1.Reflux Gastritis after Gastrectomy.

We studied prospectively on the grade of severity of reflux gastritis after Billroth- I (15 patients and B-II (66 patients) using gastrofiberscopy mostly 3~4 montsh after operations from December 1988 to February.The grade of severity af gastritis was arbitarily defined as follows; Grade 0-almost no reflux of bile and no redness on gastric mucosa. Grade 1-mild redness of the mucosa limited within an inch from anastomotic line. Giade 2-Edema and mucosal redness involves almost one half of remaining gastric mucosa. Grade 3-the above cbanges involves most of remnant gastric mucosa. Gtade 4-in addition to grade 3, friability of mucosa and/or, erosions is noted. Grade 5- ulceraitions of mucosa in addition to the above changes. Using the above defined criteria, we obtained the following results; 1) there was no rieflux gastritis in one patient in B- I group and remainders have varing grades of reflux gastritis, ie; 93% (14/15) (see Table 2). 2) In B- II reconstructed patients, grade 0 was 2 patients and remainder had reflux gastrits, ie; 96% (64/66) (see table 4). 3) Grade 5 patients have ulcerations in the esophagus and grade 2 changes in the stomach.

2.A Novel Fluoroscopic View for Positioning the AO Clavicle Hook Plate Decreases Its Associated in situ Complications.

BACKGROUND: The goal of this study was to evaluate whether a modified fluoroscopic technique for positioning a hook plate affected the clinical results of treating Neer type II distal clavicle fractures and Rockwood type V acromioclavicular (AC) joint separations with this device. METHODS: The study was a retrospective consecutive case series with data analysis. Sixty-four patients with a Neer type II distal clavicle fracture or a Rockwood type V AC joint injury treated between March 2009 and June 2013 were divided into 2 groups: traditional fluoroscopic technique (traditional view, 31 patients) or modified fluoroscopic technique ('hook' view, 33 patients). A visual analogue scale (VAS) score, the modified University of California-Los Angeles (UCLA) shoulder scale score, and radiographic osteolysis were the main outcome measures. RESULTS: The traditional group included a significantly larger number of patients with acromial osteolysis than the hook view group: 23 patients (74.2%) vs. 11 patients (33.3%), respectively (p=0.01). Before plate removal, the hook group reported less pain and higher UCLA shoulder scale scores than the traditional group: average VAS score, 1.55 vs. 2.26, respectively; average UCLA score, 30.88 vs. 27.06, respectively. However, there was no significant difference after plate removal. CONCLUSIONS: The hook view allows more accurate bending of the hook plate around the contour of the acromion, resulting in decreased osteolysis, decreased pain, and better function with the plate in situ.

3.Sex- and age group-specific associations between intakes of dairy foods and pulses and bone health in Koreans aged 50 years and older: Based on 2008~2011 Korea National Health and Nutrition Examination Survey.

PURPOSE: This study was performed to examine associations of intakes of milk and dairy products, pulses, and soy foods with bone health in Koreans aged 50 yr and older. METHODS: A total of 3,201 men and 3,581 women aged 50 yr and older who participated in the 2008~2011 Korea National Health and Nutrition Examination Survey were grouped by sex and age groups of 50~64 yr and 65 yr and older. Subjects within each sex and age group were divided into three bone health groups: normal, osteopenia, and osteoporosis groups based on bone mineral density. Intakes of nutrients and foods derived from 24-hour recall data were compared among three bone health groups. Associations between intake frequencies of foods, including milk, yogurt, tofu, or soy milk, and osteoporosis risk were evaluated based on confounding risk factor-adjusted logistic regression. RESULTS: Calcium intake was in the order of normal, osteopenia, and osteoporosis in men (p < 0.01) and women (p < 0.05) aged 50~64 yr as well as in men aged 65 yr and older (p < 0.001). In women aged 50~64 yr, intake of milk and dairy products was lower in the osteoporosis group (p < 0.01) as compared with the osteopenia group. Intake of pulses or tofu was not significantly different among bone health groups. Odds ratio (OR) for milk intake frequency (≥ 2 times/week) compared to intake frequency less than 1 time/month was 0.45 (95% CI 0.24~0.85, p for trend = 0.022) in men aged 65 yr and older. The OR for yogurt intake frequency (1 time/month~1 time/week) was 0.47 (95% CI 0.30~0.73, p for trend = 0.019) in women aged 50~64 yr. Intake frequency of tofu or soy milk was not associated with reduced risk of osteoporosis in all groups. CONCLUSION: Dairy food intake was significantly associated with bone health, and its effect was sex- and age group-specific, whereas soy food intake was not. Dietary intervention to prevent osteoporosis would be effective for women aged 50~64 yr old and for men aged 65 yr and older.

4.Cutaneous asthenia (Ehlers-Danlos syndrome) in a Korean short-haired cat.

A 1-year-old Korean domestic short-haired cat presented with skin hyperextensibility and a severely macerated wound on the skin of the dorsal part of the neck. Diagnostic studies including histopathology and skin extensibility index revealed congenital cutaneous asthenia (Ehlers-Danlos syndrome). In this cat, the skin wounds and defects were successfully managed with standard wound management and cosmetic surgery. Although skin hyperextensibility is persistent, the cat has lived well without other complications to date. To the best of our knowledge, this is the first report of cutaneous asthenia in a cat in Korea.

5.A Clinical Study on Coenzyme Q10(Neuquinon(R)) in the Treatment of Congestive Heart Failure.

Coenzyme Q is concentrated in Golgi apparatus membranes and mitochondria, but not in other membranes. Although it is difficult to prove the metabolic action of coenzyme Q administered exogenously in clinical cases, the effect of this substance can be evaluated by criteria based on clinical findings. In an attempt to evaluate the effect of coenzyme Q for the treatment of 67 patients(male 26 cases, female 41 cases) of congestive heart failure, we administered Coenzyme Q1030mg daily for 4 to 8 weeks. Most of them were valvular heart disease(74.6%) and hypertension (14.9%). Clinical effects were evaluated at least 4 weeks later by the criteria using a scoring method of severity of congestive heart failure which was devised by Ishiyama, etc. In summary, a definite effect was found in 13 cases(19%) and a mild effect was observed in 46 cases(69%). During treatment there were no significant side effects, and also no significant changes in heart rate and blood pressure.

6.Nutrient Intakes and Obesity-Related Factors of Obese Children and the Effect of Nutrition Education Program.

This study was conducted to investigate nutrient intakes and obesity-related factors of obese children by interviewing the subjects aged from 11 to 13 in Daegu. The collected data were consisted of items on general characteristics, dietary behavior, nutrition knowledge and daily nutrient intakes of subjects. The subjects were classified into obese and non-obese control groups according to their relative weights. Frequency of skipping breakfast and eating rate of obese group were significantly higher than those of control group. There was no significant difference between obese and control group in the nutrition knowledge score. Except vitamins B1, B2 and iron, the average daily intakes of other nutrients in obese were greater than control group. The most contributing factor to BMI turned out to be cholesterol intake. After the nutrition education targeting obese children, their nutrition knowledge scores improved, but the dietary behavior score was not significantly changed. Therefore, childhood obesity may be prevented by continuous education programs including the behavior modification of obese children.

9.6 Cases of Expanded PTFE Graft after Abdominal Wall Wide Excision.

PURPOSE: Extensive involvement of the abdominal wall or the diaphragm by benign or malignant tumor usually has precluded wide excision. The major reason has been the lack of a satisfactory replacement for the abdominal wall or the diaphragm. When skin and subcutaneous tissues can be preserved, it is possible to restore the abdominal wall or diaphragm by using expanded polytetrafluoroethylene (GORE-TEX soft tissue patch). METHODS: From August 1993 to November 1999, we performed 6 reconstructions of the large abdominal wall or diaphragmatic defects using ePTFE following wide en bloc resection of the abdominal wall or diaphragm. RESULTS: The patients included 4 cases requiring a reconstruction of the abdominal wall. These consisted of one transverse colon cancer invading into the upper abdominal wall, a multiple teratoma recurring in the lower part of the abdominal wall, a recurrent cervix cancer inn the median line of the lower part of the abdominal wall, and an enormous desmoid tumor appearing in the right lower part of the abdominal wall. They were 22, 22, 8 and 4 months respectively after the surgical treatment. Diseases requiring extensive resection of the diaphragm included a case of hepatoma encroaching into the diaphragm and another case of a huge adrenal pheochromocytoma appearing in the right part of the diaphragm. They passed 8 and 4 months respectively following surgical treatment. There were no cases of wound infection or abdominal wall hernia, although seroma occurred in 2 cases (50%). CONCLUSION: In all 6 cases, a good result was achieved by restoring excised tissue using ePTFE graft after wide excision of tumors involving the abdominal wall or the diaphragm. Therefore, it may be possible to use this technique in cases of active excision of tumors that involve an extensive part of the abdominal wall or the diaphragm.
