Cubitus varus is a complex three-dimentsional deformity. In addition to deformity on varus-valgus plane, internal torsion of distal humerus is frequently combined. Also limitation of flexion-extension could be associated in some patients. Surgical correction of the cubitus varus after skeletal maturity, could have some different clinical course from those of growing children. However there have been few papers dealing with the surgical treatment of cubitus varus in adults. From May 1988 to December 1993, 23 patients who were older than 17 years of age, had correc- tive osteotomy for cubitus varus considering its three planes of the deformity at Seoul National University Hospital. To prevent serpentine deformity medial displacement of distal fragment was also combined during operation. The avereage age was 26 years(17-47), and the average follow-up after the operation was 1 year and 6 months(6 months-3 years). The presumed diagnosis of initial injury deduced from history and preoperative radiograms, was supracondylar fracture in 19 cases(86.4%). Others were lateral condylar fracture of humerus in two, and fractures associated with elbow dislocation in two. The average age at the time of initial injury was 8 years(2-14), and average delay from occurrence of deformity to the operation, were 13 years. OBJECTIVES: On the basis of the relationship between positron emission tomography and symptom profiles in schizophrenia by Liddle et al, the authors attempted to investigate the related brain regions associated with clinical symptoms by studying the correlations between the performance of neuro-psychological tests likely to reflect functioning of dorsolateral preftontal, orbitofiontal or cingulate, parietal, and temporal cortices and 3 dimensions (psychotic or reality distortion, negative, and disorganization) of symptoms. METHODS: 41 subjects with a confirmed diagnosis of schizophrenia were scored for each of the three dimensions by Positive and Negative Syndrome Scale. Subjects performed 12 neuropsychological tests designed to measure impairment in specific areas of the brain. RESULTS: According to partial co-rrelations to remove possible confounding variables, the neuropsychological correlates of psychotic(reality distortion) and disorganization dimensions were some tests considered to be related to dorsolateral prefrontal and parietal lobes, and cingulate and dorsolateral preliontal cortices, respectively. CONCLUSION: The results support a part of hypotheses, a specific relation between disorganization and cingulate cortex. In addition our results suggest the possibile relations between a psychotic dimension and functions of dorsolateral prefrontal and parietal lobes, and between a disorganization one and functions of cingulate and dorsolateral prefrontal cortices. The authors believe that our study supports different neural circuits associated with each of dimensions of symptoms, particularly psychotic and disorganization, in schizophrenia.

Thirty patient with essential hypertension were administered Manidipine, a new calcium antagonist, 10~20mg once daily to evaluate the hypotensive efficacy and safety for 8 weeks. And the followings were the result. 1) Patients were consists of 14 male, 16 female, aged 53 in average and classified as mild in 21 and moderate in 9 patients. 2) Optimum intial dose was 10mg and 10 to 20mg were the doses recommended. 3) Blood pressure dropped after 8 weeks 24/13mmHg in average, rewarding 80% effectiveness and normalized in 87%. 4) Most frequent side reaction was facial flushing in 5 patiens followed by palpitation and dizziness, all of which did not disturb the continuation of medication. 5) Most of routine laboratory parameter were normal and unchanged between before and after the trial. 6) Overall rating of usefulness was 77%. In conclusion, Manidipine 10 to 20mg once daily regimen is well tolerated and effective in the treatment of mild to moderate essential hypertension.

There has been a few articles dealing with the incidence of congenital anomalies of the hand in Korea. In this paper, we have analyzed patients who have visited the hand clinic of Seoul National University Children s Hospital from March 1993 to February 1995 to study the relative incidence of congenital anomalies of the hand. We regarded more than two visits of the same patients as one. They were categorized into two groups by our classification system. First group consisted of the patients who showed hand anomaly only. The other group consisted of the patients who had anomalies in the other sites as well as in the hand. The first group was divided into two subgroups; the patients who had single entity of hand anomaly, and those who had more than two entities of hand anomalies. From the clinical analysis based on above criteria, following results were obtained. There were 626 anomalies in 547 patients, 319 (58.3%) males and 228 (41.7%) females (1.4: 1). Unilateral involvements of hand anomaly were noted in 385 (70.3%) patients (229 in right, 156 in left) and bilateral in 162 (29.7%). As a whole, the most common anomaly was trigger thumb (16.5%) and the second was polydactyly (16.1%). Other anomalies, in order of frequency, were syndactyly, camptodactyly, hypoplasia, brachydactyly, cleft hand and so on. In the first group who had hand anomaly only, there were 407 patients who had single entity of hand anomaly. Among these, trigger thumb were 25.3%, and polydactyly 16.2%. There were 68 patients who had multiple entities of anomalies. Among these, the syndactyly was the most common anomaly (23.9%) and the polydactyly was the next (16.2%). Fifty eight (10.6%) patients had anomalies in the hand as well as in the other site. Among these, camptodactyly was the most commonly found (36.5%) and polydactyly was the next common variety (19.0%) in the hand. In the anomalies which occurred in other than hand, arthrogryposis multiplex congenita was the most common (30.0%). Syndactyly of the foot (22.9%) and polydactyly of the foot (18.6%) were the next common anomalies.

The Psychopathology of schizophrenia is composed of many heterogeneous symptom complex, and multifaceted approaches have been done on this subject. It is so simplified a method that the complex symptoms of schizophrenia are classified into only positive and negative symptoms. Preceded studies of other researchers said cognitive deficit of the schizophrenics played important roles in the symptom formation. The purpose of this study is the evaluation of the relationship between the symptoms of schizophrenia and the memory function and low level of cognitive function. In this study, 30 subacute and chronic schizophrenic patients were included. PANSS(Positive and Negative Symptom Scale) was applied to evaluate the clinical symptoms in each subject. The memory function and low level cognitive function were evaluated using WMS-R(Wechsler Memory Scale-revised version) and Decision-reaction timer. The results showed negative correlations between cognitive factor of PANSS(5 factor model) and delayed recall of WMS-R. These results suggest the possibility that symptom evaluation using PANSS can be a preliminary estimation of cognitive function in schizophrenia.

BACKGROUND: As people have been interested in healt,h and as the microcomputers with multimedia steeing have been supplied more and more, there is increasing need of multimedia information about health. The CD-ROM, which is a representative multimedia tool, is capable of interacting between information-provider and users, offering audiovisual interface, and saving great deal of knowledge. METHODS: We developed the health-information system to provide ordinary people with hea1th information using multimedia tools. It, has 7 parts ; self-diagnosis, exploring human body, drug information, hospital information, first-aid treatment, Q and A, and medical common sense. We got image data such as figures, photos, X-rays, electrocardiograms and pathologic specimens with scanner and film scanner, also moving image(video) with video-capturing program. RESULTS & CONCLUSIONS: We hope the multimedia health-information system to become a useful methodology of patient education and to be used as an information source in medical information network in the near future.

Twenty four hyperlipidemic patients, consist of 13 males and 11 females, were administration a new hypolipidemic agents, Pantethine, and blood lipid level were checked sereally for 16 weeks. The following results are obtained. 1. The serum high density lipoprotein-cholesterol are markedly increased by 11.8%. 2. The serum cholesterol level are reduced mildly by 4.8% & it seems to be insignificant. 3. The serum triglyceride level are moderately reduced by 10.7%. 4. In 11 female patients, pantethine are more than effect on male patients. 5. Pantethine has been well tolerated in most patients. With a consideration of remakable safety, it is promising that pantethine are effective in reducing cholesterol, Triglyceride and increasing high density lipoprotein-cholesterol level.

To assess the prognostic value of the 14 ECG variables obtained from the pre-discharge ECG, 35 patients were followed up for at least one year among the 80 patients of the acute myocardial infarction admitted at Seoul Paik Hospital from Sep. 1983 to Aug. 1986. The following results were obtained. 1) The overall in-hospital mortality rate was 20% and the mortality rate tended to decline year by year. 2) The one-year mortality and morbidity rate of the followed patients was 31.4%. 3) Among the 14 variables obtained from the pre-discharge ECG, T-negativity was the only statistically significant (p<0.05) one to predict the one-year prognosis in the patients after the first myocardial infarction, and we could not derive any meaningful datd from the analysis of the combined effects of the three ECG variables (PTF, ST depression and ST elevation).

The pilomatricoma is rare benign tumor of the hair follicle. It occurs as a solitary tumor usually on the face and upper extremities of children and young adults. It involves the periorbital region, the upper eyelids and the brow are involved most frequently. It appears as a firm, deep-seated subcutaneous nodule that is covered by normal or pink skin. It is not hereditary, there are a few instances of familial occurrence. The authors experienced a case of pilomatricoma of the right upper eyelid in a 20-year-old male and present with the review of literature.
