1.Progress of Experimental Trichophyton Verrucosum Infection in Guinea Pig.

T. verrucosum Infection has been reported for the first time in 1986 in Korea and has been increasing progressively. To evaluate the progress of clinical and histopathological change of dermatophytosis caused by T. verrucosum, inoculation study, using T. verrucosum isolated from infected human (human strain) and from infected cattle (cattle strain), was done in 24 male albino Hartley guinea pig. Their clinical and histopathological changes were evaluated. In addition, comparison for the growth rate between human strain and cattle strain on Sabouraud's glucose agar was made. The results were as follows: 1. Growth rate on Sabouraud's glucose agar: Cattle strain showed significantly more rapid growth rate than human strain on Sabouraud's glucose rate at 25℃ and 37℃. And cattle strain showed more rapid growth rate at 37℃ than 25℃. But human strain showed no significant difference of growth rate at both temperature. 2. Clinical findings: Initial erythema, scale and crust were developed about 8th after inoculation. All three findings reached maximum severity about 12th to 16th day and disappeared about 30th to 34th day after inoculation. There was no significant difference in progress of erythema, scale and crust between cattle strain and human strain. 3. Histopathological findings: Although mild acanthosis was noticed on the 3rd day after inoculation, the other findings including parakeratosis, intraepidermal abscess, spongiosis and vascular change, cellular infiltration were found on 9th day after inoculation. They reached maximum severity on the 12th day and lasted to the 25th day after inoculation. After that, all three findings were decreased gradually between 29th day and 33th day. On the PAS staining, hyphae and spores were found on the 6th day and disappeared on the 21th day after inoculation. 4. In trichophyton skin test, all of the 24 guinea pigs became positive within average 9.83±1.17 days These findings suggested that dermatophytosis caused by T.verrucosum induced rapid cell mediated immunity and contributed to rapid resolution of the lesion.

3.Three Cases of Kaposi's Varicelliform Eruption.

Kaposi's varicelliform eruption is a more or less generalized infection of the skin, and sometimes of internal organs, with herpes simplex, vaccinia or Coxsackie virus A 16; it appears in people who have atopic dermatitis or some other skin diseases. There is a predilection for infants and children, but no age-group is exempt. We reported 3 cases of Kaposi's varicelliform eruption with atopic dermatitis. They had characteristic multiple umbilicated vesicles on the sites that atopic dermatitis had been involved. A 14-year-old boy and a 2-month-old infant had fever. A 17-year-old boy had wide-spread vesicles. All three patients showed multinucleated giant cells on Tzanck test, that suggests herpes simplex virus origin. They were treated with acyclovir. Within 1 to 2 days after the initiation of the therapy, new lesions had ceased to develop. Most of the lesions were cleared in 7 days without complication.

4.An experimental study on the mandibular growth by the partial glossectomy of the rats.

The purpose of this study was to investigate the effect of the partial glossectomy on the mandibular growth of growing rats. Thirty one Sprague-Dawley rats four weeks old were divided into two groups : the first group served as the control and the second group was partially excised in the tongue of the rats. The experimental animals were sacrificed at 4 and 8 weeks after partial glossectomy. The changes of the rat mandibular growth following partial glossectomy were observed biometrically from dry bone specimens. The findings were as follows: 1. Partial glossectomy of growing rat produced inhibitory effects on the growth of the mandible and did not change the original shape of the mandible. 2. Among the factors affecting mandibular growth, the presence of the proper tongue size was essential. 3. Partial glossectomy of the growing rats could not change general growth of the rats whole bodies.

6.Infection Rate and Serotype Distribution of Orientia tsutsugamushi arnong Field Rodents in Chollanamdo using Nested Polymerase Chain Reaction.

The 86 strains of field rodents captured in Chollanamdo were analyzed its infection rates of Orientia tsutsugamushi by nested PCR. The detection rate of O. tsutsugamushi was 14.3 % in A. agrarius whereas O. tsutsugamushi could not be detected in C. lasiura. In locality, the rodents captured in the mountainous area showed higher detection rate than suburban area. In the case of detection rate by organs, the spleen was most appropriate specimen, but in two cases, O. tsutsugamushi could be detected in only kidney specimens. The major serotype of detected O. tsutsugamushi was serotype Karp, but four cases were serotype Boryong. These serotypes were confirmed by nucleotide sequence determination of amplified PCR products. In conclusion, the serotype Karp was the major O. tsutsugamushi in Chollanamdo.

7.Long-Term Survival Rates and Prognostic Factors for a Hepatocellular Carcinoma after a Curative Hepatic Resection.

BACKGROUND: The prognosis for a hepatocellular carcinoma (HCC) is very poor because of delayed diagnosis caused by the absence of specific clinical manifestations in the early stage, the limitation of the extent of resection, the high postoperative complication rate due to associated liver cirrhosis, and the high recurrence rate due to multifocal tumorigenesis. Among the various kinds of treatment modalities for HCC, surgical resection is still recognized as the first treatment method. However, it is true that surgical resection has many problems, such as a high operative risk and a high postoperative recurrence rate. Therefore, an evaluation of the factors associated with the overall survival rate and with the recurrence rate is very important for improving the results of operative therapy for HCC. METHODS: We retrospectively analyzed the clinical and the pathological results of 44 curative hepatic resections for HCC performed at Chonnam University Hospital from 1991 to 1997. We evaluated 19 clinical and pathological factors by univariate and multivariate analysis, and we calculated the survival rate by using the Kaplan-Meier method. RESULTS: The cumulative 1-, 3-, and 5-year survival rates were 81%, 66%, and 28%, respectively. In 25 of the 44 cases, recurrences developed, and the 1-, 3-, and 5-year recurrence rates were 38%, 60%, and 65%, respectively. Factors with an independent effect on the overall survival rates were multiplicity of tumors, HBs Ag status, and Child classification. However, liver cirrhosis, ascites, prothrombin time, AFP level, and portal vein invasion were not statistically significant. CONCLUSIONS: The significant prognostic factors detected by multivariate analysis were multiplicity of tumors, HBs Ag status and Child classification. A preoperative evaluation for these factors should be done. If early diagnosis and multidisciplinary therapies are done through frequent postoperative follow-up surveys in these high risk groups, we can anticipate better long-term survival rates after a hepatectomy.

8.Atypical Hemolytic Uremic Syndrome Associated with Streptococcus pneumoniae Infection.

Hemolytic uremic syndrome is the most common cause of acute renal failure in childhood. Atypical hemolytic uremic syndrome which is not usually associated with prodromal symptoms has a higher mortality rate and more tendency to progress to chronic renal failure. Children younger than two years of age are a more susceptible to pneumococcal infection and can develop more serious disease. We report a 23 month-old male who suffered from pneumococcal pneumonia and progressed to hemolytic uremic syndrome requiring peritoneal dialysis. He recovered completely from acute renal failure after peritoneal dialysis and intravenous use of vancomycin.

9.166Ho - chitosan as a radiation synovectomy agent - Biocompatibility study of 166Ho - chitosan in rabbits.

We developed the 166Ho-chitosan, the new radiation synovectomy agent. Holmium is the more practical isotope based on its longer half-life. And chitosan, is ideal and suitable particles based on its soluble and biodegradable characteristics. We investigated the biocompatibility of the 166Ho-chitosan complex to evaluate the suitability as a radiation synovectomy agent. In vitro stability test, the 166Ho-chitosan complex suspension in saline was stored at room temperture and 37degrees C for 25 days and decay rate was of determined by ITLC(Instant Thin Layer Chromatography). In vivo stability test, the 166Ho-chitosan complex was injected into rabbit joints and followed by gamma camera imaging to quantify extra-articular leakage. Biodistribution study, the 166Ho-chitosan complex was injected into rabbit joints. After 48 hours heart, liver, urinary bladder, spleen, lung, brain, kidney, blood were extracted and radioactivities were measured. In vitro stability test, there was no significant change of radioactivity and no leakage problem indicating the prepared 166Ho-chitosan complex is sufficiently stable. In vivo stability tests revealed that more than 98% of the 166Ho-chitosan remained in joint over a 2 days period. The mean retention percentage of 166Ho-chitosan in knee were 99.9%, 99.9%, 99.8%, 99.7% at 2 h, 6 h, 1 day and 2 days, respectively. A biodistribution study of the rabbits revealed that leakage to heart, liver, urinary bladder, spleen, lung, brain, kidney, blood is 0.71, 1.5, 0.50, 1.5, 0.25, 0.26, 0.81, 0.065(% Injected Dose x 10-3/gram), respectively. The 166Ho-chitosan complex shows less leakge than any other radiation synovectomy agents. Our results indicate that 166Ho-chitosan have the biocompatibility and the suitability as a radiation synovectomy agent.

10.A Case of Apocrine Adenocarcinoma associated with Basal Cell Carcinoma.

Apocrine adenocarcinoma is rarely reported in the world and only 4 cases are reported in Korean literature. We encountered a case of apocrine adenocarcinoma associated with basal cell carcinoma in a 76-year-old female. Excisional biopsy showed well demarcated basophilic cell mass in the upper dermis and glandular structure in the lower dermis. Basophilic cell mass consisted with basaloid cells and had peripheral palisading arrangement. Glandular structures characterized by cells with abundant eosinophilic cytoplasm showed decapitation secretion. The cells showed nuclear atypicality, plemorphism and prominent nucleoli. The cytoplasm contained PAS-positive, diastase-resistant granules and stains for lysozyme and GCDFP-15(Gross Cystic Disease Fluid Protein-15) were positive.
