1.A Case of Ovarian Mucinous Cystadenocarcinoma with Mural Nodule of Anaplastic Carcinoma.

Mural nodules in ovarian mucinous tumors, whether benign, borderline, or malignant, have been described by several authors since Prat and Scully first described sarcoma or sarcoma-like mural nodules occurring in association with mucinous epithelial neoplasia in 1979. Three distinct types of the mural nodule have been identified; i. e., 1) sarcoma-like lesion, 2) true mesenchymal sarcome, and 3) anaplastic carcinoma simulating a sarcoma. Mural nodules of sarcomatous and carcinomatous foci associated with mucinous ovarian tumors should be separated from sarcoma-like nodules because of the poor prognosis of the former compared to the favorable prognosis of the latter. We experienced a case of ovarian mucinous cystadenocarcinoma with sarcoma-appearing mural nodule of anaplastic carcinoma occurred in a 59-year-old woman and reported with brief review of the literatures.

2.A Study on the Degree of Physical Psychological and Social Adaptation of CVA Patients.

This study was made on 274 apoplectics patients who received the rehabilitation therapy and tests on physical, psychological and social adaptations as outpatients in 23 general hospitals in the Seoul and Kyungi area. The basic data on degree of improvement of apoplectic patients studied from rehabilitation therapy. Data was collected over a period of 63 days, from February 21st till April, 23, 1996. The assigned physical therapist conducted direct interviews with patients after he answered the distributed questionnaires for each individual patient. The collected data was processed by the SPCC/C+ method. The results of the tests conducted to meascne the degree of ADL dependency, depression and social activity corresponding to the physical, psychological, and social adaptation. The details are; 1) The test to meascne the degree of ADL dependency, corresponding to the study of physical adaptation of CVA patients, indicated a mean score of 2.57 (ideal score is 1.0) with a standard deviation of +/-0.75. The worst score was 3.95 while the best score was a perfect 1.0, representing a severe range of dependency. The distribution was centered with a median of 2.65 and a mode of 2.68. 2) The test to meascne the degree of depression which corresponds to the level of psychological adaptation yielded a mean of 2.99 which is higher than the normal limit of 2.45. The standard deviation was +/-0.52 and the worst score and the best score were 4.35 and Respectively. The distribution was centered with a median of 3.00 and a mode of 3.00. 3) The test to meascne the degree of social activities for the level of social adaptation indicated a very low mean score of 26.52 (perfect score is 144), with the standard deviation of +/-16.23. Some patients scored as high as 100, but others scored as low as 3. The distribution of social activities at a very low level was shifted to the left with a median of 24.00 and a mode of 20.00. 4) Factors influencing the level of physical, psychological and social adaptation are as follows: Factors significantly influencing the level of physical adaption measured by ADL dependency are age, personal guardian, payer of medical expenses, and paralysis of the right arm, right leg and facial paralysis. Factors significantly influencing the level of psychological adaptation measured by the degree of depression are age, marital status, education, medical history of individual and family, speech impediment, and facial paralysis. Factors significantly influencing the level of social adaptation measured by the degree of social activity are age, marital status, education, employment status, and the burden of medical expense. 5) The Corelationship is significant (9.00), between ADL depending as degree of physical adaptation and depression as degree of psychological adaptation. ADL dependency is proportional to depression. But social activity is inversely portional to ADL dependency and depression. In conclusion, the increased care for physical function of the patients is not the only necessary means to better facilitate the appropriate adaptation of CVA patients. The introduction of a solid rehabilitation program for psychological and social adaptation will also play the integral part of the treatment of CVA patients.

3.The Effect of Shift Directions of Clinical Nurses on the circadian Rhythm.

The circadian system represents a temporal order which is mediated by the mutual coupling of oscillators and by the synchronizing effects of zeitgebers. It is known that well-being of man depends partly on the maintenance of this order, and that repeated or long lasting disturbances to it such as shift work will cause harmful effects. This study was a quasi-experimental study to test the effect of shift directions for the clinical nurses on the circadian rhythm. Fourteen nurses working at the general units of Y hospital were selected according to the established criteria. Fourteen subjects were assigned to a weekly shift but the directions of shift work were phase delay first and then phase advance or vice versa. Oral temperature, total sleeping time, frequency of sleep-wake cycle, fatigue, mental performance, and physical symptom were measured during these days except holidays. The data collection period was from April 26, 1993 to July 3, 1993. MANOVA and Wilcoxon signed rank test were used for statistical analysis. The results are summarized as follows. 1. Having worked on evening and night shifts in either phase delay or phase advance schedules, temperature rhythms of shift workers were gradually adapted to the new sleep-wake cycles. A complete adaptation to work on the night shift was achieved the sixth day of the night shift in the phase delay schedule compared to the partial adaptation to the work on the night shift in the phase advance schedule. Accordingly, by putting evening shift between day and night shifts, it will be possible for circadian rhythm to adapt easily to the night shift. 2. There were differences in the total sleeping time, frequency of sleep-wake cycle, fatigue, and physical symptom except for mental performance between night shift and day, evening shift. This indicates further that shift workers working on the night shift have a hard time adapting to the shift work compared to the other shifts. 3. Evaluating all the acrophases of temperature rhythm either in phase delay or phase advance schedules, it was shown that night to evening shift in the phase advance schedule revealed the smallest phase move. Also phase advance schedule showed poorer adaptation to shift work than phase delay schedule in connection with total sleeping time, frequency of sleep-wake cycle, fatigue, mental performance, and physical symptom. It is suggested, taken together, these findings reflect that phase delay schedule facilitated the degree of adjustment to the shift work compared to the phase advance schedule.

4.Parenting Stress in Mothers of Premature Infants.

PURPOSE: The purposes of this study were to assess the parenting stress in mothers of premature infants and stress related characteristics of mothers and infants. METHODS: The methodology was a cross sectional survey study using self-report questionnaires. Participants in this study were 36 mothers of infants with corrected ages of 4 months to 12 months who were born prematurely. Data were analyzed using t-test, ANOVA, and Pearson correlation with the SPSS WIN 19.0 program. RESULTS: The mean score for parenting stress in mothers of premature infants was 74.639+/-17.570, indicating that the mothers actually experienced stress. When mothers were able to have some private time, parenting stress was statistically significantly lower. When the residential status was having one's own house and the local community provided informational support regarding child nurturing, there was a statistically significant decrease in the sub-category of particular infant temperament. There was a statistically significant positive correlation between mothers' depression and parenting stress. CONCLUSION: These results indicate that there is a need for nursing interventions to increase mothers' private time and to include informational support regarding health management of children including developmental status assessment from local communities.

5.Serum Insulin-like Growth Factors and their Binding Proteins in the Women With Polycystic Ovary.

OBJECTIVE: The involvement of IGF system in hyperandrogenism and abnormal follicular development is controversial. This study is to assess whether IGF system contribute to it in the women with polycystic ovary(PCO). METHODS: Baseline serum levels of luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2), testosterone (T), androstenedione (ADD), prolactin, thyroid stimulating hormone (TSH), free insulin-like growth factor(IGF)-I, free IGF-II, insulin-like growth factor binding protein(IGFBP)-1, and IGFBP-3 were measured in twelve healthy regularly cycling volunteers and forty-two women with PCO then, the changes of baseline serum levels were evaluated after laparoscopic ovarian electrocauterization in nine PCO patients. In addition, the expression pattern of IGF-I and IGF-II was examined in the ovary of control and PCO group. RESULTS: Baseline levels of LH, ADD, free IGF-II, and IGFBP-3 were significantly higher in PCO group. However, there were no significant differences in the levels of free IGF-I and IGFBP-1, although free IGF-I showed decreasing tendency in PCO group. And there was a significant positive correlation between the LH and free IGF-II level in the PCO(P=0.011, r2=0.3899), but not in the control. After ovarian electrocauterization, LH, T, and ADD levels decreased, and free IGF-I and IGFBP-3 level increased. While free IGF-II and IGFBP-1 level showed no significant changes. In the ovary, expression of both IGFs showed similar pattern in normal and PCO ovaries. CONCLUSIONS: The elevated IGFBP-3 level may alter the bioavailability of IGF(s) in the PCO. The change in IGF-I level and resumption of ovulation after electrocauterization, suggest a possible role of IGF system in the impairment of follicular development in the PCO.

6.Effects of PCA (Patient Controlled Analgesics) Education Program including Practicum on Post-op Pain of Gynecologic Laparoscopic Surgery Patients.

PURPOSE: The purpose of this study was to identify the effects of the patient controlled analgesics (PCA) education program (including practicum) on post-op pain suffered by patients who have undergone gynecologic laparoscopic surgery. METHODS: The research was designed for a nonequivalent control group before and after the test design. The subjects of this study were 54 in all and were divided into an experimental group and a control group. The program consisted of a brochure for PCA use and a practicum with an actual PCA instrument. Data were collected with questionnaires and observations and were analyzed on the basis of frequency, percentage, mean, standard deviation, chi2 and t-test. RESULTS: Complaints regarding pain by the experimental group were significantly less than those of the control group in both post-op 24-hour and post-op 48-hour reporting. The experimental group pressed the PCA button much less frequently than the control group in post-op 24 hours. However, there were no significant differences in side effects between the two groups. CONCLUSION: Based on the results of this study, this PCA education program can be useful in the clinical nursing field and helpful for patients who use PCA.

7.The Influences of Quality of Sleep and Mood State on Fatigue in Primary Brain Tumor Patients.

PURPOSE: The purpose of this study was to identify the influences of sleep quality and mood state on fatigue in primary brain tumor patients. METHODS: The participants were 118 patients with primary brain tumors between May 2015 and March 2016. Data were collected using questionnaires including individual and disease related characteristics, the Pittsburgh Sleep Quality Index, Linear Analogue Self-Assessment scales, and The Fatigue Scale for Cancer Patients developed by Kim Kyeong-hee (2006). Data were analyzed using descriptive statistics, Pearson correlation, and hierarchical multiple regression. RESULTS: Fatigue had significant correlations with sleep quality (r=.55, p<.001) and mood state (r=.74, p<.001). The influencing factors for fatigue were mood state (β=.46, p<.001) was the best predictor of fatigue, followed by age (β=.17, p=.049), and performance status (β=-.19, p=.011). CONCLUSION: The research findings show that understanding of psychological factors such as anxiety, depression, confusion, energy and anger should be given priority for fatigue management in primary brain tumor patients. This study provides base data for managing fatigue in patients with primary brain tumors, and is expected to contribute to the improvement of the health of patients with primary brain tumor.

9.Consideration of Birth Weight by Gestational Age.

This study was undertaken to make current fetal growth curve throughout later part of pregnancy. 5,110 normal singleton deliveries at Il Shin Christian Hospital were included. The 10th, 25th, 50th, 75th, 90th percentiles of birth weight were calculated from 21 to 42 weeks' gestation and the 10th, 50th, 90th percentiles of birth weight compared by fetal sex, maternal parity and height were graphed to know the potency of factors. There was a linear growth pattern between 28 and 38 weeks' gestation. During last month of prgnancy, three factors have significant effect on median birth weight but no significant differences in 10th percentiles. Maternal height & parity significantly affeect on 90th percentiles. So, these factors must be considered when diagnosing large for date.

10.Mediating effect of communication ability in the relation between empathy and interpersonal relation in nursing students

Purpose:
This study aimed to grasp the correlation between communication ability, empathy, and interpersonal relations among nursing students, and to explore the mediating effect of empathy on the relation between communication and interpersonal relation in nursing students.
Methods:
Participants were 209 college students in the Department of Nursing at two universities. The data collection period was from October 1, 2019 to November 1, 2019. The questionnaire consisted of measuring tools for general characteristics, communication ability, interpersonal relation and empathy. SPSS/23.0 program was used. The analysis used descriptive statistics, Pearson's correlation coefficient, independent t-test, one-way ANOVA, multiple regression analysis, and Sobel's tests.
Results:
The communication ability was nursing major satisfaction (t=8.25, p<.001), and last year's average grade (t=7.29, p=.001). Interpersonal relationships showed significant differences in gender (t=2.51, p=.013) and nursing major satisfaction (t=10.99, p<.001). Communication ability and empathy (r=.37, p<.001), communication ability and interpersonal relation (r=.77, p<.001), empathy and interpersonal relation (r=.43, p<.001) showed a significant static correlation. In communication ability and interpersonal relation, empathy had a partial mediating effect (Z=2.97, p<.001), and explanatory power was 62%.
Conclusion
To improve the communication ability and interpersonal relation of nursing students, it is necessary to develop a personalized and empathy-enhanced education program for each grade.
