3.The Effect of the Team-based Cardiac Rehabilitation Program on the Health Behavior Performance, Anxiety, and Quality of Life in the Group of Ischemic Heart Disease with PTCA.

This study was designed to determine the outcomes of the team-based cardiac rehabilitation programs for patients with Ischemic Heart Disease(IHD) on their level of health behavior performance, anxiety, and quality of life. A total of 19 in-patients with IHD who have had PTCA treatment at G.Hospital in Inchon were selected as subject to data collection from} May 1998 to 30 Oct. 1998. The experimental group, consisting of 11 patients participated in the Hospital's cardiac rehabilitation program was compared with the non-rehabilitation control group of 8 patients. The experimental group has been trained on the cardiac rehabliltation programfor 120-150minute during hospitalization by a team consisted of four specialists, those are cardiologist, head nurse, dietian, and the author. Also individual follow-up training and interview were performed for the members of experimental group at their every visit to the Hospital after discharge. For all subjects, scores on health behavior performance, anxiety, and quality of life were collected two days and 12 weeks after PTCA treatment by questionnaire. The collected data were analyzed with the SPSS. The results of this study were summarized as follows: 1) The experimental group shows significant increase in health behavior performance score compared to the control group(-2.27, p=.022). 2) There was no significant difference between two group' s anxiety scores (- .24, p= .803) and quality of life scores(-1.86, p=.061). 3) The correlation between health behavior performance and anxiety ( .07, p=.771), and that between health behavior performance and quality of life( .12, p=.621) respectively were not significant statistically. But the correlation between anxiety and quality of life were significant reversely ( .49, p=.032).

4.Six Cases of Iron Containing Plasma Cells.

Iron in plasma cells has been described in patients with diseases characterized by iron overload. We observed iron-containing plasma cells in the bone marrow aspirates of 6 patients with anemia. In five of these 6 patients, there were alcoholic liver disease and in one there was abdominal aortic occlusion. Medical records of patients and previous reports were reviewed. Marrow storage irons were adequate or increased, but other morphologic changes of alcoholism such as erythroid vacuolization or ringed sideroblasts were not the features. The presence of iron-containing plasma cells is suggestive of alcoholism and its complications, and diseases associated with iron overload or inability of RBCs to utilize iron. The exact mechanism of entry of iron into plasma cells is controversial.

5.A Study on the Empathy of Pediatric nurses.

Empathy is an essential factor for establishing therapeutic relationship. Especially for pediatric nursing, the nurses need to know how to perceive children's feeling and thought and to express her understanding about them affectively. So, empathy is considered as an important concept in pediatric nursing. This study was attempted to measure the empathic ability of pediatric nurses by pediatric nurse and mothers of hospitalized children. The subject of this study consisted of 83 pediatric nurses and 106 mothers of hospitalized children in 4 hospitals in Seoul and 1 hospital in Kyung-Gi. Data were collected from Jan. 14 to Feb. 6, 1997, by the questionnaire method. The instrument for this study was the Empathic Construct Rating Scale : ECRS(60-item, 5 point scale). Data Were analyzed by descriptive statistics, t-test and ANOVA. Result of this study are summarized as follows : 1. Pediatric nurses perceived their empathic ability to be moderately high(Mn=3.71) The level of perceived empathic ability of the pediatric nurses showed no significance by general characteristics. 2. The mothers of hospitalized children perceived nurses' empathic ability to be moderate(Mn=3.27). The level of perceived nurses' empathic ability of the mothers of hospitalized children showed significant differences by the experience of hospitalization of the children and the age of the mothers. 3. The perceived nurses' empathic ability of the mothers was significantly low than of the pediatric nurses(t=6.50, P<.001). From the above results, it can be concluded. that the empathic ability of the pediatric nurses is not sufficient for caring children and mothers efficiently. So, the pediatric nurses' empathic ability is needed to improve through facilitative relationship training as refreshment in-service education program.

6.Current Status and the Future Prospect of Rehabilitation Nursing in Korea.

The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981. Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows 1. According to the 2000 census of disabilities. the number of persons with disabilities in Korea is estimated at 1.449.500. or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1.449, 496 out of the total population and 1.024,371 persons are registered for disability. making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41. 7% (605.127) and mentally retarded persons stood at the smallest 9% (13.481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing /speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989. 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1.024.371. These figures are different from the actual number of disabled persons. According to the 1995 census. 1.053.486 were disabled persons but only 378,323 registered for disability. And. in the 2000 census. 1.024,371 out of the 1.449.496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didn't know the registration procedure. The rest of them replied that they didn't think they were disabled or that registration didn't seem to give any benefits 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers. Permission to use LPG fuel. Reduction of public facility fees. Household allowance. Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type. The welfare institutions for disabilities numbered 188i in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10). hearing/speaking disability(l4). mental retardation(59), and sanatoriums(68). 6. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursmg IS divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication, prevention and care, prevention of injury from a fall. etc.

8.The Comparative Study on the Quality of Life, Health Perception, Self-efficacy of Ostomates by the Fecal Control Type.

The study was conducted to compare the quality life (QOL), health perception and self efficacy of the ostomates by the fecal control type. The study sbjects included 128 ostomates in Pusan, Daegu, Daejon and Incheon branches of the Korean Ostomy Association. Data collection was performed for 27 day periods from September 10th to October 6th. 1988. The instruments used in this study were the OL scale developed by Bang (1991). the health perception scale developed by Ware and translated to Korean by Lee (1987), the self efficacy scale developed by Kim( 1995). The analysis of the data was done by the use of descriptive statistics. Chi-square test, ANCOVA and Pearson Correlation with the SAS program. The results of the study are summarized below. 1. Only age was a significant difference between the two groups(p=.014). 2. The ostomate-related characteristics which showed significant differences were the number of participations in the ostomate meetings each month(p=.001). the care manager(p=.017), the number of defecations(p=.001). problems with the colostomy(p=.003), the duration of the ostomates(p=.00l). These variables were controlled using the ANCOVA. 3. The first hypothesis. "There will be differences at the level of the QOL according to the fecal control type" was supported(F=7.87, p .0059). 4. The second hypothesis. "There will be differences at the level of health perception depending on the fecal control type" was supported (F =8.68, p = .0039). 5. The third hypothesis, "There will be differences at the level of self efficacy depending on the fecal control type" was supported(F=4.58. p=.0344). 6. There were significant positive correlation between the QOL and health perception(r= .8225. p=.0001), and self efficacy(r=.6356, p=000l). From the above results, it can be concluded that colostomy irrigation is a good method to improve the QOL, health perception and self-efficacy. Therefore, colostomy irrigation is an important method for rehabilitation nursing use in the care of ostomates.
