1.Cloning of Chromosomal Band Specific cDNA - cDNA related with neural development-.

Recently, surmountable amounts of genes are being cloned without information about them and it has become neccessary to develop new techniques for discovering genes with more informaiion like as chromosomal location and possible functions. We have developed one such a method and applied it to search for genes that may be related with the neural development. The mRNAs were extracted from cerebral cortex of 18 week old human fetus, cDNAs were made by reverse transcription from these mRNAs and Uni-amp cDNAs having Uni-amp adapters at both ends were made for subsequent PCR. To observe the distribution of the Uni-amp cDNAs on the chromosome, fluorescent in situ hybridization was performed with biotin labeled Uni-amp cDNAs. Among the chromosome bands showing strong hybridization with the cDNAs, 7q22 was microdissected from the chromosome hybridized with unlabeled Uni-amp cDNAs and amplified by PCR with Uni-amp primers. These amplified cDNA fragment were subcloned to vectors and the nucleic acid sequences were analysed. As a result, 46 different clones were obtained. They were categorized as 12 clones of well characterized genes, 14 clones showing low homology with known genes, 13 clones of simply registered uncharacterized human cDNAs, 7 clones of unknown genes. In situ hybridization histochemistry of 34 novel genes, except 12 known genes, were performed on developing and adult rat tissue sections to see the tissue specificity and developmental expression of these genes. The expression of several novel genes were restricted to the nervous system. From these results, it may be suggested that our technique is very useful to clone the genes expressed in the developing human braine with confirmed chromosomal location. In addition, this cloning technique can be used to discover the new genes related with neural development in combination with functional screening methods.

3.A Case of Pheochromocytoma Presented with Acute Myocardial Infarction.

A 36-year-old woman was presented with extensive anterior wall myocardial infarction. We tried to perform direct coronary angiography for the purpose of primary stenting. However, coronary angiogram revealed normal coronary arteries without intracoronary thrombi. We continued further evaluations to find out the cause of normal coronary myocardial infarction. The findings of severe hypertensive retinopathy and concentric left ventricular hypertrophy suggested that she had secondary hypertension. The detailed history, laboratory and radiological findings revealed the pheochromocytoma. The tumor was successfully removed by operation.

4.Early and Mid-term Results of Coronary Stenting in the Diabetic Patient.

BACKGROUNG AND OBJECTIVES: Diabetes mellitus is a significant risk factor for adverse outcome after PTCA, which is associated with an increased late mortality and target lesion revascularization (TLR) rates. The beneficial role of coronary stenting on the clinical and angiographic outcomes of diabetic patients is not clearly defined. The aim of this study was to evaluate the early and mid-term outcomes in diabetic patients undergoing elective stenting of native coronary lesions compared with those in non-diabetic patients. MATERIALS AND METHODS: Between July 1997 and June 1998, coronary stenting was performed on 46 lesions in 38 diabetic patients and 126 lesions in 117 non-diabetic patients. Follow-up angiography at mean day of 189+/-45 was performed in 58.7% (91 patients) and analysed by quantitative coronary angiography (QCA). RESULTS: There was a higher incidence of multi-vessel disease in diabetic patients than non-diabetic patients but not statistically significant (71.1% vs 51.3%, p=0.106). There were no differences in major procedural complications and in-hospital events (myocardial infarction, angina and death) in diabetics and non-diabetics. During the follow-up, the incidence of target lesion revascularizton (TLR) and cardiac event free survival did not differ between two groups. CONCLUSION: Coronary stenting in diabetics resulted in a low rate of immediate procedural com-plications and early major adverse cardiac event (MACE), similar to non-diabetics. There were no differences in the mid-term clinical and angiographic outcomes in diabetics and non-diabetics.

6.Clinical Outcomes of In-office Sutureless Amniotic Membrane Transplantation in Persistent Epithelial Defect

Purpose:
To investigate the efficacy of outpatient clinic-based sutureless amniotic membrane transplantation (AMT) along with therapeutic contact lens (T-lens) application in eyes with persistent epithelial defects (PED).
Methods:
Nine eyes of nine patients (mean age, 71.7 ± 5.2 years) diagnosed with PED and treated with in-office sutureless AMT combined with T-lens application were retrospectively reviewed. Demographics, systemic diseases, PED etiology, corneal epithelial defect size, visual acuity, corneal scraping culture results, and clinical course were evaluated.
Results:
Among nine eyes with PED, three had neurotrophic keratopathy, four had infectious keratitis (three with fungal keratitis and one with bacterial keratitis), one had limbal deficiency, and one had marginal keratitis. The mean epithelial defect size (calculated as an average of the horizontal and vertical diameters) was 3.13 ± 1.42 mm, and the mean duration from AMT to epithelial healing was 30.1 ± 10.5 days (range, 14–51 days) in successful trials. The success rates were 77.8% (7/9) per patient and 66.7% (8/12) per trial. The causes of failure in two patients were AMT displacement and uncontrolled infection.
Conclusions
Our results demonstrate that in-office sutureless AMT combined with T-lens application can be used in patients with PED who are refractory to medications. It will be especially helpful for elderly patients because of its easy-to-use method. To achieve successful outcomes with AMT, an appropriate periocular environment as well as infection control need to be considered.

8.The Effect of Piezoelectric Shock Wave Lithotripsy (EDAP LT02) for Pediatric Urolithiasis.

PURPOSE: The efficacy of extracorporeal shock wave lithotripsy (ESWL) in the treatment of pediatric urolithiasis was evaluated, and a retrospective analysis of the feasibility of various methods of pain control for ESWL performed according to the age of the patients. MATERIALS AND METHODS: Between 1987 and 2004, 30 boys and 32 girls diagnosed with urolithiasis were treated. Their records were reviewed and analyzed for metabolic and anatomical anomalies, the size and location of the stones and for the treatment outcomes and complications. RESULTS: The success rates were 27.4, 61.3 and 80.6% after one, two and three sessions of ESWL, respectively, with an overall success rate of 93.5%. The locations of the stone were the upper calyx, the mid calyx, the lower calyx, the upper ureter, the mid ureter and the lower ureter in 6, 4, 7, 15, 4 and 25 cases, respectively. All patients underwent ESWL with a piezoelectric lithotriptor (EDAP LT02). There was only one patient who required general anesthesia. Most of the patients younger than 6 years underwent the ESWL with IM or IV sedation only. Generally, the patients older than 6 years old required no sedation. Pyelonephritis occurred in one patient (1.6%). CONCLUSIONS: ESWL is an efficient and safe treatment modality for pediatric urolithiasis. Patients older than 6 years can undergo ESWL with analgesics or oral sedation only.

Melanocytic nevi are subject to change with age in both clinical and histopathologic findings. In 1991, Cho et al. first reported three cases of lobulated intradermal nevi and suggested their cases represented an unusual form of regressing melanocytic nevus. 10.Paraplegia after transforaminal epidural steroid injection in a patient with severe lumbar disc herniation - A case report -

Background:
Transforaminal epidural steroid injection (TFESI) is a conservative treatment for patients with lumbar disc herniation (LDH). However, there are reports of various complications that can occur after TFESI; among these, paraplegia is a serious complication. Case: A 70-year-old woman who was unable to lie supine due to low back pain exacerbation during back extension underwent TFESI. After injection, there was pain relief and the patient was able to lie supine; however, paraplegia developed immediately. Magnetic resonance imaging confirmed cauda equina syndrome (CES) due to nerve compression from L1–2 LDH. We determined that the patient's LDH was already severe enough to be considered CES and that the TFESI procedure performed without an accurate understanding of the patient's condition aggravated the disease.
Conclusions
It is important to accurately determine the cause of pain and disease state of a patient to establish a correct treatment plan before TFESI is performed.
