of phenytoin failed to bring the episode under control. All patients received a bolus of midazolam(0.15mg/kg) followed by a continuous infusion at 1microgram/kg/min. The dose was increased every 15 min until the episode of seizure was brought under control. Time required to control seizures, infusion rate, and side-effects were monitored. RESULTS: The mean age of the patient population was 6.06 yrs(range 2 months to 16 yrs; 6 females and 6 males). In 11 patients, seizures were controlled in a mean time of 60.1 min(range 15-180 min). The mean infusion rate was 8.5microgram/kg/min(range 1-20). The total treatment duration was 17.6 days(range 1-54 days). One patient who was confirmed to have cortical dysplasia failed to respond. Two of the patients showed respiratory depression and bradycardia. CONCLUSION: Midazolam is an effective and safe drug to control refractory seizures in children with status epilepticus.

7.Posterior closed Wdge Lumbar Osteotomy in the Kyphotic Deformity of Ankylosing Spondylitis.

Typical features of Ankylosing spondylitis are progressive ankylosis of the sacroiliac joint and ossification of the annulus fibrosus of discs, interlaminar & interspinous ligaments. Fixed kyphotic deformity in ankylosing spondylitis causes many problems, such as difficulty in looking forward, gastrointestinal problems, restricted cardiopulmonary functions and psychotic depression. There are limitations of conservative treatment to delay progression of the disease. So surgical osteotomy for the correction is inevitable. Osteotomy is indicated in poor horizontal vision, poor cardiopulmonary & gastrointestinal function and psychological & social problem. The purpose of this study is to assess the results of posterior closed wedge osteotomy in ankylosed kyphotic deformity. The posterior closed wedge osteotomy was performed for 30 patients in ankylosed kyphotic deformity between April 1995 and February 1997, who were 29 male and 1 female and whose average age at the time of operation was 35.7 years (17-54 yrs). Mean duration of the symptoms was 18 years (11-23 yrs). The level of correction was fully evaluated preoperatively with anterior and lateral radiographs of whole spine. The patient was placed pronely on an operating table which could be angulated in reverse direction. The determined correction level of the spine was exposed through the posterior midline approach. Following the insertion of pedicle screws, transpedicular decancellation was performed with curret and punch forceps. Posterior bony element and pedicle were completely removed. The correction was achieved by gradual extension of the operating table angulated previously and by opposing the surface of osteotomy. Then prebent rod was applied. Walking brace was applied in 2 weeks postoperatively. The osteotomy was performed at single level in 28 cases, double level in 2 cases. The level of osteotomy was distributed as follows; T10: 1 case, T12: 1 case, L2: 3 cases, L3: 14 cases, and L4: 13 cases. Mean correction angle was 48.7degrees (13degrees-72degrees ), and mean increment of height 13.7cm (4-23.5cm). There was no fatal complication. But 5 patients had paralytic ileus and 3 patients visual loss; one was permanent, the others were temporary. Temporary neurologic deficit was also noted in 5 cases. Posterior closed wedge osteotomy with transpedicular decancellation in ankylosed kyphotic deformity is relatively safe and simple procedure.

9.Effects of Low Oxygen Condition on the Development of Mouse Embryos Cultured In Viro.

OBJECTIVE: It is known that mouse embryos before implantation develop in a low oxygen environment of 3- 8% concentration and with antioxidant materials such as vitamins, antioxidant enzymes, ferrous binding proteins, and albumin in follicular and tubal fluids. However, the 20% oxygen culture condition with chemically defined media might be produce an abundance of ROS, and leads to developmental delay or developmental block in vitro. In this study, we attempt to elucidate the relationship between intracellular H2O2 production and embryo development in different oxygen culture conditions of mouse embryos. METHODS: Prenuclear embryos from C57BL/CBA Fl hybrid and ICR mouse were cultured in incubators which provided 5% carbon dioxide, 20% oxygen and 5% carbon dioxide, 5% oxygen. Measurement of H2O2 level in a embryo was performed with DCHFDA(2, 7 -dichlorodihydroflourescein diacetate)and analyzed with Quanti-cell 700, and the number of blastomeres was counted with DAPI( 4, 6'-diamidino-2-phenylindole). RESULTS: Oxygen concentration of the culture medias was significantly higher in the 20% oxygen environment compared to that of 5% oxygen environment. Culture of mice embryos in high oxygen condition leads to high HO concentrations at 2 cell stage and developmental delay or ""2-cell block"" regardless of the strain. But in a 5% oxygen environment, which is similar to in-vivo conditions HO production was suppressed continuously through out culture and development of embryos was definitely improved. CONCLUSION: These results suggest that there is a difference in the production of ROS or protective mechanism according to the mouse strains and stage of development, and it is thought that in-vitro culture in 5% oxygen environment provides stable in vivo equilibrium but in a 20% oxygen environment there is production of ROS which overcome the protective mechanism which leads to cellular damage and embryo developmental delay.
