4.Treatment of Erythroplasia of Queyrat with Topical 5 % 5 - Fluorouracil Cream under Occlusion.

We herein reported a typical case of erythroplasia of Queyrat in a 35-year-old male patient who had a 6 month-duration of a erythroplastic lesion on the glans penis and coronal sulcus. He was treated 5% 5-fluorouracil cream under occlusion twice a day for 10 days. Three months after treatment, no erythroplastic lesion was found and rebiopsy showed marked improvement histopathologically.

6.A Study on Pulmonary Extraction Rates of Biogenic Amines and Permeability of Serum Albumin in Dogs with Experimental Acute Respiratory Failure.

The extraction rates of biogenic amines such as 5-hydroxytryptamine(5-HT) and norepinephrine(NE) in pulmonary endothelium, and the permeability of human serum albumin(HSA) to pulmonary epithelium were measured in experimental adult respiratory distress syndrome(ARDS), in order to evaluate their usefulness for the early recognition of ARDS. Sixteen mongrel dogs(body weight, 13+/-0.9 kg) were divided into 3 groups; Oleic acid 0.06 ml/kg was injected to 6 dogs(M group) and oleic acid 0.12 ml/kg to 6 dogs(S group). Four dogs were used as a control group. The extraction rates(%) of 14C-5-HT and 3H-NE, the blood concentration of (99m)Tc-HSA, hemodynamic changes and changes of arterial gas profile were measured and compared among three groups. In control group with 100% oxygen ventilation, the extraction rates of 14C-5-HT, 3H-NE and the blood concentration of 99mTc-HSA were not changed. In M group, the extraction rate(%) of 14C-5-HT was decreased from 61.5+/-3.1 to 43.8+/-5.2 in 10 minutes, and such decreased extraction rate(%) was maintained for 2 hours. The extraction rate(%) of (3)H-NE was decreased from 32.3+/-3.6 to 19.5+/-2.5 in 10 minutes. In S group, the extraction rate(%) of 14C-5-HT was decreased from 60.9+/-5.1 to 23.2+/-5.7 in the first 10 minutes, and the extraction rate(%) of H-NE was decreased from 30.1+/-4.3 to l3.1+/-1.9 in 10 min, and decreased slowly thereafter. The blood concentration of 99mTc-HSA was decreased significantly in M group and more in S group compared with control group during two-hour observation. These results suggest that measuring extraction rates of biogenic amines couid be applied for early recognition of ARDS, however, the change of blood 99mTc-HSA concentration could not be applied for early recognition of ARDS, because it has poor correlation to the progression of respiratory failure.

9.Radiofrequency Tissue Ablation with Cooled-Tip Electrodes:An Experimental Study in a Bovine Liver Model on Variables Influencing Lesion Size.

PURPOSE: The purpose of this study was to determine the influence of various factors on the extent of thermal coagulation necrosis after radiofrequency (RF) tissue ablation using a cooled-tip electrode in bovine liver. MATERIALS AND METHODS: RF ablation was induced by a monopolar 500 KHz-RF generator (CC-1; Radionics,Burlington, Mass., U.S.A.) and an 18-G cooled-tip with single or clustered electrodes. The ablation protocol involveda combination of varying current, ablation time, power output, gradual or abrupt increase of this out-put, and pulsed radiofrequency techniques. The maximum diameter of all thermal lesions which showed a color change was measured perpendicular to the electrode axis by two observers who reached their decisions by consensus. Twenty representative lesions were pathologically examined. RESULTS: With increasing current lesion diameter also increased, but above 1500 mA no further increase was induced. Extending the ablation time to 9 minutes for a single electrode and 15 minutes for a clustered electrode increased lesion diameter until a steady state was reached. Higher power levels caused larger lesions, but above 100 W no increase was observed. Ample exposure time coupled with a stepwise increase in power level induced a lesion larger than that resulting from an abrupt increase. Continuous pulsed RF with a high current led to increased coagulation necrosis diameter. CONCLUSION: These experimental findings may be useful thermotherapy. The data suggest that all involved factors significantly affect lesion size: if the factors are better understood, cancer thermotherapy can be better controlled.

We have experienced a patient (4-year-old female) with a granular cell tumor (GCT), Examination of patients skin showed a bean sized asymtomatic firm nodule on the left baek. Exciaional biopsy and histopathologic examination were perforrned under the impression of angiolipoma. The specirnen revealed a distinctive picture of GCT.
