1.Job Description of the Nurses Who Work in Operating Room Using DACUM Technique.

PURPOSE: Developing a curriculum(DACUM) is a method of analyzing job focused competency, which is obtained from the data of an expert belonging to a certain career. In this study the DACUM method was used to analyze the jobs of operating room nurses. METHOD: Through the DACUM workshop which was arranged by two DACUM facillitators, a definition of the role of operating room nurses was developed and then duties and tasks of operating room nurses' were identified. For the workshop, a DACUM committee with 10 operating room nurses was organized. Finally, the duties and tasks which were identified were validated by 422 nurses for importance, difficulty, and frequency. RESULTS: Thirteen duties and 105 tasks were identified on the DACUM chart, where importance, difficulty, and frequency of tasks were represented by alphabet letters A, B, and C as higher degree of importance. The determinant coefficient(DC) showed that the most important duty was assisting with operations(DC=6.61), and the least, managing operating materials(DC=4.22). For tasks, the most important ones were assisting in orthostatic surgery(DC=7.60), and assisting in thoracic surgery(DC=7.38), and the least important making gauze ball(DC=2.39), and saving of operation site((DC=3.27). CONCLUSION: The results suggest a need to develop an education program using the DACUM chart as a basis for the development and as a clinical career ladder and for curriculum of operating room nursing.

2.A Case of Papillary Adenoma of the Nipple Showing Histopathologic Findings of Syringocystadenoma Papilliferum.

A 24-year-old woman was seen for evaluation of an skin lesion on thiright nipple. Examination revealed a solitary 1.5 x 1cm-sized, erythematous, oozing, eroded andruted, verrucous nodule on the right nipple. A small amount of serous discharge was found but there was no pain or tenderness. Histopathologic findings showed cystic invaginations extending down vard from the epidermis and papillary projections extending into the lumina of the invagination. Pupiae were lined by two rows of cells. In stroma, there were marked inflammatory infiltrates containing any plasma cells. The overlying dermis showed an adenomatous growth pattern with a prolifertion of round, oval or irregular shaped ducts and fibrotic stroma.

3.Two Cases of Pseudoepitheliomatous , Keratotic and Micaceous Balanitis.

Pseudoepitheliomatous, keratotic and micaceous balanitis is a rare distictive clinical entity that represents a histologic spectrum ranging from hypertrophic hyperpalstic penile dystrophy to verrucous carcinoma. This condition is thought to be a malignant growth potential by resistance to treatment and its tendency toward local recurrence. We report two cases with similar clinical presentation of hyperkeratotic plaque and micaceous scaly patches on the glans penis that were compatible with pseudoepitheliomatous, keratotic and micaceous balanitis. Histopathologically, case 1, 48 year-old male was progressed to squamous cell carcinoma and case 2, 78 year-old male, was shown pseudoepitheliomatous hyperplasia without malignant changes.

4.A Case of cytophagic Histiocytic Panniculitis.

We describe a case of typical cytophagic histiocytic panniculitis occuring in 10-year-old male. He had recurrent subcutaneous nodules, fever, pancytopenia and abnormal liver function tests since 5 months of his age, and recently showed hemorrhagic diathesis. The histologic picture showed lobular histiocytic panniculitis with "bean bag" cytophagic cells.

5.A Case of Factitial Panniculitis.

We report a case of factitial panniculitis in a 50-year-old female patient who had multiple variable sized subcutaneous nodules on area accessible to the hands and disappeared by only prohibition of habitual skin massage. Histologic findings showed mostly lobular panniculitis with hemosiderin deposits, many foamy histiocytes and pseudomicrocysts.

6.Dermographism ( IV ): The Prevalence in Atopic Dermatitis and Allergic Rhinitis.

The prevalences of dermographism in 106 patients with atopic dermatitis and 45 patients with allergic rhinitis were studied using a dermographic tester. The results are as follows : 1. Dermographism was more common in atopic dermatitis(17.0%) than in the general population(6.7%), but no significant difference was observed between allergic rhinitis(6.7%) and general population. 2. The prevalence rates of dermographism were not significantly correlated with the level of serum IgE in both atopic dermatitis and allergic rhinitis.

8.A Case of Bart's Syndrome.

Bart's syndrome was initially described as a genodermatosis characterized by congenital localized absence of the skin with blistering and nail deformities 1-3. However, it is considered as any type of epidermolysis bullosa(EB) with localized congenital absence of the skin on the extremities. A 33-day-old fbmale baby was presented with congenital absence of the skin over the left shin and dorsa of both feet which were covered with the thin, translucent, and brown-red glistening membranes. Blistering of the right calf and left great toe nail deformity were also noted. She was diagnosed as a recessive dystrophic EB by the histopathological, ultra- structural and immunomapping studies.

9.Observations of Arrhythmias by 24 Hour Ambulatory ECG Monitoring in Early Convalescent Phase of Acute Myocardial Infarction.

In order to observe the arrhythmia which occured in early convalescent phase of acute myocardial infarction, a 24 hour ambulartory ECG monitiring was performed in 15 patients who is admitted at dept of Int Medicine, Ewha Womans University hospital from Aug. 1985 to Sept. 1986. during 7.5+/-3.4 days after admission. In all cases one or more of the following arrhythmias were observed;atrial premature beat, ventricular premature beat(VPB), transient paroxysmal ventricular thchycardia, atrial fibrillation, sinus arrest and junctional beat. Of 15 cases, 11(77.3%) had VPB's:Simple VPB's in 4 cases(26.7%) and complex VPB's(6.52+/-2.52)(P<0.05). But there were no difference of premature and abberancy index between complex VPB's and simple VPB's(P>0.1). In addition, we compaired the arrhythmias in CCU with those of early convalescent phase of acute myocardial infarction. But there was no correlation between them.

10.The Relationship between Perceived Family Functioning and Depression in Patients with Chronic Low-Back Pain.

This study was conducted using a descriptive correlational survey design to identify how patients with chronic low-back pain perceived family function and to examine the relationship between family function and depression. The participants of the study were 116 patients with chronic low-back pain patient who were having follow-up treatment in a local pain clinic, located in Daegu. A structured questionnaire was filled out participants themselves and the structured interview was given during January and February, 2000. The data collection instrument were FACE II-Korean version and Beck's Depression Inventory(1983). The data collected were analysed using mean, standard deviation(SD), t-test, F-test, Duncan test and Pearson correlation coefficient, done with SAS. The results are as follows; 1. The mean of the respondents' perceived family function was 3.47, with emotional bonding at 3.61, acceptability and sharing at 3.61, participation at 3.23, negotiation and satisfaction at 3.24 and responsibility and expectation at 3.76. 2. The mean of the respondents' perceived depression was 17.53+/-8.76. 3. There was significantly negative correlation between family function and depression(r=-0.4817, p=0.0001). 4. Research variables included demographic characteristics and showed that subjects who were aged 65 years or older showed significantly lower scores in patient's perceived family function than those who were aged 45 years or below(p=0.0502). Perceived family function of females respondents were significantly lower than males(p=0.0026). The illiterate showed significantly lower scores than other groups(p=.0001). Respondents with spouse and extended family tended to show significantly higher scores than those who were living with spouse only or by themselves alone(p=0.0091). Those who had higher income showed significantly higher scores than in the lower income group(p=0.0001). 5. For family function regarding pain characteristics, those who reported severe pain-related daily activity impediments showed significantly lower scores in family function than others with less pain(p=0.0357).
