2.Cost-effectiveness Analysis of Home Care Services for Patients with Diabetic Foot.

PURPOSE: This study was a retrospective survey to examine economic feasibility of home care services for patients with diabetic foot. METHODS: The participants were 33 patients in the home care services (HC) group and 27 in the non-home care services (non-HC) group, all of whom were discharged early after inpatient treatment. Data were collected from medical records. Direct medical costs were calculated using medical fee payment data. Cost-effectiveness ratio was calculated using direct medical costs paid by the patient and the insurer until complete cure of the diabetic foot. Effectiveness was the time required for a complete cure. Direct medical costs included fees for hospitalization, emergency care, home care, ambulatory fees, and hospitalization or ambulatory fees at other medical institutions. RESULTS: Mean for direct medical costs was 11,118,773 won per person in the HC group, and 16,005,883 won in the non-HC group. The difference between the groups was statistically significant (p=.042). Analysis of the results for cost-effectiveness ratio showed 91,891 won per day in the HC patients, and 109,629 won per day in the non-HC patients. CONCLUSION: Result shows that the cost-effectiveness ratio is lower HC patients than non-HC patients, that indicates home care services are economically feasible.

3.Cystic Lymphangioma of Breast: A Case Report.

Cystic lymphangioma are rare benign tumors that originate as a congenital anomaly of the lymphatic system. Their common presentation is in neck and axillary area at the birth, during infancy or early pediatric age group. Author reports an unusual case of cystic lymphangioma of breast which was diagnosed in a 32-year-old woman. The findings were very dense, sharply defined, and multilobulated masses on mammography, and mu-Itiloculated echo free lesions on ultrasonogram.

6.The effect of third molar extraction on the periodontal status of the adjacent second molar.

The aim of this study was to investigate the effect of third molar extraction on the periodontal status of the adjacent second molar. A total of 61 second molars in 31 adult periodontitis patients were examined. Among them, 27 second molars without adjacent third molars were included in the test group, and 34 second molars with third molar were included in the control group. Clinical parameters including plaque index, gingival index, and pocket depth and radiographic bone loss were measured around the second molar both in test and control group. The result showed that: (1) the mean plaque index and gingival index of control group were higher than these of the test group but the difference was not statistically significant, (2) the mean pocket depth of the control group was higher than the test group significantly at distal and buccal surface, (3) radiographic bone loss was greater in control group than test group significantly, (4) in Pearson correlation analysis between the age of extraction and radiographic bone loss in the test group, a positive relationship was shown(p<0.01). Within limitation of this study, it may be concluded that third molar extraction in periodontitis patients showed an improvement in periodontal status in contrast the patients group having third molar, therefore earlier a removal of third molar may minimize radiographic bone loss of the adjacent second molar.

9.Effectiveness of an 'Integrated Medication Adherence Enhancement Program for Persons with Schizophrenia' by Using Online Education Contents.

OBJECTIVE: Schizophrenia seriously degrades the ability to function normally in those who suffer from this disease. The purpose of this study was to develop a comprehensive online medication adherence enhancement program and to test its effectiveness among schizophrenia patients who live in community relatively neglected with few inputs or oversights from community mental health centers. METHODS: A nonequivalent control group with prepost test was employed during the study. Participants included fifty-two chronic schizophrenia patients, twentysix experimental, and a control group of twenty-six. The study variables were medication knowledge, perceived benefits, self efficacy, social support and medication compliance. RESULTS: There were significant differences between two groups in perceived benefits (F=5.67, p=0.02), self-efficacy (F=6.38, p=0.01), social support (F=14.63, p<0.001) but no significant differences in medication knowledge (F=0.82, p=0.37), and medication compliance (F=0.89, p=0.35) when the program was completed. However, medication knowledge in posttest I and posttest II which was taken after 3 months of the program were significantly increased in the experimental group (F=8.88, p<0.001). Medication adherence in posttest II was significantly increased in the experimental group (F=6.10, p<0.001). CONCLUSION: The online medication adherence enhancement program for persons with schizophrenia was effective at increasing psychosocial variables such as perceived benefits, self efficacy, and social support. This program can be used for schizophrenic persons in community mental health centers, patients' homes, hospitals, and community centers.
