1.A Case of Tuberculosis VErrucosa Cutis.

One case of tuberculosis verrucosa cutis associated with inguinal lymphnode enlargement is presented. The patient, 24 years old male, has had palm-sized, dark-brown colored, indurated verrucous patch on left buttoek since 12 years old. Diagnosis was confirmed by characteristic clinical appearance, laboratory findings and histopathology. So he has been treated with I.N.A.H., P.A.S., streptomycin and pyridoxine. Remarkable effect was observed.

3.Relationships Between the amount of the Premium and Benefits and Utilization of Enrollees in a Health Insurance Cooperative.

This study attempts to assess the effect of the 1st class health insurance program to the income redistribution among the participants in a unit health insurance cooperative. One health insurance cooperative, located in Seoul, with 1558 members and 768 households was selected for this purpose. The relationships between amount of premium payed and benefits from the cooperative were compared. Necessary data were obtained from the bills submitted to the health insurance cooperative by the contracted medical institutions from 1st January 1977 to 30th June 1979. Households and individuals were the unit of the assessment. The indicators measuring income redistribution effect were the ratios between the benefit and expected benefit and the ratios between the mean benefit. The major finds were : 1. The ratios between the benefits and the expected benefits were lower than 1 in the high income group and greater than 1 in the middle and lower income group. This fact imply that the income redistribution effect was shown in the studied group. It was shown that the middle income group received the greatest benefit, and then the lower income group. 2. The ratios between the benefit and the mean benefit of the households in the higher standard income grade, were found to be higher. This means that the equity of the benefits of households were not achieved by the policy of the health insurance plan. 3. The health insurance utilization rates of the higher standard income group, measured by the household unit, were higher, and by the individual unit, the same rates of the middle income group were higher than other groups.

4.A Case of Imcontinentia Pigmenti.

One case of Incontinentia pigmenti associated with convulsion has been presented and she has delayed growing and emerciation. The patient who was four months old girl had been affected irregular mesh-like dark brown or chocolate colored pigmentation on both leg (inner surfeee). Diagnosis was confirmed by characteristic clinical appearance, laboratory findings and histopathology.

6.Computerization of radiology departmental management using the personal computer: application in medium sized hospital.

The author developed a computer program for use in registration, monthly statistics, printing of reports, data storage and retrieval in the Radiology department, This program has been used in the Department of Radiology, MoonHwa Hospital since November 1990. This program was written in FoxBASE language, consisted of two independent subprograms, one installed in a registration computer without a printer and another in a reporting computer with a printer. The subprograms were designed to link their data by floppy disk. Each computer's hard disk contained permanent files for retrieval and temporary files for data input. All permanent files were indexed on several keywords including the patient's identification data. 1. Registration was performed easily and rapidly. 2. A monthly statistic was obtained simply. 3. Retrieval of the results of previous radiologic studies, printing of report, storage and indexing of data were achieved automatically. This program had merits of simple operation, large storage capacity, rapid retrieval speed, relative low price, easy adjustment for other hospitals. Therefore this program is considered to be an economic substitute for computerization of radiology departmental management in medium sized hospitals.

7.Reduction Mammaplasty by the Inferior Dermal Flap (Modified Mckissock Method).

The goal of reduction mammaplasty is a breast with natural contour and volume, aesthetically situated scars, and a well-placed, sensate nipple and areola. The most successful techniques achieve this through the excision of tissue from the lower part of the breast based on some variation. However, the Mckissock's vertical bipedicle technique is the popular method for reduction mammaplasty. As an alternative modified Mckissock's method, we have found the use of only an inferior dermal flap with a keyhole pattern to be a simple and safe method for obtaining satisfactory aesthetic results. We have used inferior dermal flap for 4 patients recently and obtained the advantages as below compare to the Mckissock's method. 1. Rich blood supply to the broad based inferior flap. 2. More easy transposition of the nipple and areola. 3. More good operation field for resection of breast tissue. 4. Short operation time. 5. Can applied to the gigantomastia.

8.Radiological Evaluation of Posterior Cruciate Ligament Injury

The evaluation of posterior cruciate ligament injury of the knee should be objective. Then stress machine, Gonylaxometer, knee ligament arthrometer (K-T 1000) and Genucome were introduced recently, but they have many problems such as soft tissue error, expensiveness and low popularity. Authors modified the radiological assessment of anterior cruciate ligament injury, reported by Hooper(1986), and applied to the evaluation of posterior cruciate ligament injury. The patient was positioned supine with the one side of leg held on a support so that the knee was flexed approximately 45°to 60°. The thigh was unsupported and a weight(0.9 Kg to 4.5 Kg with increment of 0.9 Kg) was placed on the anterior leg (just distal to the patellar tendon) and lateral view had been taken in this stressed position. Same procedure was done on the other side and the posterior displacement of both knees were compaired. The differences of posterior displacement between both knees of 50 normal adults were studied and 9 cases of posterior cruciate ligament injury were studied, too. The following results were obtained. 1. The difference of posterior displacement could be measured objectively and it is useful for the diagnosis of posterior cruciate ligament injury and the objective evaluation after conservative or operative treatment of posterior cruciate ligament injury patient. 2. The differences of posterior displacement between both knees of 50 normal adults were ranged from 0 to 3 mm with a mean of from 0.88 ±0.87mm to 1.14 ±0.94mm. 3. The differences of posterior displacement between injured knee and normal knee of posterior cruciate ligament injury patient were more than 6mm in 9 patients. 4. The radiological method was inexpensive and easy to perform.
