With the continuous development of diagnostic techniques of precancerous lesion and an improved patient-transfer system, it is unusual to examine patients who have a life-threating malignant skin lesion. However it is still a problem in some far-advanced inoperable skin tumors which have been neglected and have become aggravated, Another problem in oncologic surgery is previous irradiation around a lesion. The only effective way to treat these lesions is to excise widely and cover with an appropriate free flap. But in such conditions, selection of recipient vessel is not easy. Many surgeons have used vein graft as the conduit from a distant non-radiated vessel to the free flap. This technique requires a long segment of vein for anastomosing both the artery and vein. We used intra-lesional vessels as the recipient vessel of the free flap, which were suspiciously damaged by irradiation and malignant cell infiltration. The guidelines include: 1) malignancy-infiltrated vessels are to be dissected until a clear field is obtained under a frozen section 2) irradiated vessel can be used only if active spurting is present without intraluminal thrombosis. All of our case obtained satisfactory results in all cases except one of irradiated squamous cell carcinoma at inguinal area whose gracilis myocutaneous free flap loss was in 10%. But even this case, the cause of partial loss of the flap may not be due to a vascular problem but an internal failure of the flap. We concluded, that carefully selected intra-lesional vessels, although they may be damaged by irradiation or malignant cell infiltration can be used as the recipient vessels of free flaps.

Ankylosing spondylitis is a chronic inflammatory disease which most frequently affects the sacro-iliac joints of young men, and also the synovial joints of the whole spine. Bony fusion of these joints and ossification along the longitudinal ligaments lead to total immobility of the vertebrae. Sixty-two cases of ankylosing spondylitis treated at the Severance Hospital from June 1980 to June 1986 were analysed clinically and the followings were obtained. 1. The average age was 38 years and the sex ratio between the male and the female was 58: 4(Male=94%). 2. The most frequent location of pain was low back (32%), and the pain was usually bilateral (81%). 3. The main stiff regions were the hip joint (35%) and the lumbar spine (27%). 4. The most frequent joint affected was the hip (44%), and the extraarticular manifestations were pleuropulmonary disease (29%), gastrointestinal disorder (15%), iritis (10%), etc. 5. The characteristic X-ray findings were sacro-iliac changes (72%), facet joint blurring (53%), and syndesmophyte (37%). 6. Laborstory findings showed increased ESR (85%), positive HLA B-27 (97%), negative rheumatoid factor (94%), and the serum immunoglobulins snd complements were not decreased.

PURPOSE: This study was aimed at identifying the levels of parenting stress among mothers of children with cerebral palsy and factors influencing parenting stress. METHOD: The research design was cross-sectional survey. Data were collected from 122 mothers of hospitalized children (under 15 years of age) with cerebral palsy at the Y medical center using the questionnaires. The data were analyzed using one way analysis of variance with Scheffe test for post-hoc analysis or t-test, Pearson correlation, and stepwise multiple regression. RESULTS: The mean score of parenting stress was 2.52 (SD=0.79; range: 1~4). The father's participation, mother's parenting hours, self-esteem and social support were significant predictors of parenting stress, significantly accounting for the 33.3% variance (F=16.118, p<.001). CONCLUSION: It is essential for health professionals to consider the aforementioned four factors when developing interventions to reduce parenting stress for mothers of children with cerebral palsy.

The finger-tip is a specially-differentiated end structure of the human body in anatomic aspect. Although it is covered with thick dermis and a heavily-cornified epithelial layer, the finger-tip represents tremendous sensory perception because of dense sensory nerve endings. This highly sensitive skin is attached to the phalangeal bone or fascia and stabilizd during grip or pinch by the fascial septa, which form compartments of fat pads in the pulp tissue. In addition to its functional aspect, the finger tip plays an important cosmetic role because it is an exposed area like the face. So, when reconstructing finger-tips, especially the thumb or radial side of the index finger, one must take into account both the functional and esthetic results. In this study, 8 cases of great toe pulp sensory free flaps for fresh amputations (n=4) and previously reconstructed wounds (n=4) are included. In previously-reconstructedcases cases, all of them complained of incomplete recovery of sensation or cosmetic dissatifaction. The fingers covered with flaps were the index finger (n=5), ring finger (n=2), and thumb (n=1). The mean size of flaps was 3 x 1.9 cm and all of the flaps survived without significant complications, except 1 case of venous insufficiency. Sensory complications, except 1 case of venous insufficiency. Sensory perception was detected within 4 months and improved gradually during the follow-up period to 10.5 mm/8 mm in static/moving two-point discrimination test. Even though, some patients complained of postoperative paresthesia including cold intolerance (37.5%), pain (25%) and hyperesthesia (12.5%), the great toe pulp sensory flap is an aesthetically superior and sensuously more satisfactory alternative to other reconstructive procedures for fingertip defects.

T. verrucosum Infection has been reported for the first time in 1986 in Korea and has been increasing progressively. To evaluate the progress of clinical and histopathological change of dermatophytosis caused by T. verrucosum, inoculation study, using T. verrucosum isolated from infected human (human strain) and from infected cattle (cattle strain), was done in 24 male albino Hartley guinea pig. Their clinical and histopathological changes were evaluated. In addition, comparison for the growth rate between human strain and cattle strain on Sabouraud's glucose agar was made. The results were as follows: 1. Growth rate on Sabouraud's glucose agar: Cattle strain showed significantly more rapid growth rate than human strain on Sabouraud's glucose rate at 25℃ and 37℃. And cattle strain showed more rapid growth rate at 37℃ than 25℃. But human strain showed no significant difference of growth rate at both temperature. 2. Clinical findings: Initial erythema, scale and crust were developed about 8th after inoculation. All three findings reached maximum severity about 12th to 16th day and disappeared about 30th to 34th day after inoculation. There was no significant difference in progress of erythema, scale and crust between cattle strain and human strain. 3. Histopathological findings: Although mild acanthosis was noticed on the 3rd day after inoculation, the other findings including parakeratosis, intraepidermal abscess, spongiosis and vascular change, cellular infiltration were found on 9th day after inoculation. They reached maximum severity on the 12th day and lasted to the 25th day after inoculation. After that, all three findings were decreased gradually between 29th day and 33th day. On the PAS staining, hyphae and spores were found on the 6th day and disappeared on the 21th day after inoculation. 4. In trichophyton skin test, all of the 24 guinea pigs became positive within average 9.83±1.17 days These findings suggested that dermatophytosis caused by T.verrucosum induced rapid cell mediated immunity and contributed to rapid resolution of the lesion.

Lymphomatoid papulosis(LyP) is a chronic recurrent dermatosi characterized by involuting and recurring papules, plaques, and nodules showing histologic feaurs suggesting rnalignant lymphoma. On histologic ground, it is divided into two types. type A and type B. In type A, large atypical lymphocytes are the main cellular cornponent and most of these express CD 30(Ki-1) antigen, which has been p!reviously thought to be specific for Read Sternberg cells of Hodgkins disease. In type B, cerebriform mononuclear lymphocytes sirr la to those in mycosis fungoides predominates. We report two cases of lymphomatoid papulosis, type A and type B, respectively. The large atypical cells in type A LyP expressed Ki 1 antigen. Both case were treated with PUVA and the outcome of the treatment has been fair.

The mandibular contour determines the shape of the lower part of face and thus influences the appearance of head and neck. A prominent mandibular angle produce characteristic quadriangular, coarse and muscular appearance. To correct this facial contour to oval shape, the prominent angle can be contoured. the cutaneous approach has been used, but it leaves noticeble scar and has risk of facial nerve injury. Thus the intraoral approach has been used commonly. But because of restricition of operation field, the intraoral approach disadvantages of long operation time, high risk of complications. So, we have performed mandibular contouring procedure by new approach which uses reciprocating saw through stab incision under the mandibular body to correct the 9 patients of mandibular angle protrusion who visited our clinics recently. And we found that mandibular angle contouring by this new approach is easy, and makes the operation and recovery time shorter. There were no complications and cosmetic results were quite satisfactory.
