1.A Case of Epidermolysis Bullosa Dystrophica.

A case of Epidermolysis Bullosa Dystrophica is reported with review of pathogenesis with literatures. Epidermolysis Bulloaa Dystrophica is a rare genetic disease which is characterized by the formation of blister upon the minor trauma or friction to the skin. A 7-year old boy was referred to our dept. in Aug. 1976 because of a severe bullous eruption primarily on his hands and feet. On admission tense bullae were noted on the dorsa of hands and feet which is usually serous but may be hermorhagic tendency. Soon thereafter tbis bullous lesion tend to break down into painful shallow denudation which may leave depigmented scars and milia after healing. Family history was negative for skin disorders.

2.Clinical Trials of Frason on Various Dermatosis.

Esperson(R), a new topical steroid ointment containing 0. 05% 17-Desoxymethasone,. was used in several kinds of dermatoses. Framycetin which was isolated and uientified by the research organisation of Roussel Laboratories (SOFRAPEN) in 1950 from culture broths of a strain of Streptomyces species is a wide spectrum antibiotics on various bacterial infectious diseases. Total 50 patients including 10 cases of 2ndary infected Atopic dermatitis, 10 of infected Contact dermatitis, 6 of infected Seborrheic dermatitis, 7 of infected Neurodermatitis, 3 of infected Chronic dermatitis, 5 of lmpetigo, 2 of infected Nummular eczema, 2 of infected Infantile ecze;na, 1 of infected Intertrigo, 1 of Eczematoid Otitis extema, 1 of 2nd degree Bum, 1 of infected Pruritis ani, 1 of infeeted 1nsect bite. were treated with Frason(R) (0.05% 17-desoxymethasone & 0.5% Framycetin combination). Among these 50 patiants, we observed excellent effects in 7 patients (14%), good effect in 29 patients (58%), fair effect in 10 patients (20%), Totally 36 patients (72%) revealed excellent to good therapeutic effects on their skin conditione after unoccluded application of Frason(R). 3 times daily for about 14 days. There was no sid ffect in all subjects after application of Esperson-F.

5.Posterior closed Wdge Lumbar Osteotomy in the Kyphotic Deformity of Ankylosing Spondylitis.

Typical features of Ankylosing spondylitis are progressive ankylosis of the sacroiliac joint and ossification of the annulus fibrosus of discs, interlaminar & interspinous ligaments. Fixed kyphotic deformity in ankylosing spondylitis causes many problems, such as difficulty in looking forward, gastrointestinal problems, restricted cardiopulmonary functions and psychotic depression. There are limitations of conservative treatment to delay progression of the disease. So surgical osteotomy for the correction is inevitable. Osteotomy is indicated in poor horizontal vision, poor cardiopulmonary & gastrointestinal function and psychological & social problem. The purpose of this study is to assess the results of posterior closed wedge osteotomy in ankylosed kyphotic deformity. The posterior closed wedge osteotomy was performed for 30 patients in ankylosed kyphotic deformity between April 1995 and February 1997, who were 29 male and 1 female and whose average age at the time of operation was 35.7 years (17-54 yrs). Mean duration of the symptoms was 18 years (11-23 yrs). The level of correction was fully evaluated preoperatively with anterior and lateral radiographs of whole spine. The patient was placed pronely on an operating table which could be angulated in reverse direction. The determined correction level of the spine was exposed through the posterior midline approach. Following the insertion of pedicle screws, transpedicular decancellation was performed with curret and punch forceps. Posterior bony element and pedicle were completely removed. The correction was achieved by gradual extension of the operating table angulated previously and by opposing the surface of osteotomy. Then prebent rod was applied. Walking brace was applied in 2 weeks postoperatively. The osteotomy was performed at single level in 28 cases, double level in 2 cases. The level of osteotomy was distributed as follows; T10: 1 case, T12: 1 case, L2: 3 cases, L3: 14 cases, and L4: 13 cases. Mean correction angle was 48.7degrees (13degrees-72degrees ), and mean increment of height 13.7cm (4-23.5cm). There was no fatal complication. But 5 patients had paralytic ileus and 3 patients visual loss; one was permanent, the others were temporary. Temporary neurologic deficit was also noted in 5 cases. Posterior closed wedge osteotomy with transpedicular decancellation in ankylosed kyphotic deformity is relatively safe and simple procedure.

7.Comparison of Functional Magnetic Stimulation Therapy with Biofeedback in Patient with Female Urethral Syndrome.

PURPOSE: The aim of this study was to evaluate the effectiveness of functional magnetic stimulation (FMS) compared to biofeedback (BFB), with functional electrical stimulation (FES), in patients with recurrent or incurable female urethral syndrome. MATERIALS AND METHODS: Fifty women with female urethral syndrome, based on the criteria of the NIADDK (the National Institute of Arthritis, Diabetes, Digestive and Kidney Diseases), and refractory to antibiotics therapy for more than two months, were consecutively assigned to one of the two treatment modalities. The BFB, with FES, group (32 patients) was treated with biofeedback and electrical stimulation. The FMS group (18 patients) received functional magnetic stimulation. Sessions of 30min were performed twice a week for 6 to 8 weeks. A detailed clinical evaluation and cystoscopy were performed prior to treatment. The results were determined by obtaining pre and post Bristol female lower tract symptoms questionnaires and from the subjective overall satisfaction. RESULTS: The irritative and obstructive voiding symptom scores improved markedly in both groups (p<0.05). The score for the quality of life decreased from 4.97+/-1.0 to 2.25+/-1.74 in the BFB with FES group, and from 4.96+/-1.64 to 2.89+/-2.32 in the FMS group. Improvements in the quality of life and pain relief were shown in both groups. 13 of the 32 patients (40.6%) in the BFB, with FES, group and 5 of the 18 patients (27.8%) in FMS group, were subjectively satisfied with their treatments. Minor complications, such as discomfort, unpleasantness and a vaginal itching sense, were note in 30% of the patients in the BFB, with FES, group only. CONCLUSIONS: FMS, compared to BFB, with FES, results in a lesser improvement of the symptom and subjective satisfaction of patients with recurrent or incurable female urethral syndrome, but may be an acceptable alternative to other more invasive treatment modalities.

8.Tumors in the Sacrum: 7 cases report

Tumors of the sacrum are relatively uncommon. The diagnosis is difficult, especially in the early phase of the disease, because the clinical manifestations are nonspecific, the clinical signs may be vague and the radiologic changes are needed to differentiate from intestinal gas shadow. The clinical features are sacral pain, sensory change on perineum, urination difficulty, constipation, and sciatica. Seven cases of sacral tumor were treated at the Department of Orthopaedic Surgery, Kyung Hee University, from October 1981 to October 1992, and the results were as follows: 1. Because of the variability of the sacral tumor, we cannot define the most common tumor. 2. There is difficult in early diagnosis of the tumor, because the symptoms of tumor are similar to that of lumbar disc herniation and the radiologic findings are subtle. 3. CT and MRI are very effective methods to detect the tumor in the sacrum. 4. Curettage, excision, resection, sacrectomy and internal fixation were done according to the kinds of tumors and the amount of bony destruction.

9.Human Papilloma Virus Type 16 E7 Genes Protect Astrocytes against Apoptotic and Necrotic Death Induced by Hydrogen Peroxide .

Hydrogen peroxide is considered to be a dose- and time-dependent mediator in apoptotic and necrotic death. In this study, we examined the signaling of the E6 and E7 proteins with respect to apoptosis or necrosis after H2O2 injury using an in vitro model with overexpressed E6 or E7 genes. For this purpose, the E6 and E7 gene expressing astrocytes were exposed to 0.01 mM and 0.2 mM H2 O2 solutions. Twenty- four hours after treatment with the lower dosage(0.01 mM H2O2), control, E6-expressing cells suffered about 45% injury and LXSN-expressi ng cells decreased by 67% as assessed by LDH release. However, E7-expressing cells showed less injury, resulting in 20-30% of LDH release. Astrocytes expressing E6, E7, LXSN and mock-infected cells showed a typical apoptotic death patter n on the DNA gel after treatment with a low-dose of H2O2 (0.01 mM), however the y died from necrotic death after a high-dose (0.2 mM) H2O2. Overexpression of HPV-E7 genes protected the cells from apoptotic death after a low-dose of H2O2 and from necrotic death after a high-dose of H2O2, while the overexpression of E 6 genes from the necrotic death. E7 expressing astrocytes showed higher catalas e activity and the levels of E2F protein surged more than 100-folds compared with the control astrocytes. We believe that the activity of E7 protein to protect astrocytes from H2O2 injury was at least partly due to increased catalase, a scavenger protein.

10.The Video-Urodynamic Findings of Spinal Cord Injury Patients according to the Injured Level.

PURPOSE: Spinal cord injuries can lead to varying degrees and patterns of neurologic deficits, which depend on the level of the injury, duration, completeness. So we analyzed the relationship between the types of neurogenic bladder and the level of injury in the spinal cord injury. MATERIALS AND METHODS: From June 1997 to August 2002, one-hundred twenty seven spinal cord injury patients were studied by neurological and urological examination and video-urodynamic evaluation. The neurogenic bladder in spinal cord injury were analyzed into five types according to the injured level. RESULTS: The 94 males and 33 females had a mean age of 45.7+/-15.1 years (range 15 to 92). Mean follow up interval after injury was 33.9+/-48.4 months. Injured level was classified into five levels; cervical, thoracic, lumbar, and sacral. Of the 127 patients 37 (29.1%) was cervical, 18 (14.2%) was thoracic, 33 (26.0%) was lumbar, and 39 (30.7%) was sacral. When all patients were divided suprasacral and infrasacral lesion. Typically, suprasacral lesions show detrusor hyperreflexia, and infrasacral lesions have detrusor areflexia. 30 (37.5%) of 88 suprasacral lesions and 12 (30.8%) of 39 infrasacral lesions were presented to atypical. 35.7% of patients have combine spinal lesion, but 47.6% of patients have no causes for atypical patterns. 3 (27.3%) of 11 patients were followed up, they changed neurogenic pattern. CONCLUSIONS: In our results, video urodynamic findings showed that about 30 percent patients had atypical patterns of neurogenic bladder, and they have underlying disease about 52.4%. And 27% of followed patients changed neurogenic pattern of bladder. So, urodynamic study have an important role in the spinal cord injury for diagnosis of neurogenic pattern and selection proper treatment, catching out the interval change and correction of treatment method, and evaluation of prognosis.
