2.Effect of UVB Phototherapy on Pruritus of Pityriasis Rosea.

Pityriasis rosea is sometimes accompanied with considerable pruritus. Ultraviolet B irradiation is reported to be beneficial in relieving pruritus. The purpose was to evaluate the effect of UVB phototherapy on severe pruritus of pityriasis rosea. Nine patients with severe pruritus had UVB irradiation three times a week Five patients became symptom free and three showed a marked improvement. One dropped out. UVB phototherapy could be an effective modality for relieving pruritus in pityriasis rosea.

5.Immediate Internal Fixation of Open Fractures of the Long Bones

Between May, 1984 and October, 1986, twenty patients were treated at Han-Ra Hospital of JejuCity and Chung-Ang University Hospital of Seoul with immediate internal fixation with or without bone graft of open fractures of the long bones within three days. Fractured bones were ten tibiae, six femurs, three humeri and one radius and ulna. The injuries were classified on the basis of extent of soft tissue injury as defined by Gustilo and Anderson and consisted of four TypeI injuries, five Type II injuries and eleven Type III injuries. All wounds were closed primarily and radiological follow up was qhecked every month till union occurred. The complications included two deep infections, one non-union and one amputation. This study demonstrates that immediate internal fixation with bone graft when necessary is a beneficial method of treatment of open fractures of the long bones if copious irrigation, adequate debridement and skillful internal fixation technique are given golden period after injury. The results were related to the severity of the initial soft tissue injury and surgical technique and the period of union was not related to the individual bone. Autogenous bone grafting at the time of immediate internal fixation in comminuted or segmental fractures in which interfragmental compression cannot be obtained is recommended.

6.Treatment of Post-traumatic Scapho-lunate Dissociation

We have analyzed 27 cases(38.8%) of scapho-lunate(SL) dissociation which were the most com- mon type of total 72 cases of post-traumatic wrist instabilities since 1985. As a result, SL dissociations associated with wrist fracture were 23 cases, more common than simple dynamic SL dissociation(4 cases). Early diagnosis and treatment influenced on their prognosis, and both wrist PA roentgenograms were useful for the confirmative diagnosis of SL dissociation. In 16 cases with early diagnosis, closed reduction, 2 or 3 K-wires fixation under C-arm image and short arm cast for 8 weeks were effective. But in 11 cases with neglected diagnosis and with wrist fractures showed poor results. Excessive wrist ligamentous reconstruction in chronic cases should be avoided because of osteoporosis and wrist stiffness. Conclusively, clinical wrist symptoms are more important than radiological changes in wrist insta- bilites. So if the symptoms are not so severe, we must consider the patient's age, occupation and their social activities etc. to decide the proper way of treatment.

8.Risk Factors of Cerebral Infarction in Patients with Atrial Fibrillation.

BACKGROUND: It is well known that atrial fibrillation is common cardiac arrythmia in old age and poses a definitive risk factor of cerebral infarction. Therefore, effective treatment of atrial fibrillation is very important in the prevention of cerebral infarction. However, oral anticoagulant medication for the prevention of embolic ischemic stroke may be dangerous due to cerebral hemorrhage side effects. METHODS: This is a controlled case study designed to identify the risk factors in a large numbers of stroke and atrial fibrillation patients and to assess those patients with atrial fibrillation as a high risk group for cerebral infarction. All patient discharged from Chonnam National University Hospital were identified over a 42 month period who met our case standards of atrial fibrillation and ischemic stroke (n=62), and compared them with the control groups who were discharged with atrial fibrillation without stroke(n=68). We excluded the atrial fibrillation due to valvular heart disease, ischemic heart disease and congestive heart failure during the selection of subjects. RESULTS: Subjects and the controls were characteristically similar with common past medical histories of diabets, smoking, and hyperlipidemia. Sex, familial history, left ventricular hypertrophy in 2D-echocardiogram were also similar in both groups, however differences did exist. Subjects were significantly older than controls(68.9 : 63.9, p<0.001) and more likely to have a history of hypertension(56.5% : 23.5%, p<0.001) and left atrial enlargement(>40mm)(52.6% : 29.0%, p<0.001). Each of these 3 factors were assinged a measure of 1 point as a risk score, ischemic embolic stroke was found in 4 out of 28 patients(14.3%) with a risk score of 0, in 16 out of 39 patients(41.0%) with a risk score of 1, in 29 out of 41 patients(70.7%) with a risk score of 2, in 8 out of 11 patients(72.7%) with a risk score of 3. CONCLUSIONS: Based on the above results, a subject having more than two risk factors should be regarded as a high risk group for cerebral infarction and the long term anticoagulant therapy for the prevention of stroke may also be necessary even though some complications are present.
