1.Current Status and the Future Prospect of Rehabilitation Nursing in Korea.

The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981. Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows 1. According to the 2000 census of disabilities. the number of persons with disabilities in Korea is estimated at 1.449.500. or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1.449, 496 out of the total population and 1.024,371 persons are registered for disability. making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41. 7% (605.127) and mentally retarded persons stood at the smallest 9% (13.481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing /speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989. 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1.024.371. These figures are different from the actual number of disabled persons. According to the 1995 census. 1.053.486 were disabled persons but only 378,323 registered for disability. And. in the 2000 census. 1.024,371 out of the 1.449.496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didn't know the registration procedure. The rest of them replied that they didn't think they were disabled or that registration didn't seem to give any benefits 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers. Permission to use LPG fuel. Reduction of public facility fees. Household allowance. Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type. The welfare institutions for disabilities numbered 188i in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10). hearing/speaking disability(l4). mental retardation(59), and sanatoriums(68). 6. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursmg IS divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication, prevention and care, prevention of injury from a fall. etc.

2.The Comparative Study on the Quality of Life, Health Perception, Self-efficacy of Ostomates by the Fecal Control Type.

The study was conducted to compare the quality life (QOL), health perception and self efficacy of the ostomates by the fecal control type. The study sbjects included 128 ostomates in Pusan, Daegu, Daejon and Incheon branches of the Korean Ostomy Association. Data collection was performed for 27 day periods from September 10th to October 6th. 1988. The instruments used in this study were the OL scale developed by Bang (1991). the health perception scale developed by Ware and translated to Korean by Lee (1987), the self efficacy scale developed by Kim( 1995). The analysis of the data was done by the use of descriptive statistics. Chi-square test, ANCOVA and Pearson Correlation with the SAS program. The results of the study are summarized below. 1. Only age was a significant difference between the two groups(p=.014). 2. The ostomate-related characteristics which showed significant differences were the number of participations in the ostomate meetings each month(p=.001). the care manager(p=.017), the number of defecations(p=.001). problems with the colostomy(p=.003), the duration of the ostomates(p=.00l). These variables were controlled using the ANCOVA. 3. The first hypothesis. "There will be differences at the level of the QOL according to the fecal control type" was supported(F=7.87, p .0059). 4. The second hypothesis. "There will be differences at the level of health perception depending on the fecal control type" was supported (F =8.68, p = .0039). 5. The third hypothesis, "There will be differences at the level of self efficacy depending on the fecal control type" was supported(F=4.58. p=.0344). 6. There were significant positive correlation between the QOL and health perception(r= .8225. p=.0001), and self efficacy(r=.6356, p=000l). From the above results, it can be concluded that colostomy irrigation is a good method to improve the QOL, health perception and self-efficacy. Therefore, colostomy irrigation is an important method for rehabilitation nursing use in the care of ostomates.

4.Morphometric Analysis of Preeciamptic Nephropathy with Focal Segmental Glomerulosclerosis.

To evaluate the structural characteristics that might be related to the clinical features noted in preeclamptic patients with focal segmental glomerulosclerosis(FSGS), we analyzed post-partum renal biopsies of 8 preeclamptic patients with FSGS (group 1) by morphometry and glomerular studied the structural-functional relationships. These findings were also compared with those from three postpartum cases with minimal change lesion(group 2) and normal age-matched women(group 3). Mean glomerular volume (MGV) in group 1 and group 2 was (2.64 +/- 0.49) x 10(6) micrometer3 and (2.56+/-0.25)x 10(6) micrometer3, respectively. MGV in both groups was significantly increased compared with that of the control group [(1.11+/-0.22)x10(6) micrometer3](p<0.0005). The volume density of the mesangium/glomerulus [Vv(mes/glom)] in the group 1 patients was significantly increased (p<0.0001) when compared with that of the group 2 and the control group patients. The increment of Vv(mes/glom) was related to both the mesangial cell proliferation and expansion of mesangial matrix. The volume density of the capillary lumen/glomerulus [Vv(cap/glom)] in group I was significantly decreased(p<0.0001) when compared with that of group 2 and the control group. Vv(cap/glom) was directly related to Ccr in group l(r=0.70, p=0.05). These results suggest that reduced capillary luminal area caused by mesangial interposition is related to the decreased glomerular filtration rate in preeclamptic FSGS.

6.Sun Xi-miao in The Biography of The Avatamsaka-Sutra.

This paper aimed to introduce and examine the biography of Sun Xi-miao (581-682) which I found in The Biography of The Avatamsaka-Sutra, that Fazang (643-712) wrote in 692 A.D. This document was neglected to understand Sun who was the famous medical writer of the collection of prescriptions, the Bei ji qian jin yao fang. His life is rather well documented, because he has his own biographies in Jiu Tang shu and Xin Tang shu which cited from Da Tang sin yu, published in 807. But I found several new informations about Sun in The Biography of The Avatamsaka-Sutra, such as he use to be a military medicine in the troops of Li Yuan who became the first emperor Kao Tsu of Tang dynasty and treat Sun with great favour. This document let us know that the Bei ji qian jin yao fang was dedicated to Kao Tsu, known as published in 652 A.D. MY CONCLUSIONS ARE AS FOLLOWS: First, it was written by Fazang in 692 A.D, who was the real establisher of the fraternity of the Avatamsaka in China, for the purpose of encouraging to copy the Avatamsaka-Sutra. According to this biography, Sun made 750 copies to persuade the monks and the peoples, and that's the reason Fazang wrote his biography. Secondly, it was not conveyed to posterity, such as Sun was good looking, tall and use to be the medicine of Kao Tsu and dedicated his medical book to the first emperor. It might be left out for Tai Tsung's sake in the official records, who murdered his brother, the heir apparent to the throne and became the second emperor by himself. On the contrary, it was written in Da Tang sin yu, Jiu and Xin Tang shu that Sun made a prediction that his collection of prescriptions would help the holyman after 50 years from Xuan Di (578-579) of Northern Chou Dynasty. Holyman meant Tai Tsung. It shows that Sun's biographies in the Da Tang sin yu, Jiu and Xin Tang Shu were based on the documents that might be fabricated and embellished for Tai Tsung. Thirdly, this biography let us know that Sun wote the Bei ji qian jin yao fang under the circumstances that the epidemic disease was spreaded in Changan at 618 A.D. and the population of Kao Tsu era (618-626) was decreased rapidly. I think that's why he wrote down the medical morals as the first chapter, and the prescriptions about the gynecology and pediatrics as the second chapter.

7.Effects of a Proper Positioning on Prevention of Musculoskeletal Complication on Patients with Stroke..

This study was done to identify the effectiveness of a proper positioning on musculoskeletal complication on patient with Stroke by using a quasi-experimental study. A total of 18 patients were selected as a subject from June 1st to October 31th 1998 who had been hospitalized at Intensive Care Units in K medical center. A experimental group consists of 8 patients who were given proper positioning every two hours. A control group consists of 10 patients who were given traditional positioning. The collected data were analyzed by SPSS windows including chi-test(Fisher's exact method), Wilcoxon Rank Sum test and McNemar test. The result of this study was summerized as follows: 1) The experimental group with proper positioning has shown lower shoulder pain score, dorsiflexion contracture score, Shoulder subluxation score and higher Range of Motion and than the control group. 2) The experimental group with proper positioning has shown lower muscle pain score and edema score than the control group, but it was not statistically significant. Therefore, proper positioning could be applied as an independent nursing intervention for patients on Stroke in order to facilitate rehabilitation.

9.The Change of Arterial Blood Gas during High Frequency Jet Ventilation via 14 French Suction Catheter in Microlaryngeal Endoscopic Surgery.

BACKGROUND: It is a routine practice that High Frequency Jet Ventilation (HFJV) is applied through a 14~16 gauge (about 12 French (Fr.)) angiocath. The 14 Fr. suction catheter which is similar to angiocath in its internal diameter is commonly available in the operating room. We evaluated the suction catheter as a carrier of HFJV in point of ventilation, oxygenation and operating field during HFJV at microlaryngeal endoscopic surgery (MES). METHODS: Fifteen adult patients undergoing HFJV via 14 Fr. suction catheter during MES were studied. Time-based arterial blood gas analyses were done before and after HFJV. The movement of operating field was examined using laryngoscopic examination by surgeon and anesthesiologist. We also evaluated complications such as abdominal distension, barotrauma and so on. RESULTS: The mean arterial oxygen tension was maintained above 250 mmHg all the time during HFJV. The mean carbon dioxide tension was less than 51 mmHg. There were no remarkable catheter movement and complications. CONCLUSION: The 14 Fr. suction catheter is a good replacement of angiocath. It provided good operating field, ventilation and oxygenation without complications.
