4.Supramalleolar Stepcut Osteotomy for Tibial Deformity in Vitamin D

In treatment of Vitamin D-resistant rickets, the authors supplemented supramalleolar stepcut osteotomy for complex deformity of tibia consisted of varus, anterior bowing and intemal torsional supramalleolar level which accompanied severe waddling gait. The results of 10 tibia operations from 5 patients were generally satisfactory in terms of appearance and function.

5.Teh Effect of Local anesthetic on Degeneration and Regeneration of the Experimentally Traumatized Striated Muscle

The repair of striated muscle after acute trauma occurs simultaneously by the regeneration of disrupted muscle and formation of connective scar. But, there had been no attention concerning effects of local anesthetics on the degeneration and regeneration process of the injured muscle. The author induced a constant contusion injury on the calf of rats. Lidocaine and/or epinephrine was intramuscularly injected at the site of injury. Morphological and histochemical observation was conducted and the results were as follows: 1. Traumatized striated muscle showed evidences of the healing process for upto 90 days. 2. Morphological and histochemical changes in the traumatized muscle became more severe after the injection of lidocaine and/or epinephrine. 3. Formation of collagenous scar tissue significantly increased after injection of lidocaine and/or epinephrine. Injection of lidocaine or epinephrine developed similar amount of scar after 15 days. 4. Cross section area of the regenerated muscle fibers decreased in the injection groups than in the control groups. Atrophy was significant on day 90 in all types of fiber after injection of the mixture of lidocaine and epinephrine, and in type B after injection of epinephrine. In conclusion, injection of the lidocaine and epinephrine inhibited regeneration of the traumatized striated muscle. Mixture of the drugs showed more inhibitory effect than a single drug.

6.Mass-Forming Immunoglobulin G4-Related Ampullitis Mimicking a Duodenal Subepithelial Tumor: A Case Report

Subepithelial tumors (SETs) are often found accidentally during gastroscopy, most commonly in the stomach. If a raised lesion is observed around the major papilla, the relationship with the common bile duct or pancreas should be evaluated, and not only SET but also cancer or autoimmune disease should be considered in the differential diagnosis. Herein, we present a case of prominent major papilla mimicking a SET that was accidentally discovered during endoscopy. The patient was asymptomatic, and the physical and laboratory findings were normal. The biopsy findings were nonspecific and imaging findings suggested possible malignancy; thus, surgical ampullectomy was performed. The histopathological results confirmed IgG4-related ampullitis. This case suggests that immunoglobulin G4 (IgG4)-related disease may manifest with atypical findings, hindering its diagnosis and treatment.

8.Quantitative Evaluation of Dysphagia Using Scintigraphy.

PURPOSE: To evaluate dysphagia objectively and quantitatively, and ta clarify the effect of neck position and viscosity changes in patients with aspiration and laryngeal penetration. MATERIALS AND METHODS: We studied 35 patients with dysphagia and 21 normal controls using videofluoroscopy and scintigraphy. Videofluoroscopy was performed with barium with three different viscosity, and scintigraphy was done, with water, yogurt, and steamed egg mixed with Tc-99m tin colloid. 1f aspiration was found during videofluoroscopic examination, patients neck position was changed and study repeated. Videofluoroscopy was analyzed qualitatively. We calculated 7 quantitative parameters from scintigraphy. According to the videofluoroscopic findings, we divided patients into 3 subgroups; aspiration, laryngeal penetration, and no-aspiration group. RESULTS: The result of videofluoroscopy revealed that the most cornmon finding was the delay in triggering pharyngeal swallow. Pharyrigeal transit time (PTT) and pharyngeal swallowing efficiency(PSE) in patients with aspiration were significantly different from other groups. After neck position change, aspiration could be reduced in all of 7 patients, and laryngeal penetration reduced hy about 82%. PTT ancl PSE were also improved after position change. Aspiration and laryngeal penetration occurred more frequently in thin liquid swallowing than in thick liquid and solid swallowing. CONCLUSION: PTT and PSE were useful for the evaluation of dysphagia. Aspiration and laryngeal penetration could be reduced when appropriate position assumed, We could decrease the chance of aspiration by changing the patient diet consisIency. Scintigraphy might be useful tool to quantitate and follow up these changes.

10.The Usefulness of Cytokeratin 7 and Colon Ovarian Tumor Antigen in the Differential Diagnosis of Primary and Metastatic Ovarian Tumors.

Cytokeratin 7 has been known to be present in various types of human epithelial cells including the ovarian neoplasms, but not in colon cancers. The antibody to colon ovarian tumor antigen (COTA) has been introduced as a marker of colon and ovarian tumors. The aim of this study was to evaluate the usefulness of cytokeratin 7 and COTA in the differential diagnosis between ovarian primary and metastatic tumors. Nineteen primary ovarian epithelial tumors, seven metastatic carcinomas of the ovary from the stomach, three metastatic carcinomas of the ovary from the colon, one mucinous tumor of the ovary associated with a mucinous tumor of the appendix and pseudomyxoma peritonei, and nineteen colonic and twenty gastric adenocarcinomas were stained with monoclonal antibodies to cytokeratin 7 and COTA. The results are summerized as follows; In the primary ovarian tumors, 94.4% were positive for cytokeratin 7 and 50% were positive for COTA. In the primary colonic adenocarcinomas, 94.7% were negative for cytokeratin 7 and 68% were positive for COTA. In the metastatic ovarian tumor from the colonic adenocarcinomas, 100% were negative for cytokeratin 7 and positive for COTA. In the primary gastric adenocarcinomas, 40% were negative for cytokeratin 7 and 85% were negative for COTA. In the metastatic ovarian tumor from the gastric adenocarcinomas, 43% were negative for cytokeratin 7 and 14% were negative for COTA. From the results of this study, it could be concluded that in the differential diagnosis of primary ovarian tumors from metastatic colonic carcinomas, positive reaction for cytokeratin 7 suggests a primary ovarian tumor but a negative reaction for cytokeratin 7 and positive reaction for COTA suggest metastatic colonic carcinomas. The results of this study also reveal that cytokeratin 7 and COTA are not useful in the differential diagnosis of primary ovarian tumors from metastatic gastric carcinomas.
