4.The Impact of Marriage Migrant Women's Korean Language Ability, Self-esteem, and Acculturative Stress on Their Family Health: Focused on Mediating Effects of Marital Adjustment.

PURPOSE: This study was conducted for the purpose of a structural model analysis of family health of women who came to Korea for being married to Korean men. METHODS: The data were collected from 260 immigrant women at multicultural centers located in C and B cities from May 10th to 30th, 2012. The variance analysis on the samples was conducted by using the maximum likelihood minimization function with AMOS 7.0. The fitness was evaluated by means of the SRMR, RMSEA, CFI, and TLI with a 90% confidence interval. RESULTS: First, immigrant women's self-esteem and acculturative stress were found to have significant direct effects on their family health. Second, their self-esteem and acculturative stress have direct effects on their marital adjustment. Third, their marital adjustment was found to have significant direct effects on their family health. Forth, immigrant women's Korean language ability was found not to have significant direct effects on their marital adjustment and family health. CONCLUSION: In order to enhance the family health of immigrant women, it is necessary to develop and apply nursing programs in consideration of immigrant women's self-esteem, marital adjustment and acculturative stress.

5.A Case of Systemic Air Embolism in a Child after Thoracic Blunt Trauma.

Systemic air embolism occurs when air enters the pulmonary venous system as a result of a positive gradient caused by low pulmonary venous pressure or increased airway pressure, or both. Systemic air embolism has been frequently reported after penetrating thoracic trauma and can induce life-threatening complications. In blunt thoracic trauma, systemic air embolism has been rarely diagnosed. Because air embolism associated with blunt trauma is more insidious, diagnosis is usually not appreciated unless special circumstances allow discovery of air in major vessels or coronary arteries, such as during emergency resuscitative thoracotomy. We report a case of systemic air embolism in an 18-month-old girl after accidental thoracic blunt trauma and review related literature.

6.Takotsubo Cardiomyopathy Following Cerebral Infarction Involving the Insular Cortex.

BACKGROUND: Takotsubo cardiomyopathy is characterized by clinical features similar to those of acute myocardial ischemia, but without angiographic evidence of obstructive coronary artery disease. We present a patient with takotsubo cardiomyopathy following acute infarction involving the left insular cortex. CASE REPORT: A 52-year-old man was admitted with acute infarction of the left middle cerebral artery territory and acute chest pain. Acute myocardial infarction was suspected because of elevated serum troponin levels and hypokinesia of the left ventricle on echocardiography. However, a subsequent coronary angiography revealed no stenosis within the coronary arteries or ballooning of the apical left ventricle. CONCLUSIONS: We postulated that catecholamine imbalance due to the insular lesion could be responsible for these interesting features.

7.The Comparison of Functional Status and the Level of Health Care Needs in Elderly Koreans in Health Care Institutions.

PURPOSE: The purpose of this study was to compare functional status and the level of health care needs in elderly Koreans in health care institutions. METHODS: Data were collected from 2,521 elderly patients admitted in 50 health care institutions (hospitals, oriental hospitals, geriatric hospitals, and health care facilities) selected through proportional stratified sampling in 2008. We used a long-term care (LTC) assessment tool developed by the government, which consists of 52 items with 5 subscales. RESULTS: Statistically significant differences were detected in functional status and the level of health care needs among the subjects in different health care institutions. Of the sample population in geriatric hospitals, 48.9% were eligible for LTC Category 1, 20.9% for Category 2, and 17.2% for Category 3, and 13.1% of the subjects were not eligible for any of the categories. Of the sample population in health care facilities, 29.9% were eligible for LTC Category 1, 20.5% for Category 2, and 21.8% for Category 3, and 27.0% of the subjects were not eligible for any of the categories. CONCLUSION: The findings of this study indicate the necessity of the development of an evaluation system that helps determine whether a subject is eligible for LTC.

8.Helicobacter pylori Urease Induces Mouse Death.

The component of Helicobacter pylori responsible for mouse death was identified and partially characterized. Mice that were injected with H. pylori cell lysate showed pathological changes such as decreased activity, diarrhea, mild convulsion, dramatic decline of body temperature, and even death. In order to identify the lethal factor, recently isolated H. pylori strain 335 and old culture (for 10 years) of H. pylori strain 51 were used. LD50 of the cell lysate of H. pylori 335 and 51 were 338 microgram and 985 microgram, respectively, meaning the long passage of H. pylori in the laboratory might have decreased the lethal activity in the lysate. Mouse lethal activity disappeared by either treatment of cell lysate with proteinase K or heating cell lysate at 60 degrees C for 30min. Mutation analysis and genetic complementation study revealed that active urease of H. pylori is the mouse lethal factor. The recombinant H. pylori urease expressed in Escherichia coli showed si milar lethal activity.

9.A Case of Extraskeletal Myxoid Chondrosarcoma of Pelvic cavity.

Chondrosarcoma is an extremely rare cause of pelvic mass mimicking ovarian carcinoma. Imaging techniques, even CT and MRI, failed to define the exact origin of the tumor. These findings as well as the elevated CA-125 level naturally resulted in a preoperative diagnosis of ovarian carcinoma. Surgical removal is the most important treatment of choice for chondrosarcoma. We experienced a case of extraskeletal myxoid chondrosarcoma of pelvic cavity in a 45-year-old patient, who presented with abdominal distension and lower abdominal palpable mass, and reported it with a brief review of literatures.

Tracheal agenesis with bronchoesophageal fistula should be suspected in any newborn who is cyanotic at birth, has an absent cry, and cannot be intubated but shows some improvement when ventilated by bag and mask. It is important to make the diagnosis as soon as possible because esophageal intubation or a distal tracheostomy may be the only method to provide an airway while other measures and/or diagnostic confirmation are being sought. We experienced a case of complete tracheal agenesis with carinoesophageal fistula, cloacal anomalies in a male neonate.
