2.Mesenteric Lymphadenitis Due to Yersinia enterocolitica: A case report.

Mesenteric lymphadenitis due to Yersinia enterocolitica infection is not common in Korea. Although most cases of Yersinia enterocolitica-induced mesenteric adenitis are self limited, cardinal features of Yersinia enterocolitica-induced mesenteric adenitis are so similar to those of acute appendicitis that some of the patients undergo laparotomy with suspected appendicitis. The findings on laparotomy in such patients are usually enlarged mesenteric nodes with a normal or slightly inflamed appendix. Because histologic examination of the removed mesenteric lymph nodes reveals reactive hyperplasia in most cases, it is usually difficult to suspect Yersinia enterocolitica infection on morphology of the resected nodes. But suppurative granulomata of mesenteric lymph nodes, uncommonly encountered in Yersinia enterocolitica infection, strongly suggest yersinial infection. We report a case of mesenteric lymphadenitis in a 10-year-old boy, who underwent laparotomy with suspected acute appendicitis. The removed lymph node showed several suppurative granulomata in the cortex, suggesting yersinial infection. Serologic study confirmed Yersinia enterocolitica serotype O:3 infection.

3.Rapidly Progressive Osteonecrosis of the Humeral Head after Arthroscopic Bankart and Rotator Cuff Repair in a 66-Year Old Woman: A Case Report.

Humeral head chondrolysis has been widely reported as a devastating complication after arthroscopic shoulder surgery; however little is known about post-arthroscopic humeral head osteonecrosis. We experienced a 66-year-old female patient with rapidly progressive osteonecrosis of the humeral head only seven months after arthroscopic Bankart and rotator cuff repair. The patient had no systemic risk factors for osteonecrosis. A satisfactory result was achieved with reverse total shoulder arthroplasty for severe humeral head destruction and an irreparable massive rotator cuff tear. Shoulder surgeons should be aware of such severe complication, perform routine radiographs, and pay close attention to the presence of constant pain or loss of motion after arthroscopic shoulder surgery.

5.Depositions of Complement Components and Their Inhibitors in Atuto - immune Dermatoses.

The complement system is known to be involved in the pathogenesis of the skin lesions in pernphigus vulgaris, bullous pemphigoid, dermatitis herpetiformis, epidermolysis bullosa acquisita, and systemic lupus erythematosus. Authors examined the skin specimens of each disease cases, who did not show any evidence of complement deficiency, to determine the deposition of complement components(C4, C3, Chb-9) and their inhibitors(C4bp, Factor H, S-protein) by modified direct immunofluorescence. We also looked at the staining pattern and localization, for further insights of their pathobiologic contributions in each disease. The findings of deposits of complement components up to C9, as well as inhibitor proteins at the primary histopathologic sites, in the majority of those cases, may indicate that the complement system, to certain extent, involves the inflamrnatory reactions in these diseases. The co-localization of C5b-9 and S-protein could be regarded as the consequence of in situ formation of SC5b-9 complexs or as the result of non-lytic adsorbed complexes of fluid phase SC5b-9. The pathologic role of the complement seems to depend mostly on the complement-fixing biologic property and the amount of the tissue bound immune complexes, which are often heterogeneous to different diseases and among different patients.

6.The Effects of Self-foot Reflexology on Urinary Incontinence in Middle-aged Women.

PURPOSE: This study was to identify the effects of self-foot reflexology on urinary incontinence symptoms, vaginal contraction and daily life discomfort of middle-aged women. METHOD: A quasi-experimental design was used. The subject were 39 middle-aged women with urinary incontinence, who were composed of 18 women in the experimental group, while 21 were in the control group. In the experimental group, self-foot reflexology was applied for 30 minutes, three times a week for 4 weeks. The obtained data were analyzed by using the Mann-Whitney U test of SPSS. RESULT: 1. Frequency(U=78.00, P=.001), amount(U=65.00, p=.001) and the situation score(U=81.00, P=.002)of urinary incontinence were reduced significantly in the experimental group as compared to the control group. 2. Maximum pressure(U=33.50, p=.000), mean pressure(U=38.00 p=.000) of vaginal contraction were improved significantly in the experimental group as compared to the control group. 3. Daily life discomfort in the experimental group was reduced whereas that of the control group was increased(U=63.00, p=.000) significantly. CONCLUSION: These findings indicate that self-foot reflexology is an effective method for reducing urinary incontinence symptoms and daily life discomfort and for increasing pressure of vaginal contraction of middle-aged women. Therefore self-foot reflexology can be considered as a independent nursing intervention for urinary incontinence.

7.Antibiotic Sensitivity to the Causative Organism of Acute Simple Urinary Tract Infection for Recent 3 Years.

BACKGROUND: We studied the antibiotic sensitivities to the causative microorganisms of acute simple urinary tract infection for recent 3 years. METHODS: We analyzed 112 microorganisms and their antibiotic sensitivities of the 104 patients who were admitted to or visited the Department of Urology, Catholic University St. Marys Hospital and had more than 10cfu/mL on urine culture from June 1996 to January 1999 retrospectively. RESULTS: The chance of gram negative and positive as causative microorganisms was 72.3% and 27.7% respectively. The most common pathogenic microorganisms were Escherichia coli (67.0%) followed by a-hemolytic streptococci, Entercoccus. In gram negative acute UTI, imipenem, sulperanzone showed relatively higher sensitivity, while cotrimoxazole, ampicillin showed relatively lower sensitivity. In gram positive, vancomycin, penicillin showed relatively higher sensitivity, while ampicillin, imipenem showed relatively lower sensitivity. CONCLUSIONS: We consider that gram negative microorganisms, especially E. coli, is the main cause of acute simple UTI. But, we should be concerned about the increase of gram positive organisms and other gram negative organisms besides E. coli. Regarding to the choice of adequate drug in the treatment of UTI, it is necessary to consider the change of pathologic microorganisms.

8.Difference of Physical Symptoms, PWI and JCQ according to Sasang Constitutions for Industrial Workers.

PURPOSE: This study was conducted to analyze industrial workers' stresses from physical symptoms, PWI, JCQ and tasks in classification of Sasang constitutions. METHOD: Data were collected from industrial workers in J Province from May 2003 to June 2003. Questionnaires were distributed and collected on the day of their physical examination. Collected data were analysed through chi2-test. RESULT: Health-related characteristics showed that most of smokers and drinkers were Taeumins and this result was statistically significant. Physical symptoms related to constitutions indicated that GI and neurologic symptoms are significantly more frequent in Soeumin. The percentage of subjects with moderate risk was high in order of Soyangin, Taeumin and Soeumin while the percentage of subjects with high risk was high in order of Soeumin, Taeumin and Soyangin. CONCLUSION: Through this study, Lee Jae Ma's theory was be confirmed compatible with disease characteristics. However, research with more various subjects and variables needs to be made.

9.Actual State of Recognition and Experience of Love, Sex Knowledge and Self-esteem in Girl's Middle and High School Students.

PURPOSE: This study was to identify the state of recognition and experience of love, sex knowledge and self-esteem targeting girls who attended middle and high schools. METHOD: Data was collected from 391 girls of middle and high schools from Aug. 2004 to Nov. 2004. The collected data was analysed through t-test, and ANOVA. RESULT: The average age of the girls of this study was 16.9 years, and 31.6% of them discussed sex with their parents. The girls' experiences with love were hand-holding(80.8%) the most, and shoulder to shoulder(68.5%), arm in arm(67.8%), hugging(57.0%) and kissing(37.9%) in sequence, while coitus and pregnancy appeared in 0.8% of the cases. The score of sex knowledge was 68.8 and self-esteem was 74.4. Self-esteem was related to age and discussing sex with their parents. CONCLUSION: As the results above show, the majority of girl-students were mostly knowledgable about friendship and love expressions. At school and home, society's support and encouragement are required so that sex education at a young age and a positive self-concept may be formed.

10.A Study on Self-Achievements and Related Factors in Elders.

PURPOSE: This study was to identify the relationship among self-management, economic stability, self-esteem, self-achievement and predictor of the self-achievements in elders. METHODS: A total of 294 subjects aged between 60 and 92 were selected through convenient sampling. Data were collected by self-reported questionnaire from May to June, 2008. Collected data were analysed by the SPSS/WIN 15.0 program. RESULTS: Self-management, economic stability, self-esteem, self-achievement were positively correlated. The most significant predictor of self-achievement in elders were self-management and self-esteem. CONCLUSION: These findings suggest that self-achievement is influenced by self-management and self-esteem and needed for aged well.
